4 How to Register Choose the method that s most convenient for you! Fall 2012 / Winter 2013 Registration Begins: Aquatic August 13 at 10pm Other Programs August 15 at 10pm 1 2 Online Direct your browser to ottawa.ca/recreationguide and click on the Register icon to view programs and to sign up for a course. Please have your credit card number and expiry date ready (Visa/Mastercard/Amex). You will need your Family PIN number and Client Barcode(s).* Touch Tone Telephone At Please have your credit card number and expiry date ready (Visa/Mastercard/Amex). For courses without a program code, register at the facility which offers the program. 5 How to Register Registration Terms and Conditions At the time of registering, the City of Ottawa expects full payment before any services are provided. Payment is accepted by cheque, credit card, debit card or cash. Cheques are made payable to the City of Ottawa. Cancellations Courses are subject to cancellation due to insufficient registration. If this situation occurs, a full refund will be issued. In the event that the Parks, Recreation and Cultural Services department must cancel a class or lesson, every attempt will be made to have the class or lesson re-scheduled. Refunds Refunds will be credited to your account unless otherwise requested. No processing fee will be charged. If a refund is requested, a Refund Request Form must be completed and submitted to City Staff for processing. A Processing Fee of $25 will apply to all refunds, whether by cheque or credit card and will be processed within 21 days of the receipt of the Refund Request Form. If more than 50% of the course/membership has elapsed, no refund or credit on account will be offered. There are no refunds or credits for private lessons. For admission and goods purchases, refunds will be processed at the Facility with proof of purchase. In the event that the City must cancel a program or registration, a full refund will be provided. Should a class or lesson be cancelled, every attempt will be made to re-schedule. Waiting Lists If the course you are interested in is full, you have the option of making another course selection or being put on a waiting list for the course of your choice (however, there is no guarantee that a space will become available). Participants will be contacted only if a space becomes available. Returned Cheque / Declined Credit Card Fee A service fee will be levied after two cumulative declined credit card payments and on all cheques returned as Non Sufficient Funds (NSF); Stop Payment; or Funds Not Cleared. Seniors Discount City of Ottawa residents who are 65 years of age and over will receive a discount of 10% when registering for a City program. Harmonized Sales Tax (HST) The Harmonized Sales Tax (HST) of 13% applies to recreation programs, services and materials where applicable and is included in the fees. Existing programs that were GST exempt remain HST exempt for example, qualifying recreation programs for children 14 and younger and individuals with disabilities. For more information, visit the Government of Ontario website: Non-Resident Fee Non-residents will be charged an additional 25% per person per program/membership. Clients with medical conditions Users of City facilities are personally responsible for ensuring they are fit to participate in physical activities. Anyone with a condition affecting his/her ability to participate must take appropriate safety precautions to ensure his/her own safety and is encouraged to be accompanied by an adult who is knowledgeable about their condition. Early Renewal 10% Discount Choose City of Ottawa recreations complexes for your fitness membership and receive a 10% discount when you renew your membership prior to its expiry date. It s our way of saying thank you! Medication Administration If the participant requires that City of Ottawa staff be responsible for storing, administering or supervising the administration of any medication, the participant or parent/guardian of the participant will be required to sign a release waiver at the facility where the program is being offered. Please contact the facility to obtain additional information. Severe Allergies It is the responsibility of the participant or parent/guardian of the participant to identify themselves or their child(ren) if they have a severe allergy and require an auto-injector. The City of Ottawa attempts to ensure the safety and well being of all participants with "allergensafe zones and practices". Participants or parents/guardians of participants will be required to provide two autoinjectors and sign a release waiver at the facility where the program is offered. Special Needs Participants or parents/guardians of participants are asked to provide information regarding special needs requirements before the start of the program. Please contact the staff at the facility where the program is being offered or the Special Needs Portfolio Manager at , ext for assistance. Ottawa Hand in Hand - Recreation and Culture Fee Support The City of Ottawa offers a system of financial support to ensure that all City residents can participate in recreation and culture programs and activities offered by the City, regardless of their ability to pay. For additional information or to obtain a fee assistance application, residents are invited to visit their local recreation facility or any City of Ottawa Client Service Centre. 8 Memberships Corporate Fitness Memberships We offer a 20% reduction on our 1 year all-inclusive memberships when the following criteria can be met: 1. A minimum of 10 people are required to purchase a membership. 2. There are no holds or extensions on Corporate Memberships. Senior, Student and Youth rates do apply. 3. Memberships must be purchased at the Bob MacQuarrie R.C.- Orléans, but can be used at any city facility. 9 General Information Swimming Swimming Admittance and Supervision Requirements In order to ensure adequate supervision and to identify children at greater risk, the City of Ottawa is now issuing wristbands during public swim sessions. All participants require a wristband to access public swimming. Ask facility staff for more information. Parent/guardian must be in proper bathing attire, in the water, actively participating within arms' reach at all times. High-risk participants must be in a ratio of 1:1.High-risk participants are defined as those who have a condition or illness that may put them at risk in an aquatic environment (e.g. frequent seizures, fainting conditions, etc) and those who are unable to control behavior or impulses and require direct supervision. 9

10 General information - Swimming Learn to Swim Red Cross Swim Program The City of Ottawa s goal is to offer you and your children the highest possible quality of swimming lessons. Preschool Parented 4 months to 36 months Preschool 3 to 5 years Level Assessment Unsure of what level to register for? Just drop in to any of our Leisure Swims and ask the Program Supervisor to assess you or your child. They can tell you exactly which swimming level will best suit your abilities. Low Ratio Classes Low ratio classes are regular programs with approximately half the participants of our regular classes. These programs are more expensive but allow more individual attention to the participants. Parents Day On the 6th lesson, parents are welcome to join their child s swimming class to find out how they are progressing. You can either join the class by participating in the water or simply watch the class from the pool deck. During this time the instructor will be able to provide you with detailed skill requirements for each level and indicate to you what your child needs to work on. 10 Starfish 4-12 months Babies and their caregivers work on getting wet, buoyancy and movement, front, back and vertical position in the water, and shallow water entries and exits. Duck months Babies and their caregivers work on rhythmic breathing, moving forward and backward, front and back float with recovery, and shallow water entries and exits. Sea Turtle months Toddlers and their caregivers work on submersion, front and back floats and glides, jumping into chest-deep water with assistance, kicking on front and front swim. Sea Otter Swimmers work on front and back floats and glides, kicking on front with a buoyant object, and will be able to swim 1 metre upon completion of this level. Salamander Swimmers work on improving their front and back float and jumping into chestdeep water unassisted. Kicking is added to the front and back glide and swimmers will be able to swim 2 metres upon completion. Sunfish Swimmers work on kicking and can perform glides (front and back), enter deep water safely, float in deep water, swim with a PFD, and swim 5 metres continuously. Crocodile Swimmers further develop the front and back glide with kick, front and back swim, jump into deep water, swim with a PFD in deep water, and swim 10 metres continuously. Whale Swimmers increase their distance on front and back glide with kick, increase distance on front and back swim, learn the sitting dive, and will be able to swim 15 metres continuously upon completion of the level.

11 Children - 6 to 12 years Swim Kids level 1 Swimmers develop front and back float, front and back glide, flutter kick (assisted), shallow water entries and exits, and swim 5 metres. Swim Kids level 2 Swimmers learn front and back glide with flutter kick, develop front swim, complete deep-water activities (assisted) and swim 10 metres continuously. Swim Kids level 3 Swimmers increase their distance on front and back glide with flutter kick, perform flutter kick unassisted, wear a PFD in deep water, float in deep water, and swim 15 metres continuously. Swim Kids level 4 Swimmers learn back swim with shoulder roll and front crawl (10m), work on flutter kick on back, perform kneeling dive, introduction to sculling and swim 25 metres continuously. General Information - Swimming Swim Kids level 8 Swimmers increase their distance on front and back crawl (75m) and elementary back stroke (25m), learn breast stroke (15m), learn eggbeater/tread water, perform feet-first surface dive and standing shallow dive, and swim 300 metres continuously Swim Kids level 9 Swimmers increase their distance on front and back crawl (100m), elementary back stroke (50m) and breast stroke (25m), learn sidestroke kick, perform head-first surface dive, and swim 400 metres continuously. Swim Kids level 10 Swimmers increase their distance on front and back crawl (100m), elementary back stroke (50m) and breast stroke (50m), learn sidestroke (25m), perform dolphin kick (vertical), learn feet- and head-first surface dives with underwater swim, and swim 500 metres continuously. Adults years Swim Basic 1 Swimmers learn front and back float, front and back glide with kick, front swim for 10 metres, shallow water entries and exits, and deep-water activities (assisted). Swim Stroke Swimmers choose the stroke(s) they wish to focus on. Participation-based program with individual goals. Swim Kids level 5 Swimmers develop front crawl (15m), learn back crawl (15m) and whip kick on back, learn stride dive and treading water, perform head-first sculling on back, and swim 50 metres continuously. Swim Kids level 6 Swimmers increase their distance on front and back crawl (25m), learn elementary back stroke (15m), learn dolphin kick, tread water in deep water, perform front dive, and swim 75 metres continuously. Swim Kids level 7 Swimmers increase their distance on front and back crawl (50m) and elementary back stroke (25m), learn front scull, perform whip kick on front and dolphin kick, learn stride entry and swim 150 metres continuously. 11

This is a great introduction program into aquafitness, offering easy choreography and lots of options for intensity. Aqua-Crunch This program is a medium to high intensity muscle focused class. This program uses the water as the primary training tool for building muscular strength and endurance. Throughout the session all major muscle groups will be targeted, providing a total body workout experience. A strong emphasis will be placed on core stabilizations (abs work!) throughout the session. 12 Aqua-Turbo This program is a high intensity class! It focuses on building cardiovascular strength and endurance through challenging routines of intervals and higher intensity options. You will keep your heart rate and pace up throughout the class and learn to train in both aerobic and anaerobic zones. Provides a definite challenge for veterans and avid participants. Aqua-Lite This is a low intensity program designed to meet the needs of varied clients; retired adults, arthritis sufferers and individuals with non-specific back or joint difficulties. This program is a total body workout that is low impact with a balance of cardiovascular and muscular strength and endurance training with a strong emphasis on core stabilization and de-stressing the mind. Aqua-Specialties These aquatic fitness programs fall outside of our main menu of programs and therefore outside of the membership packages. These programs are offered as a registeredbased option as they are designed specially to meet a number of criteria. The criteria is as follows: These programs are designed to meet more individualized needs or goals within a described population. These programs require specialized development, training or certification of staff compliment and/or equipment. These programs require consistent and regular attendance for learning due to the progressive nature of the genre. Schedule is subject to change.

You may only put your membership on hold once during the course of your membership term. Your membership may be put on hold for no longer than half the term of the membership. Fitness Centre Etiquette Membership passes must be worn at all times while in the Fitness Centre. Please refrain from using perfume and scented products. The use of profanity will not be tolerated. Children under 12 years of age are not permitted in the Weight or Cardio Rooms. Youth aged must take an Orientation Session with a Fitness Staff prior to using the Fitness Centre. Proper attire, including footwear is required in the Weight and cardio Room. Cardio Room Etiquette Please clean the machines with a spray bottle and paper towels after each use. Please consult the Fitness Supervisor on duty for further assistance. Failure to comply with the above noted procedures could result in the loss of equipment use and/or the cancellation of membership. Weight Room Etiquette When finished with equipment, please return it to the proper rack. Please share equipment and allow other participants to make use of it in between your sets. Please do not sit on machines to rest or watch television. Please clean the machines with a spray bottle and paper towels after each use. Please refrain from dropping or throwing the weights. Request assistance from a spotter if you are unsure about the weight you are lifting. Please lift weights in a safe, controlled manner. This one-hour personal service will give you an understanding of cardiovascular and strength training principles with an individual program suited to your needs. Our certified personal trainers will assist you in reaching your fitness goals and help you maintain them. 1 session $ sessions $ sessions $ sessions $ Body Composition Assessment - $29.75 Because scales can be deceptive and machines that measure body composition are often inaccurate, we now offer a complete Body Composition Assessment. Our certified Fitness Consultants will record height, weight, measurements and skin-folds to give you an accurate picture of your body composition and will determine if you are in a healthy range or not. Personal Training Packages! Racquetball and Squash Learn the game from a certified coach and enjoy the benefits of playing, such as: improved cardiovascular fitness, agility, muscular conditioning and flexibility. Register at the reception desk and an instructor will contact you to make arrangements for your session(s). 1 session $ sessions $ sessions $ sessions $ Weight Loss & Conditioning Package - $459 This ten-week program (1 hr/wk) is designed to assist you in reaching your personal fitness goals, whether it be weight loss, increased strength, flexibility, cardiovascular health or sport specific training. Speak to a certified trainer about your goals and they will work with you to meet those goals. Each weekly one-hour session is scheduled with your certified personal trainer. For optimal success, participants are encouraged to complete two other exercise sessions per week on their own. A three-month all-inclusive membership is included. Fees may be subject to change. Fees Include HST. Nutritional Counseling Achieve optimal health through nutrition. Introduction This class emphasizes proper bike setup, optimal riding technique and is highly recommended for beginners. Interval This workout incorporates all types of riding flats, hills, jumps, etc.) and includes the full range of Energy Zone training. Interval + An interval-training program designed to develop maximal aerobic/anaerobic power and recommended for the more advanced rider. Race Day Race day simulates outdoor racing techniques and is designed for advanced riders to experience anaerobic threshold riding. Strength This class is slow, steady, hard resistance work with a goal of building power and strength. If you enjoy climbing hills this is the ultimate workout. 18 General Information - Community Programs Community Programs Park-ticipate The Park-ticipate program is a free, informal drop-in program offered at various local rural and urban parks throughout the Complexes, Fitness and Aquatic Venues Branch s East District. The Children s Fitness Tax Credit is a federal, non-refundable, personal tax credit that lets parents claim up to $500 per year for eligible fitness expenses paid for each child who is under 16 years of age at the beginning of the year in which the expenses are paid. If a child qualifies for the disability tax credit, parents can claim up to $500 per year in eligible fitness expenses paid for the child who is under 18 years of age at the beginning of the year. Also, if at least $100 in eligible fitness expenses has been paid for the child, an additional amount of $500 can be added to the eligible fitness expenses actually incurred. Fees paid by parents must relate to the cost of a facility membership or registration in an eligible physical activity program. Your receipt will indicate whether the program that your child is registered in is eligible (see receipt wording).
