1 Guidelines Milan Special School District Automated External Defibrillator (AED) Program Purpose: To provide management or administration of the MSSD school-based AED program. Sudden cardiac arrest (SCA) is a condition that occurs when the electrical impulses of the human heart malfunction causing a disturbance in the heart s electrical rhythm called ventricular fibrillation (VF). This erratic and ineffective electrical heart rhythm causes complete cessation of the heart s normal function of pumping blood resulting in sudden death. The most effective treatment for this condition is the administration of an electrical current to the heart by a defibrillator, delivered within a short time of the onset of VF. An AED is used to treat victims who experience SCA. It is only to be applied to victims who are unconscious and without pulse, signs of circulation and normal breathing. The AED will analyze the heart rhythm and advise the operator if a shockable rhythm is detected. If a shockable rhythm is detected, the AED will charge to the appropriate energy level and advise the operator to deliver a shock. Program Coordinator: Melinda Holtzclaw, RN, BSN, School Nurse Responsibilities: Selection of employees for AED training and distribution of AED-trained employee lists as required Coordination of training for emergency responders Coordinating equipment and accessory maintenance Maintenance of a file containing a specifications/technical information sheet for each approved AED model obtained by MSSD. Revision of this procedure as required Monitoring of the effectiveness of the program Communication with the medical director on issues related to the medical emergency response program including post-event reviews Applicable documents: State AED Guidelines Medical Emergency Action Plan and AED Procedure MSSD policy relating to universal precautions (5.402) Page 1 of 6

2 911 Checklist Post Use Procedure Post Use Reporting Form AED Checklist Medical Control: The medical advisor of the AED program is. The medical advisor has ongoing responsibility for: Providing medical direction for use of AEDs Writing a prescription for AEDs Reviewing and approving guidelines for emergency procedures related to use of AEDs and CPR Evaluation of post use review forms and digital files downloaded from the AED Authorized AED users: Employees who have successfully completed an approved CPR/AED training program Any trained volunteer responder who has successfully completed an approved CPR/AED training program AED-Trained Employee Responsibilities: Activating internal emergency response system and providing prompt basic life support including AED and first aid according to training and experience Understanding and complying with requirements of this policy Following the more detailed procedures and guidelines for the AED program Volunteer Responder Responsibilities: Anyone can, at their discretion, provide voluntary assistance to victims of medical emergencies. The extent to which these individuals respond shall be appropriate to their training and experience. These responders are encouraged to contribute to emergency response only to the extent they are comfortable. The emergency medical response of these individuals my include CPR, AED or medical first aid. School Office Responsibilities: Receiving emergency medical calls from internal locations Page 2 of 6

3 Using an established 911 Checklist to assess emergency and determine appropriate level of response Deploying AED-trained employees to emergency location Assigning someone to meet responding EMS aid vehicle and direct EMS personnel to site of medical emergency Equipment All (AEDs) have been approved for this program. The AED and resuscitation kit will be brought to all medical emergencies. The AED can be used on any person who is at least 8 years of age and displays ALL the symptoms of cardiac arrest. The AED will be placed only after the following symptoms are confirmed: Victim is unresponsive Victim is not breathing, or is breathing ineffectively Victim has no signs of circulation The AED housed at Milan Elementary School will also be equipped with a set of child defibrillation pads which will allow use on children from ages 1 year to 8 years. Location of AEDs During school hours, the AEDs will be at designated locations. These locations shall be specific to each school but should allow the device to be easily seen by staff. The locations should allow staff members to retrieve the device outside of normal school hours. After school hours, the AED may be moved from its designated location by an AED-trained coach or athletic trainer to support athletic department activities. A visible sign must be left in the place of the AED, with a name and phone number to clearly indicate who has possession of the AED. One resuscitation kit will be attached to each AED. This kit will contain two pairs of latex-free gloves, one razor, one set of trauma shears, one absorbent towel, 1 biohazard bag, and one facemask barrier device. Equipment Maintenance: All equipment and accessories necessary for support of medical emergency response shall be maintained in a state of readiness. Specific maintenance requirements include: The main school office where each AED is housed shall be informed of changes in availability of emergency medical response equipment. If equipment is withdrawn from service, the office shall be informed and then notified when equipment is returned to service. Each office shall be responsible for informing response teams of changes to availability of emergency medical equipment. The AED Program Coordinator or designee shall be responsible for having regular equipment maintenance performed. All maintenance tasks shall be performed according to equipment maintenance procedures as outlined in the operating instruction manual. Following use of emergency response equipment, the AED shall be prepared for continued use by following the Post Use Procedure. The equipment shall be cleaned and/or decontaminated as required. If contamination included body fluids, the equipment shall be disinfected according to the above procedure. Routine Maintenance: The AED performs daily, weekly, monthly and quarterly self-diagnostic tests automatically to check the integrity of the unit s hardware and software. Page 4 of 6

5 A volunteer, assigned by the AED Program Coordinator or designee, will perform an AED check each day that school is in session, following the AED Checklist posted with the AED. The AED Checklist will be initialed at the completion of the daily check. If the Active Status Indicator (ASI) is not flashing green, contact the AED Program Coordinator or designee immediately. If the unit, electrode pads, or resuscitation kit appear damaged or are missing, contact the AED Program Coordinator or designee immediately. The AED Program Coordinator or designee shall make monthly checks of all AEDs and additional resuscitation equipment. Training: Trained Employees: Must complete training adequate to provide basic first-aid, CPR and AED that will be provided on site or by any state approved agency. AED training must be a course approved by the state and must include child training for those who work at MES. Each employee s training must be kept current, according to the guidelines set by the agency through which training was obtained. Trained employees will also be trained in universal precautions against blood borne pathogens. The trained employees shall be offered hepatitis B vaccination free of charge. The Program Coordinator shall maintain training records for the trained employees. Volunteer Responders: These responders will possess various amounts of training in emergency medical response, and their training may be provided by a variety of sources. Volunteer responders may assist in emergencies but must only participate to the extent allowed by their training and experience. Any volunteer wishing to use one of the AEDs deployed on any campus should have successfully completed a state approved AED course including CPR within the last two years. Each volunteer s training must be kept current according to the guidelines set by the agency through which training was obtained. The school will not maintain training records for the volunteer responders. Medical Response Documentation: Internal Post-Use Documentation: It is important to document each use of the medical emergency response system. The following form shall be sent to the AED Program Coordinator or designee within 24 hours of a medical event: Page 5 of 6

6 The AED-trained employee or volunteer responder shall complete a Post Use Reporting Form whenever an AED is used. External Post-Use Documentation: A copy of AED use information, including any recorded data and all electronic files captured by the AED, shall be presented within 48 hours of the emergency to the following: Medical director of the AED program Local EMS Post Use Review: Following each use of an AED by either a trained employee or a volunteer responder, a review shall be conducted to learn from the experience. The AED Program Coordinator or designee shall conduct and document the post use review. All key participants in the event shall participate in the review. Included in the review shall be the identification of actions that went well and the collection of opportunities for improvement as well as critical incident stress debriefing. A summary of the post use review shall be sent to the MSSD safety officer, who will keep a copy of the post use review summary. System Verification and Review: The medical emergency response system is ultimately successful if necessary medical assistance is provided to victims in a timely and safe manner. Annual System Assessment: Once each calendar year, the AED Program Coordinator or designee shall conduct and document a system readiness review. This review shall include review of the following elements: Training records Equipment operation and maintenance records Approvals Function Printed Name Signature Date Medical Director Program Coordinator Page 6 of 6
