Simply complete Sections 1 and 3 and update your mailing address in Section 2 (if applicable). Only those who did not receive a credential to the 2008, 2009 and/or 2010 US Open must complete the entire form. The 2011 US Open Junior Tennis Championships Legal Consent Waiver (Section 3) is required for players only, and must also be signed by a parent or legal guardian if the player is under the age of 18. Yes No United States Citizens Social Security Number* or Passport Number* Foreign Applicants Passport Number* Issuing Country* U.S. Driver s License Number Issuing State *Required for application approval and credential printing Section US Open Junior Tennis Championships Legal Consent Waiver Needs to be completed by ALL players and their parent or legal guardian In consideration of the acceptance of my entry, I acknowledge the risks associated with participating in the 2011 US Open Junior Tennis Championships and/or any event staged in connection therewith (collectively, the US Open ), accept those risks voluntarily and hereby for myself and on behalf of my heirs and legal representatives indemnify, release and discharge the United States Tennis Association Incorporated (USTA), USTA National Tennis Center Incorporated, the City of New York, the International Tennis Federation (ITF), and each of their respective officers, directors, employees, volunteers and representatives (collectively, the Released Parties ) of and from any and all claims and demands of every kind, nature and character for any loss, injury or damage which I may suffer or sustain, by negligence or otherwise, to the fullest extent permitted by law in connection with any aspect of the US Open, including but not limited to any action and/or decision taken regarding the conduct of any matches. In consideration of the acceptance of my entry, I hereby agree to abide by all applicable rules, regulations and codes of the USTA, Grand Slam Committee, ITF and the 2011 US Open Junior Tennis Championships including but not limited to the following: (1) Every player must be dressed in a professional manner for all matches. Clean and customary tennis attire must be worn. The Referee will have the authority to order the player to change his/her attire immediately if, in his opinion, it is not acceptable. Failure of a player to comply with such order may result in immediate default, and the player will be fined according to the Grand Slam Code of Conduct and/or ITF Junior Circuit Regulations; (2) At all times while on-site at the 2011 US Open (both on-court and off-court including press conferences), no commercial identification shall be displayed by player other than as specifically permitted by the 2011 Official Grand Slam Rule Book; and (3) Players are strictly prohibited from taking motion pictures, still pictures, sound recordings and any other reproductions of any description in the locker rooms. I am aware and accept that: (1) The Referee will have the authority to take any action he deems necessary in the interests of the US Open, including but not limited to defaulting a player, without any resulting liability for any of the Released Parties; (2) My coach (to the extent he/she receives a 2011 US Open Coach Credential) will be required to be available for and participate in televised interviews as requested by the USTA or its designee immediately prior to, during and immediately following my matches; (3) I am strictly prohibited from requesting a 2011 US Open Credential and/or player ticket for any person(s) other than my coach, trainer, family members and bona fide guests whose attendance is directly related to my participation in the 2011 US Open Junior Championships US Open Player Guest Credentials and player tickets, if available, may only be used for legitimate player guests (no resale or transfer by player guest), and player may not receive any direct financial or other benefit as consideration. The USTA will hold me responsible for any sale, purchase or transfer of my 2011 US Open Coach/Trainer/Player Guest Credentials and/or player tickets under the Grand Slam Code of Conduct and/or ITF Junior Circuit Regulations and may also revoke my coach/trainer/player guest credential and/or player ticket privileges in the future; and (4) The USTA will hold me responsible under the Grand Slam Code of Conduct and/or ITF Junior Circuit Regulations for the behavior of anyone receiving a 2011 US Open Credential and/or player ticket at my request. In addition, I am aware of and accept that: (1) I am strictly prohibited, directly or indirectly through others, from wagering anything of value in connection with the 2011 US Open; (2) I am strictly prohibited, directly or indirectly through others, from offering or receiving anything of value to or from any person with the intent to influence any player s efforts in the 2011 US Open; (3) I will strictly adhere to the Uniform Tennis Anti-Corruption Program; and (4) Any violation of these regulations will subject me to severe penalties as well as a maximum penalty of permanent suspension from play in all Grand Slams. I hereby authorize a physician, dentist, surgeon, emergency medical technician or other medical personnel to provide medical or dental treatment or diagnostic services (including but not limited to x-rays, MRIs and other diagnostic examinations) to me in the event that I become ill or sustain an injury in connection with my participation in the 2011 US Open Junior Tennis Championships. I am aware of and accept that: (1) I may be required to undergo drug testing imposed upon the 2011 US Open by authorities outside its control and/or the governing bodies of the sport; (2) I authorize any entity responsible for such drug testing to notify the USTA should it be determined that I have violated the Tennis Anti-Doping Program and/or any other applicable anti-doping regulations. In consideration of the acceptance of my entry: (1) I hereby agree that I will be respectful of US Open sponsors, television partners, business partners, and all attendees; and (2) I hereby grant to the USTA and its assignees the right to use or authorize the use of my name, voice, likeness, signature and biographical material without additional compensation for the purpose of publicizing, promoting and advertising the USTA and its affiliates, including but not limited to the Olympus US Open Series, the US Open Tennis Championships, the US Open Junior Tennis Championships and for the televising, broadcasting and filming of the same; and hereby grant and assign to the USTA and its assignees in perpetuity throughout the world the right to make, use, show and reproduce in any way now known or hereafter devised, for commercial and non-commercial purposes without additional compensation, still pictures, motion pictures, sound

3 recordings, posters, live, taped or filmed television taken of me during the 2011 US Open; provided, however, the use of my identification (e.g. name, photograph, likeness, biography, voice, etc.) shall not be identified as or represented to be an endorsement by me of any product, service or company. I further acknowledge that the player guest boxes in Arthur Ashe Stadium may be equipped with microphones and I will advise my guests accordingly. In further consideration of the acceptance of my entry, and to assist the USTA in its mission to promote and develop the growth of tennis, I hereby agree to: (1) Cooperate with the media in reasonable pre-match interviews, post-match interviews, press conferences, interviews on-court and finals ceremonies; (2) Participate, if requested, in Arthur Ashe Kids Day on Saturday, August 27, 2011, or another reasonable promotional activity (not to exceed 60 minutes) related to the 2011 US Open; and (3) Participate, if requested, in the 2012 US Open Draw Ceremony should I win the 2011 US Open. The player s participation in any of the promotional activities outlined herein shall not be identified as represented to be an endorsement by the player of any product, service or company. I understand that I will not be allowed to collect my credential or to participate in any competition at the 2011 US Open until this entry form has been duly signed by me and my parent or legal guardian. This agreement shall be construed in accordance with the laws of the State of New York, USA, without regard to its principles of conflicts of laws. Any dispute arising under this agreement shall be litigated in a federal or state court of competent jurisdiction located within the City of New York, State of New York, USA. Parent/Legal Guardian of Participant Signature Print Name Section 4 You acknowledge and agree that the issuance of credentials is subject to the prior approval of the USTA and/or its representatives and such credentials can be withheld or revoked at any time for any reason in the sole discretion of the USTA and/or its representatives. You acknowledge and agree that if you apply for and accept a 2011 US Open Coach Credential, you will be required to be available for and participate in televised interviews as requested by the USTA and/or its representatives immediately prior to, during and immediately following the 2011 US Open matches of each player that you coach. By applying for a credential, you agree to comply with all the credential terms and conditions issued by the USTA and/or its representatives, including but not limited to the following: Terms and Conditions. A non-transferable credential is issued to an approved, qualified individual ("Bearer") for the sole purpose of providing access to certain restricted areas on the grounds of the 2011 US Open, as determined by the USTA (defined below). These Terms and Conditions shall apply to all Bearers. Any violation of the Terms and Conditions, or any unauthorized use of a credential subjects Bearer to ejection from the 2011 US Open and potential prosecution for criminal trespass and denial or revocation of credentials for future US Open tournaments and other USTA events. USTA is defined, for the purposes of a credential, to include the United States Tennis Association Incorporated, USTA National Tennis Center Incorporated, and each of their respective affiliates, agents and employees. A credential is not valid for US Open seating. Each credential is property of the USTA and may be revoked at any time for any reason at the sole discretion of the USTA. Completion of the credential application constitutes agreement by Bearer to comply with these Terms and Conditions. Bearer agrees to the following: (i) refrain from taking photographs or video footage of any kind in restricted areas including but not limited to locker rooms (the taking of photographs and video footage applies to the use of all equipment having the capability of capturing such images, including but not limited to mobile phones); (ii) refrain from participating in or aiding and abetting, directly or indirectly, any form of gambling or betting on the outcome of any tennis match staged during the 2011 US Open or any other activity relating to the 2011 US Open; (iii) refrain from requesting player autographs at any time during the 2011 US Open; (iv) at all times while on-site at the 2011 US Open (both on-court and off-court, including press conferences), no commercial identification shall be displayed by Bearer other than as specifically permitted by the 2011 Official Grand Slam Rule Book; (v) refrain from requesting a 2011 US Open Credential and/or player ticket for any person(s) other than Bearer s coach, trainer, family members and bona fide guests whose attendance is directly related to Bearer s participation in the 2011 US Open US Open Player Guest Credentials and player tickets, if available, may only be used for legitimate player guests (no resale or transfer by player guest), and Bearer may not receive any direct financial or other benefit as consideration. Bearer acknowledges that the USTA will hold Bearer responsible for any sale, purchase or transfer of Bearer s 2011 US Open Coach/Trainer/Player Guest Credentials and/or player tickets under the Grand Slam Code of Conduct and may also revoke Bearer s coach/trainer/player guest credential and/or player ticket privileges in the future. The USTA will hold Bearer responsible under the Grand Slam Code of Conduct for the behavior of anyone receiving a 2011 US Open Credential and/or player ticket at Bearer s request; and (vi) abide by any lawful direction given to Bearer by US Open officials during the 2011 US Open.

4 Each Bearer: (i) assumes all risks incident to attending the 2011 US Open; (ii) agrees that, unless otherwise designated by the USTA, he/she is not acting for the USTA in any manner whatsoever and is not an employee or agent of the USTA; (iii) consents to the use of his/her image or likeness incidental to any live or recorded video program, photograph, or other transmission or reproduction of the 2011 US Open for any and all purposes in perpetuity; and (iv) agrees to defend, indemnify and hold harmless the USTA, USTA National Tennis Center Incorporated, and the City of New York, their affiliates and respective agents and employees (collectively, the "Indemnified Parties") from and against all liability, loss, damage or expense resulting from or arising out of the issuance of a credential, Bearer's presence on the 2011 US Open grounds and/or Bearer's use of any 2011 US Open transportation services or hotels, except to the extent such liability, loss, damage or expense arises directly out of the willful or intentional misconduct of any Indemnified Party. General Release of Information I, the undersigned,, hereby authorize the release to the United States criminal justice agency(ies) conducting the security clearance process for individuals to be credentialed for the 2011 and/or subsequent US Open Tennis Championships (one or more such agencies referred to herein as the Agency ) of any and all records concerning me and/or my background, including without limitation my driving history and any criminal history (including any charges against me), which may be in the possession of any local, state, federal or national entity. I understand and agree that any information obtained based upon this authorization may be utilized by the Agency in connection with: (1) any background investigation conducted for the 2011 US Open Tennis Championships or any subsequent US Open Tennis Championships for which I seek a credential; and (2) any security measure instituted for the 2011 US Open Tennis Championships or any subsequent US Open Tennis Championships. It is not intended that the United States Tennis Association Incorporated ( USTA ) will be made privy to the details or specifics resulting from the Agency s investigations; nevertheless, the USTA agrees to treat any and all information it receives in the credentialing process in strict confidence, such that no information shall be divulged by the USTA to any third party unless, and only to the extent, required by law. By signing below, I voluntarily authorize the Agency to make such investigations and I hereby release from all liability or responsibility, the USTA, the Agency, my employer (if applicable), and their respective directors, officers, employees and agents, such entities and agencies that receive such information and all persons or entities supplying or collecting such information for disclosure and release to the Agency. In addition, I hereby release the USTA from any and all liability and/or any losses or harm resulting from any ineligibility to attend, compete in and/or work at the 2011 US Open Tennis Championships (or any subsequent US Open Tennis Championships for which I seek a credential) that results from such investigation. I understand that submission of this document does not guarantee the issuance of a credential to the 2011 US Open Tennis Championships. I am aware that this instrument may be photocopied in its use and hereby acknowledge (i) the validity of my signature on such duplicated copy, and (ii) any facsimile copy, electronic copy or photocopy shall be as valid as the original. If applicable, I am also aware that my employer may retain a copy and hereby release the USTA and the Agency from any liability resulting from any misuse on the part of my employer concerning such copy. I have read and fully understand the contents of this disclosure and authorization for release of information and execute it freely and voluntarily. 5 Section 7 A current passport-size photograph (head and shoulders view against a light background) must be submitted for each new applicant with the individual's name and affiliation (Main Draw Player, Qualifying Coach, etc.) clearly written on the back of the photo. Those who received a credential for the 2008, 2009 and/or 2010 US Open do not need to submit a photo.
