1 Player Name: Team: LOUISIANA FIRE JUNIORS SOCCER CLUB PLAYER AND PARENT CONTRACT Congratulations on being selected to join the Louisiana Fire Juniors Soccer Club (LA FIRE) for the Seasonal Year! We hope that this will be an enjoyable year for you and your family. As part of the registration process, we want to ensure that you are fully aware of and understand the commitments the LA FIRE makes to you, and what our expectations and obligations are of you when accepting the position offered to you. Please initial the points below acknowledging your understanding of the mutual expectations between you and LA FIRE. We understand LA FIRE will provide training and coaching, along with the opportunities to participate in training, training matches, league and tournament games and State Cup. We understand that if my team advances to the regional and/or national events following the regular competitive season, I am required to attend any/all events with my team. We understand that since this is a premier competitive environment, there is a 30% guarantee of minimum playing time in all games for competitive teams C2 and below. For C1 teams, there is a 30% guarantee of minimum playing time in all training matches, league games and tournament games. There is no guarantee of playing time in tournament finals or the State Cup series games. Additionally, we understand that we will share in team expenses above and beyond the club registration/training fees to cover entry fees to league play, tournaments, referees fees, and coach travel costs associated with attending these events. The team may seek and receive donations or fund-raise to defray these expenses as well. 1
2 We understand that failure to fulfill our financial obligations to LA FIRE may result in the player being placed in Not in Good Standing status with LA FIRE which will result in the player s playing privileges being suspended until the obligations are fulfilled. Further, we understand the suspension will continue into additional seasons until the obligations are met. We understand that as a member of LA FIRE we (parents and guardians) will be requested to volunteer and assist with club functions and tournaments and that failure to do so may result in a player being placed in a Not in Good Standing status with LA FIRE. This will result in a player s playing privileges being suspended until the obligations are fulfilled. PLAYERS: As a participant on a Club Team, I agree to abide by the following Player Code of Conduct (this Code of Conduct ): 1. I will conduct myself, at all times, in an appropriate, professional and sportsmanlike manner, and will treat all teammates, competitors, officials, coaches, staff members and others involved in the Club and/or Club Events with courtesy and respect. As used herein, Club Events means any and all activities or events associated with the Club and/or a Club Team, or related to or occurring as a result of the Club and/or a Club Team, including, but not limited to, any training sessions, practices, scrimmages, matches, meetings, team meals, travel, lodging and/or charity events at facilities of the Club or at any other location. 2. I will take no action (or omit to take any action) that causes (or may cause) public embarrassment for myself, the Club, the Chicago Fire Soccer Club or Major League Soccer, or any of their respective affiliates, or that brings (or may bring) myself, the Club, the Chicago Fire Soccer Club or Major League Soccer, or any of their respective affiliates, into disrepute in the community. 3. I will respectfully follow the instructions and requests of coaches, staff members and other adults associated with the Club without hesitation or resistance. 4. I will follow all applicable local, state, federal and international laws while participating in Club Events. 5. I will abide by all applicable event, facility and tournament rules and will act in a manner that is respectful of the event that I am attending and its facilities. I will show respect for all event facilities by picking up after myself and disposing of trash as directed by the event or Club coaches and/or staff members. 2
3 6. I understand that my appearance is a direct reflection of my team and the Club. Accordingly, for all Club Events, I will dress appropriately and in the manner required by Club coaches and/or staff members. I will wear all required safety equipment, specifically shin guards, at all practices and games. 7. I will, at all times, use language that is appropriate. I will not use obscene language, gestures or curse words. I will not use slurs, phrases or gestures which denigrate ridicule or mock a person or persons based upon their race, gender, sexual orientation, economic status, nationality, country of origin or political affiliation. I will not bully or haze any teammate or other person involved with the Club and/or Club Events, and I will immediately report any bullying or hazing by myself or others to Club coaches and/or staff members. 8. I will not steal, harm or damage the property of others (soccer/training facility, hotel, lodging or mode of transportation, etc.) during any Club Event. I understand and accept that if I damage any property of others during any Club Event, I will be responsible for paying for any and all damages and repair costs that result from such damage. 9. I will not intentionally injure or harm another teammate, referee, player, coach or staff member ( person ) during any Club Event. I understand and accept that if I intentionally injure any person as set forth above, I will be responsible for paying for any and all medical costs and other damages that result from such injury. 10. I will not, under any circumstances, possess or use any alcohol, tobacco or illegal drugs, at any time, during Club Events. I will avoid situations where alcohol or tobacco use by minors and/or illegal drugs are present. If I am at a Club Event where alcohol or tobacco use by minors and/or illegal drugs are present, I will immediately notify Club coaches and staff members and leave the area if safe to do so. 11. I understand and accept that if I am dismissed from the Club or a Club Event while traveling, I may be sent home immediately at my expense (or the expense of my parent(s)/guardian(s)) by whatever means is most convenient for the Club coaches and/or staff members. I understand that violating this Code of Conduct in any respect, implied or otherwise, may or will result in consequences, including, but not limited to, game time restrictions, game suspensions, hotel room restrictions, loss of personal privileges, imposition of monetary fines, and suspension and expulsion from the Club (and being sent home at the expense of my parent(s) and/or legal guardian(s), if I am suspended or expelled from the program while away from home). All punishment and penalties will be determined in the sole discretion of the Club coaches and are final and non-appealable. PARENT/GUARDIAN: As a parent or legal guardian of a participant (my Participant ) on a soccer team of Louisiana Fire Juniors (the Club ), I agree to abide by the following Parent/Guardian Code of Conduct (this Code of Conduct ): 1. I will cause my Participant to comply with the Player Code of Conduct executed by him or her (the Player Code of Conduct ). 2. I will cause my Participant to arrive on time to all Club Events (at least 15 minutes prior to scheduled practice start times and 45 minutes prior to scheduled game start times). I will be on time to pick up my Participant from all Club Events. 3
4 3. I will honor the game because it sets a good example for my Participant. I will conduct myself, at all times, in an appropriate, professional and sportsmanlike manner, and will treat all competitors, officials, coaches, staff members and others involved in the Club and/or Club Events with courtesy and respect. I will adhere to the Club s 24 hour policy and will not attempt to contact my team coach until at least 24 hours after a game and/or team practice has lapsed. I understand that I might not always agree with my team coach s decision(s). This waiting period is intended to allow for cooler heads to prevail. If an official makes what I or others perceive as a bad call, I will refrain from making any comments. As used herein, Club Events means any and all activities or events associated with the Club and/or a Club Team, or related to or occurring as a result of the Club and/or a Club Team, including, but not limited to, any training sessions, practices, scrimmages, matches, meetings, team meals, travel, lodging and/or charity events at facilities of the Club or at any other location. 4. I will take no action (or omit to take any action) that causes (or may cause) public embarrassment for myself or the Club, the Chicago Fire Soccer Club or Major League Soccer or that brings (or may bring) myself or the Club, the Chicago Fire Soccer Club or Major League Soccer into disrepute in the community. 5. I will not coach from the sidelines. I will limit my comments to words of encouragement for my Participant and other players. I will refrain from making negative comments about Club coaches and/or staff members. I will stay away from the players bench area under all circumstances unless called over by one of the Club coaches. The 24-Hour Rule is in force when I have a comment, question or concern about a game or practice, I will wait 24-hours, and then write an to the Club coaches sharing the concern and asking to schedule a time to discuss it, if appropriate and logistically possible. 6. I understand that any parent or legal guardian who is removed from a game by an official may or will result in such parent or legal guardian also being automatically suspended for a minimum of three (3) games by the Club. Any parent or legal guardian that is removed from a game by an official two (2) times during any soccer year may or will be banned from ALL remaining games for that year. 7. I will use language that is appropriate during all Club Events. I will not use obscene language, gestures or curse words. I will not use slurs, phrases or gestures which denigrate ridicule or mock a person or persons based upon their race, gender, sexual orientation, economic status, nationality, country of origin or political affiliation. 8. I will not physically or verbally intimidate any coaches, players, referees, fans or volunteers during any Club Event. 9. I will be responsible for the actions of all my guests in attendance at Club Events. It is my responsibility to hold ALL of my guests to this Code of Conduct. A guest of a parent or legal guardian will be held to the same disciplinary standard as I am under this Code of Conduct. 10. I will not use alcohol, tobacco products or illegal drugs of any kind during Club Events. 11. I understand and accept that if my Participant is dismissed from any Club Event, the Participant may be sent home immediately at my expense, and by whatever means is most convenient for the Club and that I will be obligated to reimburse the Club immediately for any travel related expenses, including, but not limited to, the time allocated by coaches and staff members to schedule and/or participate in return travel with my Participant. My failure to reimburse the Club as set forth above may lead to the immediate suspension and/or removal of my Participant from the Club. 12. I understand that I will be obligated to reimburse and indemnify the Club for any and all damages, repair costs, cleaning costs, expenses, legal fees, court costs and civil or criminal penalties incurred by the Club as a direct or indirect result of my Participant violating any of the Player Code of 4
5 Conduct. My failure to reimburse the Club as set forth above may lead to the immediate suspension and/or removal of my Participant from the Club. 13. I understand and agree to accept any actions taken against my Participant by the Club coaches and/or staff members as a result of my Participant s failure to follow the Player Code of Conduct and understand that any actions which may be taken are final and non-appealable. 14. I will respect all decisions regarding player placement made by the Club s coaches and staff members. I will respect all decisions made by the Club s coaches and staff members during training, matches and other Club Events. Any and all questions can be directed to the Club s Director of Coaching. I understand that violating this Code of Conduct in any respect, implied or otherwise, may or will result in consequences, including, but not limited to, game time restrictions, game suspensions, and suspension and expulsion of my Participant from the Club (and my Participant being sent home at my expense if my Participant is suspended or expelled from the program while away from home). I understand that I have the right to consult with an attorney before signing this Code of Conduct. All punishment and penalties will be determined in the sole discretion of the Club s coaches and staff members and are final and non-appealable. Informed Consent: I, the parent/guardian of the registrant, agree that we will abide by the rules of LA FIRE, LSA, USYS and U.S. Soccer Club and all its affiliated organizations. My/our child wishes to participate in soccer during the season of registration. I/we realize risks are involved in my/our child s participation. I/we understand that the risk to my/our child includes full range of injuries from minor to severe, and the result could be death, paralysis, or other serious, permanent disability.
