2 Incidents in Marine Geophysical Operations MOB from Streamer Deck, No Life-Jacket or PLB worn (PERSON NEVER RECOVERED) Gun Deck, - Mechanics washed of their feet and across the gun deck during black ship Work Boat Capsize - it took 45 minutes to realise the work boat had capsized following by 2+hours to find and rescue four personnel, all personnel were wearing 275n Life-Jackets and PLB, the PLBs failed to activate FRB Capsize in Greenland during crew change, 6 MOB, all recovered in a few minutes 2
3 Incidents in Marine Geophysical Operations MOB from Capsized FRB alongside the mother vessel, a single chamber 150n Lifejacket was worn no PLB, (PERSON NOT RECOVERED) Two fatal incidents occurred caused by overreaching and falling into the sea An operator not wearing a Life Jacket was dragged overboard and killed when he got his leg caught in a winch cable An operator died following a fall down a slipway, recovery of the deceased person took 105 minutes An operator died following a fall down a slipway and was knocked unconscious, the deceased Life Jacket failed to inflate An operator died following being washed overboard, the deceased was wearing a work suit and Life Jacket 3

5 The System Comprises of 275n SOLAS approved Life Jacket DSC/AIS Personnel Locator Beacon Portable Alert and Search Station for Chase/Security vessels Work Suit Mother and Supply vessels own VHF DSC receiver and Radar Plotter Tested and evaluated inline with - IAGC Guidelines for the Selection of an Integrated Survival System for Marine Geophysical Operations 5
6 Life Jacket - smrt 275n PFD SOLAS approved 275n, One Hand quick release buckle Optional Attachment for fall prevention line Spray Hood Bladder design prevents water ingress into the person breathing space Hard Wearing Cordura material, with easy clean nylon/plastic front Full training and Inspection system provided and maintained in PMS Access panel for PLB inspection Tested to ensure that it turns and unconscious MOB Very positive reports during field trials Individual issue 6
7 PLB - smrt V100 Combines AIS and DSC Automatic Activation on entry to water ) Automatic Activation when lifejacket inflates ) Built in Redundancy for Activation Manual Activation available ) Only requires placing into operational mode once during it service life Simple Functional Testing for 5 weekly tests Initial Activation operates in Closed Loop for 5 minutes only alerting project fleet, if no return signal is received by PLB, it then goes into open loop and alerts all vessels Can be assigned to up to 8 receiving units Range from sea level to work boat antenna height 4.5m = 5+km Range from sea level to Mother vessel antenna height 27m = 10+km PLB unique numbers are issued to individuals onboard 7
