Violations of this rule or misrepresentation of information on the fact sheet shall subject the offending coach, manager, and/or administration to serious sanctions. GENERAL INSTRUCTIONS: Every player participating in competitive/select soccer must be given full disclosure of certain facts about prospective soccer teams and soccer clubs (including information about the coach and other people involved, the team's plans for the year, the potential financial and time commitments, etc.). This form has been developed for this purpose. Every Select/Competitive soccer team must thoroughly complete this form and give it to prospective players before they are extended an offer to play with the team. This would usually be accomplished when a player first arrives at tryouts, or when contact is first made with a prospective player who is not attending tryouts.
