2 Title: Barricades and Falling Object Protection Plans Freeport Site Function: Ecology & Standard No.: BCF Page: 2 of PURPOSE The purpose of this standard is to safely deter or stop people from entering a potentially hazardous work environment and preventing tools, materials, equipment, objects, etc., from falling from elevated surfaces. 2.0 DEFINITIONS 2.1 Falling Object Prevention Plan (FOPP) Every job that is 6 (six feet) above ground level will have a falling objects prevention plan that shall identify methods to prevent tools and materials from falling to lower elevations. Methods must include a positive means of object control at the place of work in addition to a second method. 2.2 Floor Opening an opening measuring twelve (12) inches or more in its least dimensions, in any floor, platform, pavement, or yard through which a person may fall 2.3 Job Analysis (JSA) A job safety analysis documents the hazards and the safeguards that are in place for a particular and specific job. This documentation is filled out and communicated to workers before the job begins. This documentation shall be unique to that job and be posted with the work permit at the job site. 2.4 Red Tape red barricade tape that is used for DANGER KEEP OUT of a temporary or potential hazard; examples include scaffold build and demo, rig-lifted hanging zones, chemical leaks, open holes, water blasting, and any danger to personnel safety 2.5 Road Barricades signs, swing/drop arms, breakaway rope, chain, or cable that are highly visible 2.6 Yellow/Magenta Tape This color scheme identifies X-ray activities. 2.7 Yellow Tape yellow barricade tape that is used for WARNING of a temporary or potential hazard; examples include welding, head knockers, slip/trip hazards, overhead work, and overhead load swing paths that cannot be adequately controlled by spotters/flagmen

3 Freeport Site Function: Ecology & Standard No.: BCF Page: 3 of SCOPE The procedure applies to all the operating units and construction sites on the Freeport site. 4.0 PROCEDURE BARRICADE 4.1 Barricade materials can include but are not limited to red/yellow/magenta tape, scaffolding, and mesh fencing. Barricades should enclose the entire work perimeter. All barricades will have warning signs/tags identifying the hazard and instructions for safety if entry is required. Remove all barricades as soon as the hazard is eliminated. 4.2 Barricades are restricted to the work crew and supervision addressing the hazard. This area may be entered only with approval from operating unit or the applicable work crew(s). One may enter yellow warning barricades as long as the individual is aware of the potential hazard and can assure his/her own safety. Red danger barricades are restricted to the work crew and supervision addressing the hazard. This area may be entered only with approval from the operating unit or the applicable work crew. 4.3 In situations in which work crews and/or different contractors are using the same barricade: Each work group must place their tag to ensure that the barricade is not removed before all of the work is completed. Each work group must communicate to the other crews that they are using the barricade, and when their work is done, they will remove their tags. As each crew completes their tasks, they must notify the remaining work groups that they are finished and remove their tags. When the last crew has completed their work and no other tags are on the barricade, the last work crew can remove the barricade. 4.4 Temporary hazards below grade such as holes, trenches, sumps, and openings in any floor, platform, pavement, or yard (manholes, ditch grating, drill shafts, etc.) through which persons may fall shall immediately have floor covers, barricades, or protective railings installed. If floor covers, barricades, or portions of the protective railings are removed, someone must constantly attend the area.

4 Freeport Site Function: Ecology & Standard No.: BCF Page: 4 of 7 NOTE: Auger cast piles are not considered open holes when filled with wet/fresh cement. 4.5 Persons working inside a barricaded area must have a personal fall arrest system with continuous attachment where there is a danger of falling a distance of six (6) feet (1.8 m.) or greater. Drill shafts of 18 inches or greater must have protective railings installed, or persons within six (6) feet of the working area must be attached to a personal fall arrest system. Covers shall be capable of supporting, without failure, the weight of the employee, equipment, and materials that may be imposed on the cover at any one time. Covers shall be secured when installed to prevent accidental displacement. All covers except cast iron manhole covers or steel grates used on streets or roadways must be marked with the words HOLE COVER to provide warning of the hazard. Personnel must be protected by a personal fall arrest system before removing covers. Floor openings in poorly lit traffic or walking areas shall be provided with warning lights at night. NOTE: Hinges, handles, bolts, or other parts on floor covers shall be set flush with the floor or cover surface. 4.6 Avoid entry into a drilled shaft. However, entry may be required for certain work activities or rescue purposes. No one is to enter a drilled shaft unless one is following all applicable requirements of BCF : Permitted Confined Space. These requirements include: A rescue plan and fall protection system are in place, AND The person is trained on air quality monitoring, fall protection, PPE, communication, and entry methods, AND A trained, topside observer is in place, AND The shaft itself is properly cased to prevent cave-in. 4.7 If removing permanent structure hand or guardrails becomes necessary, do not leave them unattended unless barriers are erected, or the area is roped off to keep personnel five (5) feet from the edge that the guard was protecting.

Personnel working in the area must follow applicable fall protection requirements. 4.8 Road barricades shall be highly visible and erected not to create a hazard themselves. Ropes, chains, etc., used for road barricades shall be easily breakable and visible. The recommended height should be between 36 and 46 inches. 4.9 Implement effective means to prevent unauthorized entry into permitted confined spaces. These measures may include: entry attendant at the entry point permanent closure and sealing use of physical barriers (shall be accompanied with signs) bolting and locking the space Training personnel shall supplement the above measures When performing high-pressure water blasting, use a barrier shield that is able to withstand the pressure you are using. This shield will deflect the spray if spray could become a hazard to personnel beyond the barricaded area You must use red barricade tape: at rig-lifted work zones where equipment, piping, material, etc., is being hung around water blasting areas around scaffold building and demolition 4.12 Use yellow barricade tape as a warning for potential hazards. Use it in accordance with the falling object prevention plan (FOPP) Use yellow/magenta barricade tape for X-ray activities. The radiography crew shall use yellow/magenta tape to barricade the area. No one shall enter the area unless the radiography crew authorizes entry.

6 Freeport Site Function: Ecology & Standard No.: BCF Page: 6 of PROCEDURES FALLING OBJECT PREVENTION PLAN (FOPP) 5.1 For those jobs in which work shall be performed at or above six feet (6 ), you must create a FOPP. Include the FOPP in the contractor s JSAs, and capture it in the falling hazard section of the safe work permit. 5.2 Document the FOPP in the fall hazard assessment section of the SWP if you are a BASF employee. 5.3 FOPP methods must ensure that objects do not fall to lower levels. The FOPP must employ at least two (2) of the following methods: tarps or plastic on grating netting around the work zone solid deck scaffold netting to the top rail on sold deck scaffold solid wood platform 100% tool tethers NOTE: After employing two (2) of these methods, you must place yellow barricade tape around the work area with signage noting the hazard of the work being performed. 5.4 If work is occurring on elevated grating, use plastic, tarps, or other suitable material in good condition to prevent items from falling through the grating. The covering should be 100% solid to a 2:1 ratio and maintained to prevent tripping hazards. Example: If working at a height of six (6) feet, covering on the grating should cover a twelve-foot radius. If the grating ends within the radius of the work, use netting to the top rail or the elevated deck. 5.5 Cover any openings in scaffolds or decks around pipes or other equipment to prevent tools or material from falling to lower levels. 6.0 RESPONSIBILITIES 6.1 Each unit is responsible for ensuring that all hazards in their areas are adequately barricaded and noted on the safe work permit. 6.2 The person who identifies a hazard or potential hazard is responsible for seeing that the hazard is properly barricaded.
