7 128 / ACCIDENTS These illnesses/injuries, which led directly or indirectly to an accident, included HAPE, AMS, chest pain, HACE, and dehydration. 3 These included inadequate anchors, uneven ropes, no knots in rope ends, inadequate anchor tethers, and rappelling past the anchor (ropes too short). 4 This category covers ski mountaineering. Backcountry ski touring or snowshoeing incidents, including those involving avalanches, are not counted in these tables. 5 These included failure to self-arrest, falling off a ledge while unanchored, rope came unclipped from slings, misuse of pulley on Tyrolean traverse, cornice collapse, and anchor failure. 6 These included lowering errors, miscommunication, ineffective belay, and no belay. 7 These included internal injuries, chest pain, torn ligaments, burns, hearing loss, digital avulsion, spinal cord injuries, collapsed lung, TBI, unspecified head trauma, eye injury, bat bite, dehydration, and torn rotator cuff. Note: Injuries are counted only once in each category for a given incident. For example, an accident that results in three broken bones will only be listed once under Fracture.
