2 I. DEFINITIONS OF TYPES OF TOURNAMENTS AND TEAMS (Place corresponding letter in appropriate space on application.) TYPES OF TOURNAMENTS UT UNRESTRICTED TOURNAMENT: A tournament tat is open to all Federation affiliated participants. Any tournament tat allows international participants must be an Unrestricted Tournament. RT RESTRICTED TOURNAMENT: A tournament tat is open only to members of US Yout Soccer and its State Associations. SELECT TEAMS (teams formed by a selection or tryout process): S1 S2 S3 TOURNAMENT SELECT TEAMS S4 generally a team wic competes at te igest level of play in a state or region generally a team wic competes at oter tan te igest level of play in a state or region generally a team wic competes in a local area or state a team wic is put togeter for te sole purpose of playing in a tournament or oter sanctioned non-league compeition, wose roster includes select players wo are members of one club. RECREATIONAL TEAMS RT a team wic was formed in a random nature witout regard to players' abilities. US YOUT SOCCER ODP TEAMS ODP Team: te official US Yout Soccer Olympic Development team of US Yout Soccer, any of its regions, a State Association, an Affiliate, oter Organization Member, or any district or geograpical subdivisions tereof. NATIONAL TEAMS NATIONAL TEAM: te official national team of US Soccer or any oter member of FIFA. II. OSTING RESTRICTED AND UNRESTRICTED TOURNAMENTS AND GAMES APPLYING TO OST RESTRICTED TOURNAMENTS OR GAMES 1. Tese procedures apply wen osting ONLY US Yout Soccer State Associations or US Yout Soccer Affiliates teams for tournaments or games. 2. Applications, agreements to ost and oter required information must be submitted as provided by te designated approving associations or affiliate to include: Not later tan te date establised by te State Association or Affiliate for submitting an Application to ost a Tournament or Games, te osting organization must submit to its State Association or Affiliate and, if any game is to be played in anoter state Association or Affiliate, to tat oter State Association or Affiliate, for approval a completed Application to ost a Tournament or Games signed by te designated offical of te osting organization and te following items: a) a completed US Yout Soccer Tournament or Games osting Agreement, wit appropriate supporting documents and information, signed by te president or cief officer or designee of te osting organization and by te tournament or games director; b) a copy of te approved Rules for te Tournament or Games; and c) any fees required by te State Association or Affiliate for processing te application. Incomplete applications will be returned and considered as not aving ever been submitted. 3. Te State Associatio or Affiliate is not required to approve any application not submitted by te deadline establised by te State Association or Affiliate. Any application wic is not submitted in a timely manner may be subject to late fees if approved. Te team must contact its State Association for its specific policies and fees. 4. Te State Association or US Yout Soccer Affiliate sall make a determination as to weter or not tey will sanction te tournament or games in accordance wit te policies tey ave establised witin teir organization for te sanctioning of tournaments or games. APPLYING TO OST UNRESTRICTED TOURNAMENTS INCLUDING INTERNATIONAL TEAMS (TEAMS FROM OUTSIDE TE UNITED STATES) AND OTER FEDERATION ORGANIZATION MEMBERS

3 1. Te provisions below are in addition to tose required as stated above under Applying to ost Restricted Tournaments or Games. All tose provisions so stated above must also be complied wit. 2. For all tournaments and games involving international teams (teams from outside te United States), te Rules for te Tournament or Games must comply wit te following substitution requirements: For matces involving players under 16 years of age, te number of substitutions allowed is as provided by te Rules of te Tournament or Game. For all oter matces, te number of substitutions is as agreed to prior to te matc and te referee is informed prior to te matc of tat number. Te number agreed to can be as provided in te Rules of te Tournament or Game. 3. Te completed US Soccer forms Application to ost a Tournament or Games Involving Foreign Teams and Certification Regarding Compliance wit te Ted Stevens and Olympic Amateur Sports Act, must be submitted and approved by te osting State Association or US Yout Soccer Affiliate and any additional fees required by te State Association, Affiliate and/or te United States Soccer Federation for processing te application. 4. For all teams tat are not members of US Yout Soccer or a US Yout Soccer Affiliate, Proof of medical and liability insurance must be provided by teir organization of registration.

4 US Yout Soccer TOURNAMENT OR GAMES OSTING AGREEMENT In consideration of permission being granted to Nick Trotter to old a tournament or games at Galaxy Soccer Club Naperville Illinois (osting Organization) () (State) on te dates of 4/6/2013 TO 4/7/2013 we agree to te following conditions: ABIDE BY RULES: e sall abide by all statements made in our approved US Yout Soccer Application To ost A Tournament or Games, in our tournament invitation, in our tournament rules, in te US Yout Soccer Travel and Tournament Policy and in tis US Yout Soccer Tournament or Games osting Agreement. e agree tat all decisions regarding acceptance of teams into a tournament sall be fairly and impartially made and sall not be based upon race, creed, color or national origin and tat we will not advertise by any means te tournament or games until all approvals are received. INVITATIONS: Te tournament or games approval form sall accompany all tournament or games invitations distributed. OUSING: e agree tat we will not require a team to use only accommodations approved or provided by te osting organization or oter organization, unless disclosure is made on te tournament application form of te required otel / motel names and te guaranteed rates. PROCURING LIABILITY INSURANCE: e ave procured liability insurance coverage for te tournament or games wit limits of not less tan $1,000,000 / $2,000,000 wic names te State Association or Affiliate wit wic te osting Organization is a member. A copy of te certificate of insurance, IF REQUIRED, is attaced issued by Illinois REQUIRING MEDICAL AUTORIZATIONS: e sall require all teams participating in te tournament or games to provide medical releases for eac player in an appropriate form. Tese autorizations sall be reviewed by te osting Organization at registration and kept in te possession of a team official. ADVANCE PUBLICATION OF RULES: e agree tat our tournament or games rules sall be included wit te invitation senr to eac team and sall, again, be publised to all teams accepted prior to te start of te tournament / games. CREDENTIALS CECKS: e agree tat we sall conduct credentials cecks to ensure tat all players are registered wit US Yout Soccer or US Soccer or a member tereof or teir national association, properly rostered wit teir team, and participating in accordance wit representations set fort on te US Yout Soccer Application to ost a Tournament or Games. e agree tat we will not modify or mark in any way original rosters or member passes; and will not register any player, coac, or team official or issue any member pass. USE OF US SOCCER REGISTERED REFEREES: e agree tat we sall use for all games only US Soccer registered referees wo are in good standing (unless US Soccer as granted a waiver to allow te use of autorized referees from anoter country), and sall use a one- or 3-referee system. e intend to use a 3-referee system for te following age groups: Tere will be an adequate number of US Soccer registered referees available in te area during te tournament or game dates to cover te sceduled games. e ave selected te following assignor to assign referees for te tournament or games (NOTE: ONLY US Soccer certified assignors may be used.): Name Ron Colaizzi (630) Curcill Ln ronald_colaizzi@cabotcmp.com (630) Oswego State Illinois Zip AVAILABILITY OF POLICE AND RESCUE SERVICE: e ave notified te local police, ambulance, and emergency rescue services of te date of te tournament or games and te times and te times and fields wic will be used for games, and ave been advised by tem tat tey will be available to render assistance if needed. e will use te following metods of contacting emergency services. TOURNAMENT OR GAME RULES - BEAVIOR: e agree tat our tournament or game rules contain provisions ensuring tat te beavior of teams, players, coaces, and speculators is appropriately controlled, including specific provisions tat--- Spell out te disciplinary measures to be imposed for te issuance of red and yellow cards or oter improper conduct; indicate wat procedures will be followed regarding protests and appeals;

5 indicate tat all disciplinary measures imposed by osting organizations sall be limited to placing restrictions upon an individual's group participation in te tournament / games; record te issuance of all red and yellow cards and oter matters involving te conduct of a team, its players, coaces, and supporters and also report tem immediately to te ome State Association and te ome club / league of te team; and state tat te ome State Association or member tereof and te ome club or league sall, except in te case of referee assault or abuse, ave te responsibility for imposing, sould circumstances warrant, additional penalties witin teir jurisdictions wit regard to any matters arising from te tournament or games. TOURNAMENT CANCELLATION: e agree tat our tournament or game rules sall state wat refunds, if any, sall be made to participating teams if all or a portion of te tournament or games is cancelled by te osting organization for any reason. POST TOURNAMENT OR GAMES REPORT: e agree tat we sall file any required Post Tournament or Games Report wit te State Association or Affiliate granting us permission to ost tis tournament or games witin 30 days after te conclusion of te tournament or games. e understand tat failure to file te report may preclude te tournament / games ost from receiving approval for any tournament / games for te following seasonal years until te report is filed. Te Post Tournament Games Report sall include te following information: te number of teams participating in eac age group (boys and girls); if a campion is determined, te name of te campion for eac group; te number of teams from eac State Association, Affiliate, oter Organization Member, or foreign country; if "Sportsmansip Awards" are given, te criteria for te award and to wom awards were given; te number of fields used for te tournament / games; te name of te sponsor, if any; and te names and teams of all players issued red and yellow cards, and details of any oter matters involving te improper or unsporting conduct of a team, its players, coaces or supporters. NOTE: Any incident of referee assault or referee abuse by a player, coac, manager, club official, or game official must be reported to te alleged offender's ome State Association, or member tereof immediately, but in no event later tan 48 ours after an incident of referee assault or abuse.
