3 1. IN CONSIDERATION of being permitted to participate in any way in USA Table Tennis sanctioned events, I and/or minor child, our personal representatives, assigns, heirs, and next of kin: 2. ACKNOWLEDGE, agree, and represent that I and/or minor child understand the nature of Table Tennis Activities and that I and/or minor child are qualified, in good health, and in proper physical condition to participate in such Activity. I further agree that if at any time I believe conditions or equipment to be unsafe, I and/ or minor child will immediately discontinue further participation in the Activity. 3. FULLY UNDERSTAND that (a) TABLE TENNIS ACTIVITIES INVOLVE RISKS AND DANGERS OF SERIOUS BODILY INJURY, INCLUDING PERMANENT DISABILITY, PARALYSIS, DISMEMBERMENT, AND DEATH, HARASSMENT, EXPOSURE TO INAPPROPRIATE CONDUCT AND LANGUAGE ("RISKS"); (b) these Risks and dangers may be caused by me and/or minor child's own actions, or inaction, or the actions or inaction of others participating in the Activity, the condition in which the Activity takes place, or THE NEGLIGENCE OF THE "RELEASEES" NAMED BELOW; (c) there may be OTHER RISKS AND SEVERE SOCIAL AND ECONOMIC LOSSES either not known to me or not readily foreseeable at this time; and I FULLY ACCEPT AND ASSUME ALL SUCH RISKS AND ALL RESPONSIBILITY FOR LOSSES, COSTS, AND DAMAGES I and/or minor child incur as a result of my participation in the Activity. 4. HEREBY RELEASE, DISCHARGE, AND COVENANT NOT TO SUE USA TABLE TENNIS OR FASTT, their respective administrators, directors, agents, officers, officials, volunteers, and employees, other participants, any sponsors, advertisers, and if applicable, owners and lessors of premises on which the Activity takes place, (each considered one of the "RELEASEES" herein) FROM ALL LIABILITY, CLAIMS, DEMANDS, LOSSES, OR DAMAGES ON MY AC- COUNT CAUSED OR ALLEGED TO BE CAUSED IN WHOLE OR IN PART BY THE NEGLIGENCE OF THE "RELEASEES" OR OTHERWISE, INCLUDING NEGLIGENT RESCUE OPERATIONS; AND I FURTHER AGREE that if, despite this RE- LEASE AND WAIVER OF LIABILITY, ASSUMPTION OF RISK, AND INDEMNITY AGREEMENT I and/or my minor child, or anyone on my and/or my minor child's behalf, makes a claim against any of the Releases, I WILL INDEMNI- FY, SAVE, AND HOLD HARMLESS EACH OF THE RELEASEES from any litigation expenses, attorney fees, loss, liability, damage, or cost which may incur as the result of such claim. 6. I HAVE READ THIS AGREEMENT, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND HAVE SIGNED IT FREELY AND WITHOUT ANY INDUCEMENT OR ASSURANCE OF ANY NATURE AND INTEND IT TO BE A COMPLETE AND UNCONDITIONAL RELEASE OF ALL LIABILITY TO THE GREATEST EXTENT ALLOWED BY LAW AND AGREE THAT IF ANY PORTION OF THIS AGREEMENT IS HELD TO BE INVALID THE BALANCE, NOTWITHSTANDING, SHALL CONTINUE IN FULL FORCE AND EFFECT. 7. By registering for the 2016 USA Table Tennis US Open, all participants are subject to possible selection for drug testing as conducted by the US Anti-Doping Agency. 8. MEDICAL AND TRANSPORTATION RELEASE I hereby give consent for the organizers of the 2016 USA Table Tennis US Open to provide me with athletic trainer and emergency medical treatment. This release is effective for the period of December 12-17, I swear and affirm that I am in good physical condition and I am not aware of any previous or existing diseases or injury that would result in my being injured. I further acknowledge that I have procured on my own and currently hold valid and adequate insurance for such loss, damage, or injury and if I do not have valid and adequate insurance, I accept full responsibility for the cost of the loss, damage, or injury that might occur as a result of my participation in the 2016 USA Table Tennis US Open. I give my consent to participate in any and all emergency transportation associated with activities during the 2016 USA Table Tennis US Open, in the event I become incapacitated or otherwise not conscious to give direct consent and I accept full responsibility for all costs associated with all emergency transportation as deemed necessary. Further, as the parent/guardian of a participating minor athlete, I hereby give my consent for complete medical treatment and emergency transportation services as deemed necessary beyond the on-site athletic trainer or emergency medical treatment provided. 9. MEDIA AND PHOTO RELEASE Any image, photographic, or otherwise, taken of tournament play or within the official venue is essentially the property of USA Table Tennis regardless of the Approved status of the recording instrument or photographer. Entrants agree to allow their voice and likeness in such images to be reproduced in connection with USA Table Tennis by way of any medium. USA Table Tennis is not responsible for nor can they control the use of camera phones inside the venue. Participants are hereby notified of this policy. Eligibility: The 2016 U.S. Open Table Tennis Championship is open to all members of USATT and to members of ITTFaffiliated associations. USATT membership MUST be valid through December 17, USA residents who are nonmembers or those with memberships that have expired must enclose membership fees with their entry forms. Rules: The 2016 U.S. Open Table Tennis Championship will be conducted under the ITTF Laws. Specifically Chapter 3 of the ITTF Handbook Section 3.5.3, , and All decisions made by the Tournament Committee will be final. Equipment: Tables and nets: JOOLA. Balls: Nittaku Premium 40+ (White). Floor: Two surfaces rubberized floor and concrete. This is the Official Approved Equipment for the 2016 US Open Championships. Rackets in all events are subject to random testing for volatile organic compounds (VOC s). Any glues containing VOC s will cause the racket to fail. EX- CEPT FOR HARDBAT AND SANDPAPER EVENT S, RACKETS MUST COMPLY WITH THE RED/BLACK RULE. Clothing: The USATT Dress Code will be enforced. No white clothing will be allowed. Players must wear the assigned number or their name on the back of their shirt when competing or have their name printed on the back of their jersey. Names must be large and visible to spectators. It is recommended that each player come to the table with 2 distinctively different colored shirts. Entries: Players may enter a maximum of nine events, but may not enter two events starting at the same time. All matches, both singles and doubles, will be scheduled in advance in each event. Player will receive notification when they register. Players will NOT be allowed to enter events after the final deadline or at the tournament, nor will substitutions be allowed except for the Doubles events. RATING EVENTS: 1. Players may enter any rating event they are eligible for. 2. Players that are over the rating limit of an event will automatically be entered in the next higher rating event. 3. Events may be combined if entries are insufficient. RATING DOUBLES EVENTS: 1. Players MUST be rated under 2500 to participate in the U4200 Doubles, under 2250 to participate in the U3700 Doubles, under 2000 to participate in U3200 Doubles and under 1600 to participate in the U2700 Doubles. Rating Adult and Junior Events: Players entering the Rating Junior events must be born on or after 1/1/1998 (18 and under). Players entering the Rating Adult events must be born on or before 12/31/1997 (19 and over). Doubles Partners: Players MUST enter with a partner in all doubles events. USA Table Tennis will NOT arrange for a doubles partner. USATT can not be responsible for the nonparticipation of your selected Doubles Partner. It is the responsibility of the players to contact his/her doubles partner about participating in the event. No refund will be given for an event if a partner is not available. Players without a rating at the Qualification time (10/6/2016) can t enter rating doubles events. Partners withdrawing by November 28, 2016 may be replaced provided the replacement partner has an equal or lower rating than the original partner. Partners withdrawing after November 28, 2016 may not be replaced. Need a doubles partner? Head to and create a post requesting one! Special Note: No events will be added to player schedules after 11/28/2016. For all Doubles events the prize money is for the entire doubles team NOT per person. Rating Restrictions and Seeding: For rating eligibility (qualification), the ratings list as of 10/6/2016 will be used. This can be found on the ratings page at or by to Jon.Taylor@usatt.org. For draw and seeding purposes, the most recent USATT ratings list as of 12/1/2016 will be used. Players assume the responsibility of entering the proper events. Player that are over the rating limit of an event will automatically be entered in the next event. Players may obtain their ratings by contacting USA Table Tennis via at admin@usatt.org. As a result of later seeding, some players may have ratings higher than shown for the event. Age Restrictions: Players assume the responsibility of entering the appropriate age events. Players who are over/under the age limit of an event will automatically be entered in the correct age event. Unrated Players (players without a USATT rating): Unrated U.S. players may enter any Championship event for which they are otherwise eligible. Unrated players at the seeding time will not advance in the Rated Limited events with the exception of the recreational event. Unrated players at qualification time can t enter the rated double events. The Tournament Director will be the final authority on a player s rating status. Event Scheduling: Events are not guaranteed to be completed on the same day that they begin. No refunds will be given for early departure. Any event may be cancelled and the entry fee for that event refunded if the event contains fewer than four players, pairs, or teams as appropriate. In case of event conflict, the player is responsible for notifying the Control Desk as soon possible. In the case of an inadvertent error, liability is limited to the refund of the event fee for the event(s) involved. Championship Age Events: Age: 9 and Under Must be born on or after January 1, 2007; 11 and Under Must be born on or after January 1, 2005; 13 and Under Must be born on or after January 1, 2003; Cadet Events Must be born on or after January 1, 2001; Junior Events Must be born on or after January 1, 1998; Under 21 Must be born on or after January 1, 1995; Players Must be Over 30, Over 40, Over 50, Over 60, Over 65, Over 70, Over 75, and Over 80 as of December 31, Proof of age must be readily available at the tournament. Withdrawal/Refunds: A 45 processing fee will be charged to any participant who withdraws from the tournament prior to November 21, Withdrawal notification must be in writing at USATT Headquarters in order to receive a refund. There will be no refunds given after this date except for injured players with a signed doctor s note. PLAYING FORMATS: Men s and Women s Singles: In the Men s Singles, the top 16 players will be seeded into the main draw of 64 players. In Women s Singles, the top 8 players will be seeded into the main draw of 32 players. Depending on the number of entries, all other players will be drawn into qualification RR groups to advance into the main draw. The main draw will be played in a single elimination format. All matches will be played best of seven games. U21 Men s and Women s Singles: Born on or after January 1, Top 8 players will be seeded into the main draw in these events. Depending on the number of entries all other player will be drawn into qualification RR groups to advance into the main draw. The main draw will be played in a single elimination format. For U21 Men s singles, the main draw will consist of 32 players, and for U21 Women s singles, the main draw will consist of 16 players. Matches in qualification rounds will be played best of 5 games and in the main draw best of 7 games. Men s, Women s and Mixed Doubles: 8 pairs will be seeded into the main draw in these events. The rest of the teams will play qualification matches in a single elimination format. The main draw will consist of 16 doubles pairs and will be played in a singles elimination format. Matches in qualification rounds will be played best of 5 games and in the main draw best of 7 games. Junior Boys and Girls Teams: The USATT club must send an to Jon.Taylor@usatt.org with the Club name along with names of the Team Players. The Junior Teams will not be accepted without Club approval. The Club along with the Names of the Teammates must be listed on the entry form. Playing system: Corbillon Cup System: 4 singles and 1 doubles. A-X, B-Y, doubles, A-Y, B-X. Event format will be Single Elimination or Round Robin depending on number of entries. Events 1,2,6,7,39, 40, 43 and 44: Are ITTF approved events that count for the World Ranking. All players entering these events must provide a copy of their passport. Event 57 Under 2000 Tiered Super RR: The first phase will be seeded and drawn like a normal rating event. All players will advance to the second phase round robins. First place finishers will advance to Division A, second place finishers to Division B, etc. Two players will advance from each second phase round robin group to form a single elimination final phase for each Division. Note that unrated players may not enter this event (unless they acquire a rating below 2000 by the seeding cutoff).

8 2016 U.S. PROSPECTUS Event 8 US Citizens Open: This event is for US Citizens only. All entrants must provide proof of U.S. citizenship through one of the following ways: U.S. Passport, birth certificate, certificate of naturalization, military ID, or voter registration card. State driver s licenses will not be accepted as proof of U.S. citizenship. This event is open to all ages, genders and ratings; outside of being a U.S citizen, there are no other entry restrictions. LIHA Sandpaper Event: The rules for this event will be easy return rules of the Philippines. The Liha event will be selfregulated by the participants. Balls: Nittaku orange 38mm. Hardbat & Sandpaper Events: Hardbat rules, as adopted by the USATT Classic Table Tennis Committee, will be used for all Hardbat events. See for the complete list of official rules. As in all events, a question of racquet legality is the decision of the Tournament Referee. Rackets complying with all requirements of the rules for Hardbat rackets shall be legal. These events do not count for USATT ratings points. Eligibility and seeding for the hardbat and sandpaper events will be based on the ratings maintained by the Hardbat Comittee (or by a player s USATT rating if they have no hardbat rating). If the participant enters the U-2000 Hardbat RR, U-1800 Hardbat RR, or U-1500 Hardbat RR and there are insufficient entries, he/she will automatically be moved into another Hardbat event if eligible. All Hardbat matches will be played to 21-point games. All matches will be best of 3 games except for Hardbat Singles (men s and women s) and Open Sandpaper where the quarterfinals, semifinals, and finals will be best of 5 games. Balls for sandpaper events: Nittaku orange 38mm Registration: Players must register with tournament officials at least 30 minutes before their first match and be ready to play each match 10 minutes before the scheduled starting time. See Earliest Starting Times in the Schedule of Events. Spectator Passes: ALL Persons entering the venue must obtain an entry pass. Day Passes: 10 on site only 6-Day Passes Spectator Passes: 30 with Entry, On site 35 Each entrant/usatt Active Coaches receive a free entry for the entire tournament. Credentials: Lost credentials will not be replaced. A replacement will need to be purchased at the registration desk for 35. There will be no exceptions. Players that leave their credentials in their hotel rooms will be required to buy a day pass to get in. Films/videotapes: Anyone wishing to make films or videotapes of the matches must receive written permission from USATT Headquarters. Without regard to Approved Camera status, no videotaping of the Women s or Men s Semi-Finals or Women s or Men s Final matches is allowed. Cancellation Without Fault: USATT will refund 50% of all entry fees paid by participants if the U.S. Open event is cancelled or partially cancelled due to acts of God, force majeure, war within the continental U.S., governmental authority, disaster strikes, civil disorder, curtailment of transportation services that prevent at least 50% or more of USATT participation from attending the event. Umpires: If an umpire is available, a certified umpire will be assigned by the chief umpire at the request of any player in a match. Call for Umpires: USATT is asking players who are umpires to volunteer to umpire at the US Open. The umpire must be available for a block of four hours or more. Prize Money Checks: Prize money will not be paid for no-shows, splits, dumping, or defaults (except for verified cases of injury, illness, or emergency).
