4 The Safety Plan This Safety Plan was completed by the Fontana Little League Safety Officer. The plan was submitted to California District 71 Safety Officer Rick Fontana for approval prior submission to Little League International with the registration form. The plan will be submitted and approved by Little League International before March 11, The plan will be distributed to each team, copies will be available in the snack bar, and it will be posted the website. 9 Players and Volunteers Players All League Player Registration Data will be submitted to the Little League Data Center at Volunteers FALL President and Safety Officer will be registered and put on file with Little League International. All Team Manager and Team Coach information will be submitted to the Little League Data Center at All FALL volunteers, including Board Members, Team Managers, Team Coaches, Team Moms, snack bar workers, extra practice coaches and umpires complete a 2016 Volunteer Application and will be checked through First Advantage. The FALL Safety Officer will review all the First Advantage results to determine eligibility of the volunteers. The FALL Safety Officer s First Advantage results will be reviewed by another FALL Board Member. Offences that would exclude a volunteer from participating include, but are not limited to: Child Abuse Child Molestation Risk of Injury to a Child Sexual Activity with Child Sexual Conduct with a Minor Showing Obscene Material to a Minor FALL will provide all volunteers who clear the First Advantage screening with an ID card that identifies them as being cleared to volunteer. All volunteers will have the ID cards visible while working with the children. If the ID cards are not being worn, that person will not be allowed on the field or in the dugout. Managers need to make sure they have enough parents who have been cleared in case the manager or regular coaches are unavailable for a game. 10 Equipment Equipment Manager s Responsibilities The FALL Equipment Manager, or board member designee, is responsible for the purchasing and distributing of equipment to individual teams. The equipment will be inspected prior to being issued to the manager. The FALL Equipment Manger will promptly replace damaged and ill-fitting equipment Team Manager s Responsibilities The Team Manager will: Inspect equipment when it is received from the FALL Equipment Manager Insure issued equipment is appropriate for the age and size of the players of the team Inspect equipment prior to each game or practice Report broken, damaged or equipment that does not fit to the FALL Equipment manager. Inspect player supplied equipment for damage and insure it conforms to Little League Rules. Insure that all player respect the equipment that is issued to the team Return all equipment at the end of the season to the FALL Equipment Manager or designated board member. Player Supplied Equipment Any equipment that will be used during games or practice that is supplied by a team player must meet the requirements outlined in this Safety Plan and the Official Little League Rule Book. Player supplied equipment will be inspected by the Team Manager and is subject to inspection by the FALL Safety Officer or any other FALL Board Member. Equipment found not to be in compliance with this Safety Plan or the Official Little League Rule Book will be removed from play by the Team Manager, Team Coach, FALL Board Members or Umpire. 11 On Field Equipment Catchers Male catchers must wear cup protectors and a long-model chest protector. Female catchers must wear long or short model chest protectors. All catchers must wear chest protectors with neck collar, dangle type throat guard, shin guards and catcher s helmet. Skull cap and separate mask type helmets are not permitted. In addition to games and practice, catcher s helmet and throat guard must be worn: When warming up pitchers Catching during infield/outfield warm ups Batting Helmets Batting helmets must have all protective foam inside the helmet intact. Batting helmet facemasks are optional equipment. Helmets should be checked for cracks particularly where the ear guards meet the bill of the helmet. Helmets that have been painted or covered in stickers will not be permitted to be used as the paint can conceal cracks. Helmets will be worn: During batting practice (anywhere on the field or in a batting cage) Batting in games Running bases Pitching practice (standing in the batter s box while the pitchers throw to the catcher) Player coaching first or third bases in the coaches boxes. Balls Only Official Little League balls will be used during practices and games Bats Batting donuts are not permissible. Non-wood bats may develop dents from time to time. Bats that cannot pass through the approved Little League bat ring must be removed from play. The 2¼ inch bat ring must be used for bats in all softball divisions. Any bat that has been altered shall be removed from play. The Little League bat ring will be available in the snack bar. Fontana American Little League Safety Plan Revised:
12 Baseball Majors and below: The bat shall not be more than thirty-three (33) inches in length nor more than two and one-quarter (2¼) inches in diameter. Non-wood bats shall be labeled with a BPF (bat performance factor) of 1.15 or less; Bats do not have to have a Little League Approved notation, but the must have a BPF of 1.15 or less marked on the bat. Bats labeled Tee Ball that have the 1.15 BPF can be used at any level by Little League rules, but it is highly recommended that they aren t used above Farm level. This is recommended because they are usually made of softer material and will easily dent and become unusable. EXCEPTION: For the Little League (Majors) and below, for regular season play and Tournament, composite bats are prohibited unless approved by Little League International. Approved Composite Bats If the composite bat is not listed below, it is not approved by Little League International to be used. Use of an illegal bat will result in the ejection of the manager from the game. It is the managers responsibility to ensure that the all bats meet Little League requirements. 22 Player Personal Protective Equipment FALL highly recommends the uses of protective cups for all male players. FALL also recommends the use of mouth guards, face guards on batting helmets and heart guard chest protectors for all players. Fontana American Little League Safety Plan Revised:
23 Facilities Field Inspections The FALL Safety Officer is responsible for inspection all playing fields prior to the beginning of the season. The FALL Safety Officer will also complete the Little League 2016 Facility Survey and submit it to Little League. Team Managers and Team Coaches are responsible to inspect the playing field prior to each game. The field inspection should include hazards like: Holes, damage or uneven spots Slippery area Mound condition Damage to fences Glass, rocks and other debris on the playing field and viewing area. If unsafe playing conditions are found and cannot be repaired, these conditions should be brought to the attention of a FALL Board Member prior to the game. Field Maintenance The City of Fontana Parks Department is responsible for the maintenance of the field, lighting and park area. If a maintenance issue cannot be addressed by the Team Manager or FALL Board Members, the issue needs to be promptly reported to the City of Fontana. Snack Bar The Snack Bar Coordinator is responsible for the safe operation and supervision of the snack bar. The menu shall be posted and approved the FALL President. At least one person present during snack bar operation will possess a food handler card. The snack bar will be inspected by the San Bernardino County Department of Public Health, Division of Environmental Health Services once during the season. 25 Accidents and Injuries Medical Release Form All players are required to have a completed medical release form on file with FALL. This form will authorize treatment of the child by paramedics if the parent is not present at the time of the injury. The form will also notify the Team Manager of known medical conditions the child may encounter. The Team Manager will have the medical release forms at all games, practices and any other activity where the child s parents may not be readily accessible. The Team Manager should also consider making an additional copy of the team s medical release forms and putting it in the equipment bag. The equipment bag always makes it to games/practices, even if the manager cannot make it. It is also highly recommended that the Team Manager/Coach always have a cellular phone available at all Little League functions. First Aid Kit FALL will provide first aid kits to each team. The Team Manager will have a first aid kit readily available at all team practices and games Reporting Incidents or Injuries An incident, either on field or off field during little league activities, that causes any player, manager, coach, umpire or volunteer to receive medical treatment or first aid must be reported to the Safety Officer. This includes even passive treatments such as application of an ice pack or the need for extra periods of rest. Near miss incidents should also be reported. When reporting near miss incidents, Near Miss should be written on the top of the form. How to report injuries/incidents All injuries/incidents will be reported on the Little League Activities/Reporting form. Blank forms are included in the back of this manual and additional forms can be picked up in the snack bar. Incident reports should be completed by the end of the game/practice and must be turned into the Injury Reports folder in the Snack Bar at Bill Martin Park within 24 hours. 26 At a minimum, the following information must be completed on the Activities/Reporting form: Name and phone number of the involved individuals Date, time and location of the incident As detailed description of the incident as possible Preliminary estimate of the extent of the injury. You may contact the safety officer at any time to discuss any injury (See contact information on page 4). Injury/incident Review The FALL Safety Officer will review each incident/injury to determine if the injury/incident was avoidable. If multiple avoidable injuries are observed from the same team, the board will review the reports and determine if corrective action is necessary. Corrective action can range from counseling to manager/coach removal from position. Concussions Concussions are caused by a bump, blow or jolt to the head or by a hit to the body that causes the head and brain to move quickly back and forth. The quick movement can cause the brain to bound around and strike the skull. Concussions can be a serious injury, especially for children and teenagers. Knowing the signs of concussions and getting them treated can reduce the time it takes to recover and could even save a life. At least one coach from each team must complete the on line training course on concussions provided by CDC. The link to the course can be found on the leagues web site or directly at Recognizing a Concussion Signs observed by Coaches or Parents: Appears dazed or stunned Forgets an instruction, or is unsure of the game, score, or opponent. 27 Shows mood, behavior or personality changes Can't recall events prior to or after the hit or fall Symptoms reported by the child or teen: Headache or "pressure" in head Nausea or vomiting Balance problems or dizziness Double or blurry vision Bothered by light or noise Feeling sluggish, foggy or groggy Confusion, or concentration or memory problems Just not "feeling right" Serious symptoms of a concussion: One pupil larger than the other Drowsiness or inability to wake up Headache that gets worse and does not go away Slurred speech, weakness, numbness or decreased coordination Repeated vomiting or nausea, convulsions or seizures Unusual behavior, increased confusion, restlessness, or agitation Loss of consciousness. Even a brief loss of consciousness should be taken seriously. If any of the serious symptoms are observed, medial attentions should be seen immediately, either by calling 911 or taking the child to the emergency room If a player is involved in a collision where you think a concussion is a possibility, the player should be removed from the game. You should make the parents aware of your concerns and have them closely watch their son/daughter. If any of the signs/symptoms displayed, they should seek medical attention. The player is not allowed to return to practice or a game until they are cleared by a physician in writing. The clearance should be turned in just as the injury tracking from. 30 If an injury occurs If a significant injury occurs that requires a response from the paramedics following procedures should be followed: Contact 911 from the field with a cellular phone if possible. Have someone available to answer questions for the 911 operator who is familiar with the child and the injury. If a cellular phone is not available, send someone to the Snack Bar and call from there. o Know that while asking questions, the 911 operator is dispatching assistance to the scene. The questions are necessary to give the responding paramedics as much information as possible before they arrive. o Advise the 911 operator what side of the park will be easiest to access the injured person (Cypress Street parking lot or Juniper Ave parking lot) Notify the on-duty FALL Board Member of the injury. Team Managers or Coaches should NEVER administer any medication or apply any ointments to injuries The Team Manager, Team Coach or Team Mom should gather the player s medical release form and have it readily available for the paramedics. Have a responsible parent go to the parking lot or street to direct the responding equipment into the scene. Paramedics may have to access the field through a dugout, so the opposing Team Manager and/or Coach should gather both teams in the outfield. They should also make sure there is a clear path to the field, unobstructed by equipment and players bags. When gathered in the outfield make sure players are away from the outfield field access gate in case emergency equipment has to drive onto the field. If the parents are very upset, try to find someone to give them a ride to the hospital so the parent does not have to drive themselves. 31 Insurance Little League accident insurance covers only those activities sanctioned by Little League Baseball. What parents should know about Little League insurance The Little League Player Accident Policy is an excess coverage, accident only plan to supplement other insurance carried under a family policy or insurance provided by an employer. If there is no primary coverage, Little League insurance will provide benefits for eligible charges, up to Usual and Customary allowances for the area. A $50 deductible applies for all claims, up to the maximum stated benefits. What to do if your child sustains a covered injury 1. The Little League Baseball and Softball accident notification form must be completed by parents (if the claimant is under 19 years of age) and a league official and forwarded directly to Little League Headquarters within 20 days after the accident. A photocopy of the form should be made and kept by the parent/claimant. Initial medical/dental treatment must be rendered within 30 days of the Little League accident. 2. Itemized bills, including description of service, date of service, procedure and diagnosis codes for medical services/ supplies and/or other documentation related to a claim for benefits are to be provided within 90 days after the accident. In no event shall such proof be furnished later than 12 months from the date the initial medical expense was incurred. 3. When other insurance is present, parents or claimant must forward copies of the Explanation of Benefits or Notice/ Letter of Denial for each charge directly to Little League International, even if the charges do not exceed the deductible of the primary insurance program. 4. Policy provides benefits for eligible medical expenses incurred within 52 weeks of the accident, subject to Excess Coverage and Exclusion provisions of the plan. 5. Limited deferred medical/dental benefits may be available for necessary treatment after the 52-week time limit when: (a) Deferred medical benefits apply when necessary treatment requiring the removal of a pin /plate, applied to transfix a bone in the year of injury, or scar tissue removal, after the 52-week time limit is required. The Company will pay the Reasonable Expense incurred, subject to the Policy s maximum limit of $100,000 for any one injury to any one Insured. However, in no event will any benefit be paid under this provision for any expenses incurred more than 24 months from the date the injury was sustained. 32 (b) If the Insured incurs Injury, to sound, natural teeth and Necessary Treatment requires treatment for that Injury be postponed to a date more than 52 weeks after the injury due to, but not limited to, the physiological changes of a growing child, the Company will pay the lesser of: 1. A maximum of $1,500 or 2. Reasonable Expenses incurred for the deferred dental treatment. Reasonable Expenses incurred for deferred dental treatment are only covered if they are incurred on or before the Insured s 23rd birthday. Reasonable Expenses incurred for deferred root canal therapy are only covered if they are incurred within 104 weeks after the date the Injury occurs. No payment will be made for deferred treatment unless the Physician submits written certification, within 52 weeks after the accident, that the treatment must be postponed for the above stated reasons.
