1 MSU Swimming & Diving presents 16 th Annual John Munley Long Course Memorial Swim Meet Saturday, June 16, 2018 Recognized by the Michigan LMSC # Location: Michigan State University - IM West Outdoor 50 Meter Pool. Park in Lot 62W. Set GPS to 393 Chestnut Rd, Michigan State University, East Lansing. See attached map. Facility: This competition will be held in an outdoor pool. Bring lawn chairs if you wish. The Intramural Sports West Building adjacent to the pool closes at 6:00 pm. If building doors are locked when you arrive or leave, use the fence gate on the north side of the pool. Entries: Entry fee. The entry fee for advance entries is $30 per person; deck entries cost $35. All proceeds go to the John Munley Memorial Swimming Scholarship at MSU. Additional donations to the scholarship fund are welcome. Entry fees are not tax deductible, but additional donations are tax deductible. Checks are payable to the John Munley Memorial Swimming Scholarship. No refunds. Entries submitted by US Mail. Advance entries are preferred but not required. Send the completed entry form, signed waiver forms, photocopy of USMS membership card (if you are a member), and entry fee to Gail Dummer, 3100 Scarborough Road, Lansing, MI 48910, postmarked by Saturday, June 9, Deck entries. Deck entries will be accepted until 4:15 pm on meet day. Bring the completed entry form and waiver forms (or complete forms at the meet), show your USMS membership card (if you are a member), and pay your entry fee. Rules: US Masters Swimming rules ( apply. The length of the competition course without a bulkhead is in compliance and on file with USMS in accordance with USMS rules and The meet host will assign two timers to each lane; notify the starter to request a third timer if you are attempting national or Michigan records. According to USMS rule , if the meet cannot be held at the outdoor pool because of severe weather etc., events will be moved to the indoor pool and competed as short course yards. Registration: USMS recognized meets are open to participation by both USMS members and nonmembers. If desired, go to for membership/registration information. 2 Munley Scholarship The John Munley Memorial Swimming Scholarship About. The John Munley Memorial Scholarship is awarded annually to a male swimmer or diver who demonstrates an outstanding level of competitiveness, dedication to his sport, and loyalty to Michigan State University. He continuously strives to make the most of his abilities, vigorously pursues his goals and dreams through training, and competes in a way that sets high standards for himself and his teammates. He is a leader by example and demonstrates that hard work and dedication may lead to new levels of personal accomplishment. He represents the best of what athletics, sportsmanship, and Spartan Spirit are about at Michigan State University. Recipients. Scholarship recipients to date are 2002-Nick DeFauw, 2003 & 2004-Chad Wilson, 2005-Matt Wisniewski, 2006-Matt Torres, 2007-Michael VanBeek, 2008-Matt Massman, 2009 & 2010-Ricky Forrest Mark Schramm, 2012-Nick McGowan, 2013-Bryan Williams, 2014-Ian Rodriguez, 2015-Jon Burke, 2016-Mitch Strahlman, and 2017-Brennan Labar. Funding goal. $350,000 is needed to endow the Munley Scholarship. The fund now stands $207,282. The annual value of the scholarship is 5% of the current amount of the fund, now about $10,000. Proceeds from the Munley masters swimming meet add about $8,000 annually to the scholarship fund. Why Give? Pay it forward. If you received a scholarship or other financial assistance during your college years, you know the importance of such funding to a student's education. Help a student athlete! Support men's collegiate swimming. The demise of men's collegiate swimming programs around the country has been well documented. Do your part to strengthen the infrastructure of MSU's swimming and diving program by contributing to this scholarship. Show your love for MSU! About John Munley John Munley swam for Michigan State University from Not heavily recruited out of high school, John went on to be MSU's Most Valuable Swimmer for all four years. John served as team co-captain, qualified for 2000 Olympic Trials, and was first alternate for the 1998 World University Games. He finished his senior season with the varsity record in the 200-yard freestyle. John passed away while jogging on campus during the spring of his senior year.

5 PARTICIPANT WAIVER AND RELEASE OF LIABILITY, ASSUMPTION OF RISK AND INDEMNITY AGREEMENT For and in consideration of United States Masters Swimming, Inc. ( USMS ) allowing me, the undersigned, to participate in any USMS sanctioned or approved activity, including swimming camps, clinics, and exhibitions; learn-to-swim programs; swimming tryouts; fitness and training programs (including dryland training); swim practices and workouts (for both pool and open water); pool meets; open water competitions; local, regional, and national competitions and championships (both pool and open water); and related activities ( Event or Events ); I, for myself, and on behalf of my spouse, children, heirs and next of kin, and any legal and personal representatives, executors, administrators, successors, and assigns, hereby agree to and make the following contractual representations pursuant to this Waiver and Release of Liability, Assumption of Risk and Indemnity Agreement (the Agreement ); 1. I hereby certify and represent that (i) I am in good health and in proper physical condition to participate in the Events; and (ii) I have not been advised of any medical conditions that would impair my ability to safely participate in the Events. I agree that it is my sole responsibility to determine whether I am sufficiently fit and healthy enough to participate in the Events. 2. I acknowledge the inherent risks associated with the sport of swimming. I understand that my participation involves risks and dangers, which include, without limitation, the potential for serious bodily injury, sickness and disease, permanent disability, paralysis and death (from drowning or other causes); loss of or damage to personal property and equipment; exposure to extreme conditions and circumstances; accidents involving other participants, event staff, volunteers or spectators; contact or collision with natural or manmade objects; dangers arising from adverse weather conditions; imperfect water conditions; water and surface hazards; facility issues; equipment failure; inadequate safety measures; participants of varying skill levels; situations beyond the immediate control of the Event organizers; and other undefined, not readily foreseeable and presently unknown risks and dangers ( Risks ). I understand that these Risks may be caused in whole or in part by my own actions or inactions, the actions or inactions of others participating in the Events, or the negligent acts or omissions of the Released Parties defined below, and I hereby expressly assume all such Risks and responsibility for any damages, liabilities, losses or expenses that I incur as a result of my participation in any Events. 3. I agree to be familiar with and to abide by the Rules and Regulations established by USMS, including any safety regulations. I accept sole responsibility for my own conduct and actions while participating in the Events. 4. I hereby Release, Waive and Covenant Not to Sue, and further agree to Indemnify, Defend and Hold Harmless the following parties: USMS, its members, clubs, workout groups, event hosts, employees, and volunteers (including, but not limited to, event directors, coaches, officials, judges, timers, safety marshals, lifeguards, and support boat owners and operators); the USMS Swimming Saves Lives Foundation; USMS Local Masters Swimming Committees (LMSCs); the Event organizers and promoters, sponsors and advertisers; pool facility, lake and property owners or operators hosting the Events; law enforcement agencies and other public entities providing support for the Events; and each of their respective parent, subsidiary and affiliated companies, officers, directors, partners, shareholders, members, agents, employees, and volunteers (individually and collectively, the Released Parties ), with respect to any liability, claim(s), demand(s), cause(s) of action, damage(s), loss or expense (including court costs and reasonable attorneys fees) of any kind or nature ( Liability ) which may arise out of, result from, or relate in any way to my participation in the Events, including claims for Liability caused in whole or in part by the negligent acts or omissions of the Released Parties. 5. I further agree that if, despite this Agreement, I, or anyone on my behalf, makes a claim for Liability against any of the Released Parties, I will indemnify, defend and hold harmless each of the Released Parties from any such Liabilities which any may be incurred as the result of such claim. I hereby warrant that I am of legal age and competent to enter into this Agreement, that I have read this Agreement carefully, understand its terms and conditions, acknowledge that I will be giving up substantial legal rights by signing it (including the rights of my spouse, children, heirs and next of kin, and any legal and personal representatives, executors, administrators, successors, and assigns), acknowledge that I have signed this Agreement without any inducement, assurance, or guarantee, and intend for my signature to serve as confirmation of my complete and unconditional acceptance of the terms, conditions and provisions of this Agreement. This Agreement represents the complete understanding between the parties regarding these issues and no oral representations, statements, or inducements have been made apart from this Agreement. If any provision of this Agreement is held to be unlawful, void, or for any reason unenforceable, then that provision shall be deemed severable from this Agreement and shall not affect the validity and enforceability of any remaining provisions. Those risks include, but are not limited to: permanent disability or death from competition. I recognize that the above specifications are not complete and that my participation could lead to untoward consequences which are not anticipated. I understand that my participation is voluntary and I may withdraw at any time. I understand that participation may or may not actually benefit me. HEALTH AND SAFETY: I have been advised to consult with a medical doctor regarding any personal medical needs. There are no healthrelated reasons or concerns that preclude or restrict my participation, except as stated here In case of a medical emergency occurring during my participation, I authorize, in advance, the University to secure whatever treatment is deemed necessary. The University may (but is not obligated to) take any actions it considers to be warranted under the circumstances for my health and safety. I agree to pay all expenses for such medical treatment and I release the University from any liability. ASSUMPTION OF RISK AND RELEASE FOR LIABILITY: Knowing that my participation entails some risks, and in consideration of being permitted to participate, I agree to release the University from any and all costs, claims, injury or illness resulting from my participation in the Munley Memorial Swim Meet, other than for the University s intentional misconduct or gross negligence.
