Swimmers are required to attend 60% of their dual meets (6/10). Families who fail to fulfill these requirements will not be guaranteed a spot on next year s summer team. SECTION 3- FEES (per swimmer) Please check the website for your group placement. GROUP 1-$216 GROUP 2-$284 GROUP 3-$310 In addition to team fees, A FULL PRIVILEGE SCYMCA MEMBERSHIP IS REQUIRED TO PARTCIPATE, as well as a Meet Entry Fee account ($100 PER FAMILY). If your YMCA membership has expired, you will be required to update the membership at the time of registration. Meet Entry Fee accounts are for all non-dual meets. In lieu of fundraising through concessions, we will also be requiring a $10 Booster Fee PER FAMILY to fund picnic & coaches gifts. Please send 2 separate checks payable to SCYMCA, one for $100 for the meet entry fees and another for $10 for the booster fee. Don t forget to write in the MEMO section either Meet Fees or Booster Fees. THESE FEES CANNOT BE PAID WITH A CREDIT CARD. SECTION 4: PARENTAL WAIVER AND CONSENT As the parent or legal guardian of the child or children named above and/or below, I hereby give my full consent and approval for my child to participate as a team member of the sport designated below. I understand that there are certain risks of injury inherent in the practice and play of this sport, as well as in traveling and other related activities incidental to my child s participation, and I am willing to assume these risks on behalf of my child. I hereby certify that my child is fully capable of participating in the designated sport and that my child is healthy and has no physical or mental infirmities that would restrict full participation in these activities, except as listed above. In addition to giving my full consent for my child's participation, I do hereby waive and hold harmless the organization named below, its officers, coaches, sponsors, supervisors and representatives for any injury that may be suffered by my child in the normal course of participation in the designated sport and the activities incidental thereto, whether the result of negligence or any other cause. 2 2015 SVY SHORT COURSE SUMMER LEAGUE SWIM TEAM REGISTRATION FORM FOR ALL RETURNING 2014 HURRICANE AND BLUE DOLPHIN SWIMMERS SECTION 5: MEMBERSHIP INFORMATION MUST be completed if swimmer(s) has/have individual draft membership(s) All Swimmers MUST have a Full Privilege SCYMCA Membership at the time of registration, as well as throughout the entire season, to maintain their eligibility to train and compete in our program. In the event my swimmer(s) has/have an individual draft membership, I authorize SCYMCA to update the type of membership based on their age (youth-up to 12 years old, $22 a month, teen-13 to 18 year olds, $30 a month). I understand this will increase the monthly amount drafted for his/her membership. Refunds When you register for a summer short course team, this reserves a space for your child on the team and we staff according to the registrations. If you decide that you would like to leave the summer short course team before the season is over, we do require written notification and will adhere to the following schedule: Cancellation between Registration and May 24, 2015 Y keeps 25% of the Team Fees Cancellation between May 26, 2015 June 21, 2015 Y keeps 50% of the Team Fees Cancellation after June 21, Y keeps all of the Team Fees Medical refunds will be considered in case of illness or injury, will require a doctor s verification and will be considered on a case by case basis. 3 2015 SVY SHORT COURSE SUMMER LEAGUE SWIM TEAM CODE OF CHARACTER BRIDGEWATER BLUE DOLPHINS/HILLSBOROUGH HURRICANES I understand that it is an honor and a privilege to represent the Somerset Valley YMCA Swim Team. I recognize that the reputation of the YMCA and our team is affected by and dependent upon my conduct and behavior. Therefore, I agree at all times to conduct myself in a mature, courteous, and responsible manner, both at SVY and when attending functions away from our YMCA. Swimmers will not be allowed to register for the team and may not be allowed to practice or compete if this form is not signed and returned to the Head Coach. As an athlete on the Bridgewater Blue Dolphins/Hillsborough Hurricanes swim team I agree to the following standards: *I will follow all of the rules, regulations, and procedures set forth by the team and the coaches. *I will always be encouraging & supportive of my teammates *I will practice/compete to the best of my ability at all times. *I will not go through, take, or destroy, anyone s personal belongings. *I will never endanger the safety of another participant. *I will never leave the supervision of the Y staff without their direct approval. I further acknowledge that the following conduct is not acceptable and will NOT be tolerated. *Possession or use of alcohol, illegal drugs, or tobacco products *Physically assaulting a teammate, coach, official, or fellow competitor in any manner *Use of profanity or inappropriate language *Inappropriate or unsupervised activities between participants *Refer to teammates in an inappropriate or unsporting manner I understand that failure to abide by the SVY Code of Character may result in immediate disciplinary action including, but not limited to the following: *Suspension from practice(s), event(s), or meet(s) *Suspension from the Bridgewater Blue Dolphins/Hillsborough Hurricanes Swim Team *Dismissal from the Bridgewater Blue Dolphins/Hillsborough Hurricanes Swim Team *Any combination of the above ATTENDANCE - Any swimmer that has a dual meet attendance percentage of less than 60% for the season will not be guaranteed a spot on the team for the next season. These swimmers will need to try-out for a spot on the team the following season. I understand that decisions made by the SVY/Bridgewater Blue Dolphins/Hillsborough Hurricanes coaching staff at the site of any infraction will prevail. Appeals can be made at a later date to be heard in a rational, constructive, and fair manner. I pledge to abide by the SVY Code of Character. 4 SVY SPRING/SUMMER 2015 SWIM TEAM In the spring/summer months SVY offers 2 competitive opportunities for our athletes to participate in. Each has it merits, but both are clearly different in focus and intensity. The Short Course Team The Bridgewater Blue Dolphins (Head Coach Andrea Marumoto) and Hillsborough Hurricanes (Head Coach Elizabeth Stoddard) are both very traditional competitive swimming setting (25 yard pool), not unlike other summer club teams, high school teams, or even to some extent our own SVY winter team. The focus of this team is to get as many swimmers in the water practicing regularly and having meets on a very consistent basis. Competition is set in a very relaxed and more level playing field. This is a great way to start a YMCA Club swimming career or to simply maintain strength and focus over the summer months to prepare for winter swimming (High School or Club). Most swimmers that try-out for the team for the first time will be directed to this program. As the short course team does not start until the end of May we also suggest that the swimmer signs-up for any of our "stroke" or "spring" clinics in March/April/May & August (space is very limited). Swimmers that make it onto the summer team for the first time must try-out again in August if they wish to be accepted onto the winter SVY team. Practice begins after Memorial Day. Practice and Meet schedules can be found on our websites: (Hillsborough Hurricanes) and Bridgewater Blue Dolphins, The Long Course Team is based on the International / Olympic rules of swimming and is set in a 50 meter pool. The focus with this team is based almost completely around training. A swimmer might go to 3 meets the entire 3 month season. There are very limited meets to attend and swimmers must achieve qualifying standards to be considered eligible for these meets. The competition at these meets is very elite and it is not recommended for first time club swimmers. Only swimmers with a strong background in this sport are likely to be accepted onto this team. 6 The following applies to both Hillsborough Hurricanes and Bridgewater Blue Dolphins AGE FOR GROUP SIGN-UP IS AGE AS OF 6/30/15. A $50.00 late fee will be assessed for ALL registrations received after 3/27/15, and your spot on the team is not guaranteed. *In addition to team fees, a FULL PRIVILEGE MEMBERSHIP IS REQUIRED FOR THE ENTIRE SEASON TO PARTICIPATE. YOU WILL BE REQUIRED TO SET UP MEMBERSHIP AT THE TIME OF REGISTRATION. **A Meet Entry Fee account is required for all swimmers ($100 PER FAMILY) at registration. The summer invitational meets and the league championship meet require an additional fee, which will be deducted from the Meet Fee account. Summer team group placement, although mostly determined by age, will be given final approval by the Head Coach, before new members can register. A $10 Booster Fee PER FAMILY to fund picnic & coaches gifts is also required at registration. They cannot be paid with a credit card. *** Meets are held most every Wednesday and Saturday throughout the season both at our home pool and other site in central New Jersey. ****Summer League Swimmers that qualify and register for this program are expected to participate in the full season.
