3 Team Manager Responsibilities You must decide between the team manager and the head coach the following responsibilities: -It is wise to have a binder that you can put all your info in to keep it organized. You will need this binder and info with tournaments and games. -You must collect a copy from each player on your team the registration/medical release form (a copy of one is at the back of this booklet) after they register for UYSA. Put this in your binder. You do not need to collect birth certificates! -Make sure every player is registered properly with Surf and UYSA on time. -Follow up with coach on their HEADS UP CONCUSSION TRAINING. This is REQUIRED every year and the coach and TM must take the course before the first fall season game. -Collect roster info from the head coach and keep in a binder, create a team contact sheet. -Make sure the team is properly informed EACH WEEK with updates to schedules and information from the coach even if there are no changes, TeamSnap is a great tool used club wide. -Register team for tournaments that your coach has assigned most out of state tournaments use GOTSOCCER. GOTSOCCER information is on page 15. -Collect money at times from players for tournaments, futsal, etc. -Club pass players and collect the right paper work for this. -Print rosters for games (or use digital player cards check with coach). -Check in for tournaments. -Be early to games to see if the coach needs anything. -Make sure our club is represented well with good sportsmanship with the parents. *It is extremely important to communicate with your coach to make sure all information is correct, you are doing what they need, and that the team is ready to go. 3

4 Registering for Surf All players will get an from Utah Surf that says: Congratulations, (player s name)! Utah Surf Soccer Club and (coach s name) are pleased to offer you a spot on our (team name) team for the 2018/2019 soccer season. Please send an to (coaches ) to accept your child's spot on this team within 25-hours of receiving this . (DO NOT reply to this ) The squad for this team is amazing, and we want to get organized and moving forward ASAP. To complete your registration: 1. Send acceptance to (coach s name and ). 2. Please complete the online registration to accept the offer before the registration link expires. Begin your online registration here - Do NOT share this link with anybody else - this unique link is tied to your account. Online registration includes accepting the Player/Parent Agreement and choosing your payment plan. If you have questions or need help, please let me know. The addresses selected during the registration process will be used for all future team and club-related information. 4

5 Registering for UYSA Player Registration: Go to: Please take care of this by June 15 th and give a copy of your parent/medical/ player agreement signed to your team manager. You can print this at the end of registering with UYSA. Once you get to the home screen from following the registration link Click on the "Registration Tab" in the upper right corner of the screen. Select a season - make sure you choose the correct year. From here you will first choose your team from the drop down menu. Team Name Log in or create an account. Follow the on screen instructions. Please upload a CURRENT photo. Must be front facing, head shot. No sunglasses and no hats. Photos were removed so you have to add a new one. You can just use a school photo. 2. The invite you received from your coach for the team included a registration link. This registration link is for the club fees. Make sure you click on that link to complete the registration process with the club. Coach and TM Registration: 5. All coaches and Team Managers must register as a coach/admin in the UYSA system EVERY year. Please complete your UYSA registration as soon as possible. To register as a coach/admin please follow the instructions below: (You must go through the Surf website. This is the only way that it will link your registration to the Surf.) Follow the link for the specific age group: (These instructions are the same for your players to register. You may send them the links below) Competition (U13+) X League (U9-U12) OR Click on Utah County and then Surf All Access Under "League Registration" choose UYSA X League (U9 -U12) or UYSA Competition (U13+) Click on the "Registration" tab in the upper right corner of the screen Make sure you have a photo uploaded. No photo = no coaching. You will not show up on the game day rosters. Photos must be close up, front facing, head shots. No sunglasses or hats. If the photo doesn't meet these requirements I will delete it and you will need to upload another photo. 5

6 HEADS UP CONCUSSION TRAINING Utah State Concussion Policy. You must watch the "Heads Up Concussion" video and take the quiz EVERY seasonal year. You will be issued a certificate at the end. The certificate MUST be SAVED to your computer and UPLOADED to your coach s profile. ALL coaches MUST complete this training and ALL team managers...any team administrator. The system does NOT save your certificate. If you don't save it to your computer you will have to repeat the training. The training takes about minutes. It must be done every year. If you don't have the certificate uploaded you will not show up on the game day roster and you won't be able to coach. The video can be found under the Coaches tab atwww.utahyouthsoccer.net Heads Up Concussion Training and the License rules are not up for discussion or debate. They are UYSA policy and State Law. I can't change these rules. If you don't complete these 2 items you will not be able to coach! UYSA is strictly watching the coaching licenses. They have made it very clear that if there is a license violation I will face disciplinary action. I won't risk it...so please take care of it ASAP. 6

8 How do I reschedule a game? *Review schedules as soon as possible and pursue corrections as soon as possible. Make sure parents and coach are involved, please watch for school breaks and time *Please note that you must contact the field coordinator, referee coordinator and the opposing team (see UYSA ) before you decide on a date to see if fields, refs, and other team are available. 1- Teams are only allowed to initiate two (2) reschedules (with or without the cost) Reschedules are free up to two weeks before the first regular season game is played. Reschedules cost $50 starting the 1st day of league play. 2- The reschedule is NOT COMPLETE until it has changed on the UYSA schedule. 3- Reschedules must also be complete 5 days before the original schedule date. Before clicking the button the HOME team must have field and referee assignor approval. Please all contacts outside of affinity first. 4- Team initiating the reschedule should click the button (does not matter if home or away) Away teams MUST have an from the home team with field and referee assignor approval. If you are rescheduling for weather, no referees or unplayable fields do not click the reschedule button Send reschedule information to: scheduler@utahyouthsoccer.net. (1) To reschedule ALL games, contact the opposing team using Log in to your UYSA account Make sure your picture is highlighted Click TEAMS on the tabs below On the new tab Click Tournament and Schedule Apps Click Schedules/Game Scoring on the right hand side (by the team you want) Click the Opposing Team Name to see contact info All contacts from your personal , do not use the reschedule button at this time (2) Find several dates that teams agree on The home team MUST contact their FIELD ASSIGNOR and REFEREE ASSIGNOR for approval b. Send a couple dates that may work for a reschedule. **NO GAME should be rescheduled (using the button or sending information in) without approval from your (home team) FIELD and REFEREE ASSIGNOR contact your club if you don't know who they are. **For games scheduled at the RAC the field and referee assignor approval must come from RACreschedules@gmail.com (3) After the NEW date, time and field are approved by assignors and both teams The team initiating (asking for) the reschedule should do the following: Log in to UYSA account Make sure your picture is Highlighted Click on the TEAM tab below pictures Click on the Tournament and Schedule Apps tab 8

9 Click on Schedules/Game Scoring Click on the Reschedule Button by the game you have new information for Enter ALL information Date, Time including AM or PM, Field Name and Number (all in drop down menus) Click Send Request (4) Opposing team MUST log-in follow steps above and click Accept Request or if the information is not correct, Click Send Alternate Request and correct the information. Notify the opposing team that they must now login and Accept. Do not send any information to the state if you completed your reschedule online. It is up to the teams to follow all steps correctly. The schedule will change when the teams have finished all steps correctly. UYSA reserves the right to move games back to original dates, times and locations if teams did not follow the correct steps. IF you are rescheduling for weather, no referees or unplayable fields, send an to include game number, new time, date, field name and number along with permission from field and ref assignor. DO NOT click the reschedule button. How to contact a Coach or Administrator of the Opposing Team *Only Coaches and Administrators that are listed on the UYSA roster can access Administrator information for the opposing team. To access the information for an Opposing Coach or Administrator: 1. Log into your UYSA Account 2. Click on the Teams tab underneath your family s information 3. Click on the Tournament & Schedule Apps tab 4. Click on the Schedules/Game Scoring option on the far right hand side. 5. In the schedule scroll down to the specific game with the opponent you are trying to contact. 6. Click on the name of the team in the schedule. (The contact information for the opposing team s Coaches and Administrators will pop up in a separate box.) *If you are contacting the other team about a reschedule. Please make sure you send an to EVERYONE listed as Team Contacts 9

10 How Do I Club Pass? *Only Teams that belong to Member Organizations are eligible to Club Pass players. Independent Teams may not Club Pass. If you are unsure if you are part of a Member Organization, please visit our Member Organization Links page under the Resource tab. Only coaches and teams managers who are rostered to the team in the system have access to club pass. The lending team club passes the player to the borrowing team. 1. Log into your UYSA account. (Make sure your picture is highlighted) 2. Under your family s pictures click on the Team tab. (halfway down the screen, a new window will appear with 2 tabs) 3. In the new tabs section that has opened, select the Tournament & Schedule Apps tab (There should be 2 links on the right hand side) 4. Click on the View Tourn. App link on the right hand side. 5. Click on the Player Roster tab at the top of the page. 6. Choose the player you wish to club pass and click the Assign option on the far right hand side. 7. Select the team and game # from the drop down and then Save. If the team you are trying to club pass to does not appear in the drop down there may be one of the following problems: 1. The team you are trying to club pass to has a full roster. (Teams MAY NOT deactivate a player to open up a spot for a club pass player) 2. You may be trying to club pass a player to a team that they cannot play with. a. Premier 1 and 2 players can only club pass to the other premier bracket and to D1 regardless of the age group you are club passing to. b. You cannot club pass to another team that plays in the same bracket as your team. c. Players cannot club pass down more than 2 divisions. (i.e., North A cannot club pass to North D) d. Multi-rostered players can only club pass from the team they are primary rostered to, not from their secondary team. **If none of these scenarios apply and you are still having problems please call the UYSA Office at and we can look into the problem for you. *To view a summary of all players club passing from your roster to other teams and all players club passing to your team, click on the UYSA Club Pass option above the Assign option What if a game is terminated early? 1. If a match is terminated after fifteen (15) minutes of the second half has elapsed because of the weather or grave disorder on the part of the coaches, players, or spectators of a team that is losing at the point of termination, the match shall be considered a completed match. 2. If a match is terminated at any point because of grave disorder on the part of the coaches, players, or spectators of a team that is winning or tied at the point of termination, such team will forfeit the match If a match is terminated early due to field conditions such as failure of lights, destruction of a goal, or other conditions making the field unplayable, the match shall be rescheduled. A match rescheduled due to early termination shall be replayed in its entirety. Referee fees must be paid again at a match rescheduled due to early termination. 10

11 How Do I Print a Game Day Roster? Only coaches and administrators who are rostered to the team in the system have access to print game day rosters. To print a game day roster: 1. Go to 2. Click on Member Login in the top right hand corner of the page. 3. Log into your UYSA Coaching/Admin Account (This account should still be linked to your family. The screen is split in 2 halves, pictures on top, tabs on the bottom half.) 4. Click on the Teams tab under your family information. 5. Click on the Tournament & Schedule Apps tab 6. Select the Game Scoring/Scheduling option 7. Click the Print Roster button by the game. (The print roster button will appear 24 hours before a game, Teams are encouraged to print as close to game time as possible.) **Coaches and Admins** This is also where you find contact information for the other team for reschedules, click directly on the name of other team. NOTE: It is no longer required to print a roster for the other coach. Referees should check the date and time stamp on the roster and use the one that is most recent. 11

12 How to enter a score for a game? After a match has been played, both the Home Team and Away Team Coaches and Administrators have access to input the score and any comments about the game. Comments can only be seen by UYSA office staff and cannot be seen by the opposing team or referees assigned to the match. The winning team should input the scores for the match. If the teams or referee enter different scores for the match the system will show a CS (Contested Score) and the final results will not show until the score has been verified with the referees by UYSA office staff. Coaches, Team Managers, and Referees should NOT be entering games as abandoned or forfeits. If you believe your game should be entered as abandoned or a forfeit. Please send an with the Game # and information about the game to UYSA s League Commissioner, Cindy Baron at cbaron@utahyouthsoccer.net. The center referee assigned to the match is required to: Input their game report (including the score) Enter any cautions or ejections that were issued in the match Enter any other important match information. *Referees are not required to enter which players scored goals. To enter the score for a match 1. Log into your UYSA coach/admin account 2. Click on the Teams tab underneath your family information 3. Click on the Tournament & Schedule Apps tab You should see a list with your Team(s) 4. To the right of the team you are entering the score for, click on the Schedules/Game Scoring option. 5. Find the match you are entering the score for and click on the empty score box next to your team s name 6. Enter the score for your team and the opposing team (You must enter the score for both teams, if you leave the opposing team s score blank the system will think that the opposing team s score was 0, this will most likely result in a CS for your game.) To enter which players scored goals Under the Goals/Cautions/Ejections section go to the Select Admin/Player drop down menu Select the name of the player that scored the goal Under Item Type select Goal Click the Add button To save the information click Save Stats and Comment at the bottom of the page Do not call into the UYSA office to have goal scorers added for the game. Goal Scorers should be entered by the Coaches or Team Administrators. Teams will have 72 hours after the game to enter goal scorers. 12

13 Adding jersey numbers to your roster ** Only team coaches or team administrators can add jersey numbers to the Affinity roster** How to Enter Jersey Numbers In Affinity 1. Log into your UYSA account 2. Click on the Teams tab under your Family Information 3. Click on the Tournament and Schedule Apps. Tab 4. Select the View Tourn. App option to the right of the team 5. Click on the Player Roster tab at the top of the page 6. Click the Edit Player Team Info tab located at the bottom of the roster 7. Enter the jersey number for each player. (The home jersey # and the away jersey# MUST match) 8. Click the Save Changes button at the bottom of the page 9. Once you are finished click the Sync Roster button in the bottom left corner of the Player Roster page We recommend that you enter jersey numbers in Affinity as it is a much easier process there. From your Digital Player Cards 1. Log into your Digital Player Card Account with your UYSA username and password 2. From the Menu choose the Team option 3. Click on the Roster 4. Click on the first player card shown to open up the card 5. Click on the black rectangle with the Player s name. Enter the jersey number in the box located in the upper right hand corner 7. Click the update button just below the box 8. Swipe right to move to the next player 9. Once you have entered the jersey numbers you can return to the teams schedule from the menu drop down 13

14 Register For a Tournament, Create a Tournament Roster and Print a Photo Roster Creating a Tournament Roster in Affinity 1. Go to 2. Login in under Member Login which is located on the top right hand of the UYSA homepage. 3. You will now be on the My Account page. Click the Teams tab, click on the Team Info link to the right of the team name. 4. Click on the tournament roster tab and then Create New Request. 5. Fill out tournament information (Info is on the Sanctioned Tournament Page) and click on Save & Continue. 6. You ll be taken to the roster page next. You can delete administrators and players by clicking on the red X next the admin/player s name. To add a guest player click the Add Player/Admin Inside Org at the bottom right hand side of the screen. Choose the Roster Role from the drop down and enter the ID Number and click save and continue. Jersey numbers can also be added along with player positions on the roster page. 7. If you add a guest player or a tournament player the status will say borrowed. If the player is a guest player you must have the specialty action form with their coach s signature at the tournament check in. If this player is a tournament only player the tournament only card must be submitted at check in. (Specialty Action Form) 8. Click on the Review and Submit tab and click on the Submit Application button. 9. Click on the Review Button, and select Print Photo Roster button to print photo roster. 10. Roster will be created with pictures and you can print copies for the tournament. 11. To print copies of the roster you will need to click the small printer icon at the top of the page that opened displaying the roster. 12. Roster will populate. Click on the export icon at the top of the page and export roster to a pdf file and then print. *Guest Player Roster Instructions* All forms listed below must be completed and in the possession of the team coach, assistant coach, or manager upon Tournament Check-in. UYSA Registration form w/ Consent for Medical Treatment for each guest player UYSA Specialty Player Action Form (Guest Player Section) with all required signatures for each guest player By completing the Guest Player Roster Form you certify that: 1. The information provided is accurate 2. The players listed have player passes and are following all UYSA policies regarding tournament guest players 3. The team is registered according to the rules and regulations of US Youth Soccer and Utah Youth Soccer Association 4. The team complies with all UYSA team and player registration rules 5. The team is not under suspension or in bad standing with any organization 14

15 GOT SOCCER INFORMATION When travelling outside of Utah to play tournaments, most all tournaments use Got Soccer as their software platform. Got soccer awards points for tournament wins and they keep track of these points. It is crucial to create only one account and keep this account with the team throughout the teams history the more points on your team account, the higher the state, regional, and national ranking. These rankings are used for team acceptance into premier level tournaments. If you are a new team manager, please check with the prior team manager about the team account. CREATING AN ACCOUNT 1. Go to 2. Click LOGIN in the upper right corner 3. Click on TEAMS & TEAMS OFFICIALS. Eventually your players will need to create their own accounts. They will click on PLAYERS & FAMILIES 4. Under the TEAM ACCOUNT you can click on create account or login to your account from here. ADDING A GUEST PLAYER IN GOT SOCCER 1. If a player is attending and needs to be added to your account, the player needs to log into their personal account under PLAYERS & FAMILES 2. Once logged in there is a line that says PLAYER ACCOUNT. On that line is a tab called EVENTS. Click on EVENTS. 3. The box in the middle of the screen says GUEST SEARCH EVENTS. Click on the box. 4. In search bar type the tournament you are wanting to play as a guest in. The tournament will pull up. 5. Click REGISTER AS A GUEST 6. Once the player has registered as a guest you will log into your team account. Once logged in you will see all the tournaments to the left that you have applied for. Click on the tournament you wish to add the guest to. 7. Click on the GUEST tab. There will be a list of players who have registered to guest play. Click on the player you wish to add to your team. 8. ASSIGN them to your team and put their jersey number in. 9. Click UPDATE. From here they are on your roster for the specific tournament. 15

16 Concussion Policy UTAH YOUTH SOCCER ASSOCIATION CONCUSSION & TRAUMATIC BRAIN INJURY POLICY: Effective May 10, 2011, a Utah law called the The Protection of Athletes with Head Injuries Act (UCA , et seq.), requires all amateur sports organizations to adopt and enforce a concussion head injury policy. This policy will ensure the consistent and uniform implementation of a well-established concussion management program and return-to-play guidelines for the Utah Youth Soccer Association. Education A concussion is a brain injury and ALL brain injuries are serious. They are caused by a bump, blow, or jolt to the head, or by a blow to another part of the body with the force transmitted to the head. They can range from mild to severe and can disrupt the way the brain normally functions. All concussions are potentially serious and may result in complications including prolonged brain damage and even death if not recognized and managed properly. In other words, even a ding or a bump to the head can be serious. Concussion cannot be physically seen, and most occur without loss of consciousness. Signs and symptoms may appear immediately after the injury or can take hours or days to fully appear. If an athlete reports any signs or symptoms of concussion, or is observed expressing signs or symptoms of a concussion, seek medical attention immediately. Signs and Symptoms of a Concussion Signs Appears dazed Vacant facial expression Moves clumsily or displays incoordination Answers questions slowly Slurred speech Seizures or convulsions Loss of consciousness Symptoms Headaches Pressure in head Nausea or vomiting Ringing in the ears Balance problems or dizziness Blurred, double, or fuzzy vision Sensitivity to light or noise Feeling sluggish or slowed down Feeling foggy or groggy Drowsiness Change in sleep patterns Amnesia Don t feel right Fatigue or low energy Sadness Nervousness or anxiety Irritability More emotional or irritable Confusion Concentration or memory problems Repeating the same question/comment Page 2 of 5 Modified 4/16/13 What to do if someone is suspected to have sustained a concussion: o Notify a coach, parent/legal guardian, responsible adult if it is suspected someone is experiencing signs or symptoms of a concussion. Never ignore a bump or blow to the head. o If a concussion is suspected then an evaluation by a medical professional is required. A qualified health care provider can diagnose a concussion and advise when following return-to-play guidelines is warranted. o Rest! Players suffering from a concussion need time to allow the brain to heal properly. Young developing brains recovering from a current concussion are susceptible to sustaining a second concussion known as Second Impact Syndrome. This condition can cause permanent brain damage, or even death. It is important to rest until approval from a doctor or qualified health care provider is given before fully returning to activity. o Do not take anti-inflammatory medications such as Ibuprofen, Advil, Aleve, or Motrin without the approval from a doctor. These medications thin the blood, and can possibly cause more damage to a brain that has sustained a concussion. Possible consequences for returning a player to the field from a concussion too soon: * Athletes with the signs and symptoms of a concussion should be removed from play immediately. Continuing to play with a concussion leaves the young athlete especially vulnerable to greater injury. As quoted from the Center for Disease Control (CDC), A repeat concussion that occurs before the brain recovers from the first - usually within 16

17 a short period of time (hours, days, or weeks) - can slow recovery or increase the likelihood of having long-term problems. In rare cases, repeat concussion can result in brain swelling, permanent brain damage, and even death. This more serious condition is called second impact syndrome. Keep athletes with a known or suspected concussion from play until they have been evaluated and given permission to return to play by a qualified health care provider with experience in evaluating for concussion. Remember: It s better to miss one game than the whole season. Concussion Policy Action Plan Utah Youth Soccer Associations and Clubs: Utah Youth Soccer Association (UYSA) is defined as: - UYSA paid and volunteer administrative staff. - All registered UYSA players. - UYSA-affiliated clubs and its members. - UYSA-affiliated independent teams. - UYSA-affiliated recreation programs. Education and Notification Requirements: UYSA adopts these education and notification policies for the management of concussions in youth soccer. o All coaches (paid and volunteer) must be educated in the nature and risk of concussions prior to the first practice/competition. This education shall include signs and symptoms of concussions. All players and the parent(s)/legal guardian(s) of those players are required to sign and return annually an informed consent form relating to the nature and risk of concussions. This shall include the review of the UYSA Concussion Policy which includes the signs and symptoms of concussions, and the UYSA Concussion Clearance Form. o The UYSA Concussion Policy and UYSA Concussion Clearance Form can be found on the UYSA website under Risk Management heading, or TOSH Concussion Information. Action Plan for Head Injuries: UYSA has adopted these policies for the management of concussions in youth soccer. o Any player showing signs or symptoms of a concussion shall be immediately removed from participation/competition. o All UYSA players sustaining or suspected of sustaining a traumatic brain injury MUST use the UYSA Concussion Clearance Form in order to return to play. o UYSA will not allow any athlete who has sustained or is suspected of sustaining a traumatic brain injury to return to play until UYSA has received a UYSA Concussion Clearance Form for that athlete bearing the signature of a Qualified Health Care Provider. A Qualified Health Care Provider is someone who meets the following requirements: Licensed in the state of Utah under the Division of Occupational and Professional Licensing Act May evaluate and manage a concussion within the health care provider s scope of practice Successfully completed a continuing education course in the evaluation and management of a concussion within 3 years. Appropriate Clearance of Head Injuries: UYSA will follow these guidelines for managing concussions or suspected concussions in youth soccer. o The Qualified Health Care Provider who has cleared the athlete must provide a statement that they are trained in the evaluation and management of concussions. o Optimally, all athletes should progress through the Return-To-Play Protocol (R.T.P.P) on the UYSA Concussion Clearance Form before returning to play, but athletes may return to play without completing the protocol if indicated on the UYSA Concussion Clearance Form by a Qualified Health Care Provider. o The UYSA Concussion Clearance Form must be returned to the appropriate administration before final clearance is granted. The Appropriate Administration is the person(s) responsible for the clearance information depending on the event in which they were injured. UYSA Administrative Staff is administratively responsible for the clearances of head injuries that occur during a UYSAhosted event. A UYSA-hosted event is defined as: 1. Spring State Cup Tournament 2. Fall State Cup Tournament 3. Presidents Cup Tournament 4. All State Select/ODP Events 5. US Youth Soccer Region IV and National Events Club, Independent and Recreational teams are administratively responsible for the clearance of head injuries that occur during a UYSA non-hosted event. A UYSA non-hosted event is defined as none of the events listed above as a UYSA-hosted event, and may include, but are not limited to, the following events: 1. Regular season play 2. Practices, scrimmages, tryouts, and camps 3. Club-hosted events and tournaments * Coaches Requirements: o Shall be educated as to the nature and risk of concussions including continuing to play after a concussion or head injury. This education shall include signs and symptoms of concussions. Education 17
