2 Events Description 5 1 day Junior Team Tennis Events U14/U18 Girls Doubles division Rec U14/U18 Girls Doubles division Open U14/U18 Boys Doubles division Rec U14/U18 Boys Doubles division Open U10/U12 Gender Neutral Doubles division Round Robin Format no elimination Eligibility The Junior Team Tennis Open to Girls and Boys U10, U12, U14 & U18 - as of Jan 1, 2016 U14/U18 - Team consists of one U14 doubles pair and one U18 doubles pair same gender. U10/12 Team consists of one U10 doubles pair and one U12 doubles pairs - gender neutral. Team consists of 4 to 6 players, minimum 4. Must be a member of an OTA or ICTA club. All players must bring a completed and signed waiver form to the registration desk. Full team rosters including the team captain and all players date of birth must be submitted prior to or at registration. Each player can only play for 1 team. Players must arrive at least 30 minutes prior to the start of the fixture as per OTA Rule #16 on punctuality.

3 Rules of Play Boxed round robin format Number of teams per box and number of boxes will be determined by total entries Morning will be random draw some seeding will take place across boxes Morning results will determine flights for afternoon round robin - no eliminations Afternoon results will determine final flights standings Fixture consists of 2 doubles matches one U14 and one U18 or one U10 and one U12 U14/U18 - Pro set to 8 games, (no ad, receivers choice) sudden death game at 7 all. U10/U12 Pro set to 6 games, (no ad, receivers choice) sudden death game at 5 all. Teams will play matches against all other teams in box Total games for in all matches will determine final position in boxes

4 Rules of Play In case of tie in games won, position will be determined by head to head results, then total games against, then by coin toss Position in box at end morning will determine flight for afternoon Position in box at end of afternoon will determine flight champions Substitutions permitted between matches no midmatch substitutions At least one player from each doubles team in morning play must be available to play in afternoon Rules and scoring are subject to modification based on weather and time constraints All players are expected to abide by the OTA Code of Conduct Rules

5 TOURNAMENT/EVENT PARTICIPANT WAIVER FORM This Waiver must be signed by Participant or Parent/Guardian of Participant (if participant is under 18 years old) for the Junior Team Tennis and the OTA Tennis Rocks! Festival. WAIVER The undersigned acknowledges and agrees to assume all risk for bodily injury, waive all claims for injury and property damage and release and hold harmless Tennis Canada, the Ontario Tennis Association, the ICTA, its management and partners, the participating players and volunteers from any liability for personal injury and property loss or damage whether occurring prior to, during or subsequent to the Junior Team Tennis and the OTA Tennis Rocks! Festival, and consents and grants permission to, Tennis Canada, the Ontario Tennis Association, the ICTA, the organizers of this tournament or its or their respective officers, agents, representatives, and/or assigns to use (without, for greater certainty, any further consent or fee being required) any photography, videotape or other electronic recording device of my image taken, filmed or recorded during, or in connection with, my participation in this event, for any promotional, news or other purpose. I understand the nature of this event and the inherent risks of participating in such an event.
