3 CITY OF MESA FIRE/MEDICAL DEPARTMENT LADDER COMPANY CERTIFICATION The City of Mesa Fire/Medical Department Ladder Company Certification program is designed to establish a minimum level of competency for those personnel assigned to ladder companies as well as roving members who may potentially be assigned to ladder companies. The Ladder Company Certification testing process will be administered by training personnel. Field evaluators will be ladder certified Captains and Engineers permanently assigned to ladder companies. All personnel assigned to a ladder company will have 6 months upon assignment to complete the Ladder Company Certification process. Personnel assigned to the leave pool (rovers) and members not permanently assigned to a ladder company will have 12 months to complete the Ladder Company Certification process. All applicants shall be required to obtain and/or show proof of department certified Engine Company DOC status prior to applying for Ladder Company Certification. Only members who have completed and obtained City of Mesa Fire/Medical Department Ladder Company Certification shall be authorized to operate department aerial apparatus in the emergency response mode. The steps listed below are required to complete the Ladder Company Certification: Step #1: Step #2: Step #3: Advise your Captain that you are applying for the Ladder Company Certification Program. Meet with your Captain and Engineer to review packet and coordinate. Begin your driver s training. Driver s training can include code 2 responses, highway driving, congested city traffic, driving in alleys, backing exercises, negotiating in parking lots of apartments and shopping centers, and randomly spotting for aerial operations at various structures. The purpose of driver training is to allow a driver the opportunity to become familiar with driving aerial apparatus in traffic and negotiating the apparatus around obstacles. Applicants must complete a minimum of 10 hours of driver s training. Step #4: Complete each section of the Ladder Certification packet. Applicants are required to be signed off by their respective Captains.

4 Step #5: Step #6: Step #7: Step #8: Step #8: Once you ve completed the entire packet, contact the Training Division to set up a test date. Review the completed materials in this packet and any reference materials associated, as well as the study guide prior to your test date. If necessary, contact the Training Division to set up a time to drive through the fixed obstacle/cone course prior to your test date. On your test date, return the completed packet to the Training Division. All sections must be checked off and signed by your Captain. Complete the Aerial Apparatus Driving Performance Test which covers both the road and the fixed obstacle course set up at the Training facility and surrounding areas. Both driving exams require a minimum score of 80% to pass. Step #10: Complete the multiple-choice exam administered by Training personnel. The exam will test an applicant s knowledge of department SOP s, aerial apparatus functions, driving techniques, and cover all material outlined in this packet (refer to the reference materials and study guide for review). The written exam requires a minimum score of 80% to pass. Step #11: Retest if necessary. Applicants may retake any portion of the test a maximum of 3 times. 8 NFPA SECTION REQUIREMENT Perform the routine tests and inspections specified in the following list, given a fire department aerial apparatus, so that the operational readiness of the aerial apparatus is verified: 1. Cable systems 2. Aerial device hydraulic systems 3. Stabilizing systems 4. Aerial device safety systems 5. Breathing air systems 6. Communications systems 7. Coolant system 8. Battery condition 9. Braking system 10. Fluid levels (B) The driver/operator shall demonstrate the ability to recognize system problems and how to report all problems found The driver/operator shall demonstrate the ability to maneuver and position an aerial apparatus at an incident so that the apparatus is positioned for correct aerial device deployment

9 NFPA SECTION (A) REQUIREMENT The driver /operator shall describe the capabilities and limitations of the aerial device related to reach, bucket load, angle of inclination, and angle from chassis axis. Driver/operator shall also describe the effects of topography, ground, and weather conditions on deployment and use of the aerial device (B) The driver/operator shall demonstrate the ability to determine a correct position for the apparatus, maneuver the apparatus to that position, and avoid obstacles to operations The driver/operator shall demonstrate the ability to stabilize the apparatus, given a positioned vehicle and manufacturer s recommendations, so that power can be transferred to the aerial device hydraulic system and the device can be deployed.

10 NFPA SECTION The driver/operator shall demonstrate the ability to maneuver and position the aerial device from each control station, given an incident location, a situation description, and an assignment, so that the aerial device is positioned to accomplish the assignment (A) The driver/operator shall demonstrate competence in the following areas: 1. Aerial device hydraulic systems 2. Hydraulic pressure relief systems 3. Gauges and controls 4. Cable systems 5. Communications systems 6. Electrical systems 7. Emergency operating systems 8. Locking systems 9. Manual rotation and lowering systems 10. Stabilizing systems 11. Aerial device safety systems 12. System overrides and the hazards of using overrides 13. Safe operational limitations of the given aerial device 14. Safety procedures specific to the aerial device 15. Operations near electrical hazards and overhead obstructions

11 NFPA SECTION (B) REQUIREMENT The driver/operator shall demonstrate the ability to raise, rotate, extend and position to a specified location, as well as lock, unlock, retract, lower and bed the aerial device The driver/operator shall demonstrate the ability to rotate, position to center, unlock, retract, lower and bed the aerial device using the emergency operating system The driver/operator shall demonstrate the ability to connect a water supply, deploy and operate an elevated master stream, given an aerial device, a master stream device, and a desired flow so that the stream is effective and the aerial and master stream are operating correctly (A) The driver/operator shall demonstrate understanding of nozzle reaction, range of operation, and weight limitations of the aerial device.

12 OSHKOSH ALL STEER SUPPLEMENTAL MATERIAL The following information is provided for those members taking part in the Ladder Company Certification process that are currently assigned to or may become assigned to a ladder company that has an All Steer aerial apparatus. It is the responsibility of the member to become familiar with this material and to complete the appropriate check-offs for the All Steer units. Refer to the Oshkosh All Steer Operator s Manual for this section. It is also highly recommended that members review the All Steer slide show that can be found on the Training Division s webpage. ALL STEER CHECK-OFFS The driver/operator shall demonstrate the proper procedure for selecting the three steering modes from the steering control panel. This includes Front Steer mode, Coordinated mode, and Fireground mode. The driver/operator shall demonstrate how to activate and deactivate All-Steer. The driver/operator shall demonstrate driving the apparatus in the following modes: Front Coordinated Fireground Crab

13 Section 3 - Building Construction Refer to the following reference materials for completion of this section: Truck Company Operations (Mittendorf) Chapter 3 Gilbert Fire Department Building Construction Handbook pgs Lightweight Truss Construction (Chief Crabtree) Mesa Fire Department SOP s Discuss with your Captain and crew Building Construction to include the following: Discuss with your Ladder Captain/crew the common terms used in the MFD system in reference to building construction, roof types and construction Review the types of construction common to buildings of different time periods. Review and discuss conventional construction including roofing framing, lightweight construction including lightweight truss construction, the six categories of construction methods including metal, concrete, masonry, stucco, wood frame, curtain, and fascias and parapets Review roof construction the six most common styles Demonstrate the ability to understand and communicate common orders and terminology expected on the fire ground in reference to ladder company operations (either on working fire or in training environment). Discuss with your Ladder Captain/crew the characteristics common to construction from each of the time periods and how that may affect your tactics for performing Ladder Company functions. Also discuss the hazards present with each time period and the danger they present to firefighters.

14 Discuss and demonstrate with your Ladder Captain/crew a thorough understanding of building construction including hazards present with each type and the danger they present to firefighters. Discuss and demonstrate with your Ladder Captain/crew a thorough understanding of roof construction.

15 Section 4 Ventilation Refer to the following reference materials for completion of this section: Mittendorf Chapters 14, 15, 16, 17 Mesa Fire SOP s D; , , , thru Powerpoint on Saw maintenance located in Target Solutions Videos on Ventilation Techniques located in Target Solutions Discuss with your Captain and crew the purposes and methods of Ventilation to include the following: Explain why ventilation is such an integral part of the control and extinguishment of structure fires Explain how properly timed and executed ventilation aids in all phases of firefighting tactics. Explain why vertical ventilation size-up is so important and list several items that should be noted in this size- up. Horizontal or cross ventilation is an alternate method for ventilating a structure. What does this method employ the use of and what initial factors should be considered when thinking about using horizontal ventilation? What precautions should be considered when using horizontal ventilation? What are several advantages gained through using positive pressure ventilation? Explain in detail how PPV can be used in a coordinated attack with interior lines. If fire is coming through the roof of a structure, is it considered ventilated? True or False, and explain Demonstrate the ability to break down and clean a saw after it has been used.

25 AERIAL APPARATUS DRIVING PERFORMANCE TEST FIXED OBSTACLE COURSE INSTRUCTIONS Instructions to Applicant: 1. Follow the fixed obstacle course on the handout. Read the directions for each specific evolution. 2. Applicants will only be given one attempt at each event. 3. Applicants are permitted to look out driver side window, but seat belt must remain on and snug at all times. 4. Overhead emergency lights must be activated during all backing operations. 5. Applicants must follow all instructions given by the evaluator(s). 6. Applicants will start with a total of 150 points. Points are deducted as applicants fail to meet cone course requirements.

26 All Steer Aerial Apparatus Fixed Obstacle Course

27 Applicants will begin the cone course at the northeast corner of the track starting with a Left Turn made in Coordinated Mode. Ten foot PVC pylons will be placed 36 inches away from the orange cones to simulate a wall, and place emphasis on the bucket swing of the apparatus. The Straight Line evolution will be performed using a continuous motion while travelling both forward and in reverse. Apparatus bumper shall not cross the vertical plane of the cones at the end of the evolution. Overhead emergency lights must be activated during all backing operations.

28 The Serpentine evolution must be completed using a constant speed (not to exceed 10 mph) in Fireground Mode. After completing the serpentine evolution, applicants will place the apparatus back into Coordinated Mode and proceed to the next evolution. The Alley Dock must be performed in Coordinated Mode without breaking the vertical plane of the three sides of the alley. When entering and exiting the alley the applicant may cross the vertical plane of the cones to the west with the bucket only. These cones shall serve as a guide to create a starting boundary point for the purpose of testing. Overhead lights must be activated during all backing operations.

29 Applicants will demonstrate the ability to effectively maneuver the apparatus through tight corners by making a Right Turn using Fireground Mode. Ten foot PVC pylons will be placed 36 inches away from the orange cones on two sides of the corner to simulate walls, covered parking, or other overhead obstructions. This evolution places emphasis on the simultaneous bucket and tail swing of the apparatus. Applicants must maintain constant acceleration (minimum 15 mph) while driving through the Diminishing Clearance evolution.

30 Applicants will proceed back to the alley dock and park the apparatus on the right side of the cones pulling as close to the cones as possible, and stopping at the opening to the alley. Applicant will then place the apparatus into Crab mode, and move to the left at an angle toward the opposing cones. Applicants will then stop, put the apparatus back in Coordinated Mode and advance to the finish line.

LEFT TURN IN COORDINATED MODE Function: Applicant performs a left turn in Coordinated Mode without hitting the 10-foot PVC pylons with the bucket. STRAIGHT LINE EVOLUTION Function: Applicant completes the evolution using a continuous motion while moving the apparatus both forward and in reverse. Overhead emergency lights must be activated during all backing operations. 32 3.) SERPENTINE EVOLUTION Function: Applicant drives between the cones at a constant speed no greater than 10 mph while in Fireground Mode. A. -1 point for each cone hit B. -5 points for each cone knocked over C. -5 points for not maintaining continuous motion C. -5 points for exceeding speed of 10 mph D. -5 points for not putting the apparatus in Fireground Mode E. -5 points for not returning the apparatus to Coordinated Mode TOTAL POINTS DEDUCTED Comments: 4.) ALLEY DOCK EVOLUTION Function: Applicant moves the apparatus past the alley to start the evolution. Backing the vehicle, applicant makes a turn in reverse into the defined area and stops. Only the apparatus bucket may cross the plane of the west cones. Overhead emergency lights must be activated during all backing operations. A. -1 point for each cone hit B. -5 points for each cone knocked over C. -5 points for not using overhead lights while backing TOTAL POINTS DEDUCTED Comments:

33 5.) RIGHT TURN IN FIREGROUND MODE Function: Applicant performs a right turn in Fireground Mode without hitting the 10-foot PVC pylons with the bucket or the tail of the apparatus. A. -1 point for each cone hit B. -5 points for each cone knocked over C. -5 points for each pylon hit D. -5 points for not putting the apparatus in Fireground Mode E. -5 points for not returning the apparatus to Coordinated Mode TOTAL POINTS DEDUCTED Comments: 6.) DIMINISHING CLEARANCE EVOLUTION Function: Applicant moves the apparatus through the cones while the apparatus is under constant acceleration. 36 AERIAL APPARATUS DRIVING PERFORMANCE TEST ROAD COURSE INSTRUCTIONS The purpose of the Road Course is to evaluate driving skills in normal traffic conditions. Points will be deducted for: - Unsafe practices noted by the evaluator(s) - Violation of Arizona State traffic laws - Improper start-up / shut-down procedures - Rough operations of the apparatus - Failure to follow verbal commands Instructions to Applicant: 1. Applicants will be required to follow all instructions written and/or verbally given by the evaluator(s). 2. ALL actions taken by the applicant MUST BE VERBALIZED to the evaluator(s) during the entire road course evaluation process. 3. The evaluator(s) will choose the road course and provide direction to the applicant throughout the process. 4. Due to AZDOT traffic laws, seat belt and red light violations shall be deemed as failing criteria and thus will be graded on a Pass/Fail basis. Any failure in these areas shall constitute an immediate stoppage of the Road Course portion of the Driving Performance Test. 5. Applicants will start with a total of 100 points. Points are deducted as violations occur and/or applicants fail to meet the Road Course requirements.
