1 2019 PPAI Hall of Fame Nomination Form The highest distiction the promotional products industry bestows on its professionals is induction into the Promotional Products Association International (PPAI) Hall of Fame. The Hall of Fame recognizes exceptional contributions to and long-term involvement with PPAI and the industry. Inductees are chosen by nomination from within the promotional products industry.

2 Nomination Form PPAI Hall Of FameTM ELIGIBILITY An eligible candidate: Has made significant contributions to the promotional products industry and to his or her community May be living or deceased Cannot be a current member of the PPAI Board of Directors or the Hall of Fame Advisory Group Must be off the Board for three full years before being eligible as a Hall of Fame nominee* Must be off Hall of Fame Advisory Group for four years before being eligible as a PPAI Hall of Fame Nominee Must not have been honored with PPAI Distinguished Service Award within past two years *The waiting period is reduced to one full year if in the nominee served in a board appointed position filling a board vacancy or an at-large board position. DEADLINES FOR NOMINATIONS A special thank you to our award sponsor Opens - March 5, 2018 Closes - April 6, 2018 A Division of NOMINATION AND SELECTION PROCEDURE All association members are eligible to submit a nomination for the Hall of Fame. Members of the Association s Board of Directors and the Hall of Fame Advisory Group shall not be permitted to make nominations, or second nominations, to the Hall of Fame. Nominations shall be screened by the association staff to eliminate ineligible individuals. All eligible nominations shall be forwarded to the PPAI Hall of Fame Advisory Group to select and send to the Board of Directors a slate of candidates. No more than half of the candidates may be posthumous nominations. Induction to the Hall of Fame shall be by ballot vote of the Board of Directors. Only no votes are to be cast. Seven negative votes shall be considered a rejection of the nominee and that person will not be inducted into the Hall of Fame. Candidates who are not selected will automatically be reconsidered for the next two years (a total of three years) by the Advisory Group. After that period of time, if the individual has not been inducted, he or she will need to be nominated again by a member before being reconsidered. INTRODUCTION Candidates accepted for induction into the PPAI Hall of Fame will be inducted at a special presentation ceremony at The PPAI Expo.
