Since joining the CROWNWeb team in October 2008, he has spearheaded the development of CROWNWeb educational events and materials, and published more than 33 CROWNWeb articles in a numerous national magazines and journals. Michael Seckman, CTT+ Training Manager Michael Seckman is the Training Manager for the CROWNWeb OCT team. Removal of Anemia Management and Mineral Metabolism attestation reporting in CROWNWeb. Maintain functionality that allows facilities to complete ICH CAHPS attestations in CROWNWeb. Integrate CROWNWeb with the QualityNet Authorization Role Management (QARM) system using 2-phase approach. 17 Why ICD-10? The World Health Organization (WHO) revises the list of International Classification of Diseases (ICD) to reflect changes in the medical and health care fields. The United States has been using the ninth revision of the ICD (ICD-9-CM) since countries including Canada, Costa Rica, Germany, China, and Australia have used ICD-10 codes since

18 Number of ICD-10 Codes ICD-10 codes uses alphanumeric categories that provide nearly 19 times as many procedure codes as ICD-9-CM. (Going from ~3,800 to nearly 72,000) ICD-10 codes provide nearly five times as many diagnosis codes as ICD-9-CM. (Going from ~14,000 to over 68,000) ICD codes are used on the CMS-2728 form to indicate the Primary Cause of Renal Failure. ICD-9-CM selections for the CMS-2728 included 82 code options. There will be 110 initial ICD-10-CM code selections. 18

19 Finalizing ICD-10 Codes for CMS-2728 CMS used the GEMS to cross-walk from ICD-9- CM codes to ICD-10-CM codes. CMS reviewed feedback provided by ESRD stakeholders and Nephrology lists of best codes. CMS held ICD-10 Listening Session for additional stakeholder input. CMS updated ICD-10-CM list based on feedback. CMS sent updated ICD- 10 code list to Nephrologists (adult and pediatrics) for additional input. CMS worked with a Panel of Nephrology doctors for a clinical level review. CMS utilized the Panel s list of recommended ICD-10-CM codes for posting on MyCROWNWeb.org. CMS utilized the final list of recommended ICD-10- CM codes in reprogramming CROWNWeb to update the CMS-2728 form. 19

20 The ICD-10 Transition in CROWNWeb Effective October 1, 2015, CROWNWeb will transition to ICD-10-CM code selections to allow facilities to comply with federal requirements. What can CROWNWeb users expect? CROWNWeb will continue to follow guidelines set by CMS CROWNWeb to contain an updated CMS-2728 form (Field 15) The process to complete the CMS-2728 form remains the same A list of recommended ICD-10-CM codes for the CMS-2728 form CROWNWeb to present the appropriate ICD code selections based on a patient s admission date 20

21 What s Changing? Not much will change when selecting ICD-10 codes in CROWNWeb. 22 Admission Dates ICD-9 vs. ICD-10 CROWNWeb is programmed to present users with ICD-9-CM or ICD-10-CM code selections based on the patient s admit date. Patients admitted BEFORE October 1, 2015 Users will be presented with ICD-9-CM codes Users can only select and submit ICD-9-CM codes on the CMS-2728 form Patients admitted ON or AFTER October 1, 2015 Users will be presented with ICD-10-CM codes Users can only select and submit approved ICD-10-CM codes on the CMS-2728 form 22

23 ICD Selections in CROWNWeb How will users select ICD-10 codes in CROWNWeb? CROWNWeb currently provides users with 3 ways to identify and enter a Primary Cause of Renal Failure; this will continue with the implementation of the ICD-10-CM codes Will CROWNWeb accept valid ICD-10 codes not on the Code List? Only ICD-10-CM codes from the Code List can be accepted in CROWNWeb at this time CROWNWeb will present an error message if an invalid ICD-10-CM code is entered 23

24 Updates to ICD Selections in CROWNWeb Will the ICD-10-CM list for Primary Cause of Renal Failure provided for the October 1, 2015 implementation be the final list? No. The list will be reviewed and updated as needed to help ensure that the valid diagnosis codes for Primary Causes of Renal Failure are reflected CMS recommends using the ICD-10 Code Book/Manual to select the appropriate code for the Primary Cause of Renal Failure 10-CM-and-GEMs.html How will additional ICD-10 codes be accepted in CROWNWeb that are not on the List? A Help Desk ticket to Tier 3 should be generated if an ICD-10 code noted on the CMS-2728 form is not accepted in CROWNWeb In a future release, CROWNWeb will be re-programmed to accept any valid ICD-10 code so the Code List will become a reference tool only 24

25 Accessing the Recommended ICD-10 List Will the CMS-2728 instructions on the Reports screen in CROWNWeb have a list of the recommended ICD-10-CM codes? The new CMS-2728 form (2728-U3 (08/15)) released in support of the ICD-10-CM code implementations will not include the ICD-10-CM codes in the instructions The old CMS-2728 form that supported ICD-9-CM codes (2728-U3 (03/06)) will still have the ICD-9-CM codes in the instructions Where can I access a copy of the recommended CMS-2728 ICD-10-CM codes? Users will be able to access a copy of the recommended ICD-10-CM codes for Primary Cause of Renal Failure via 25
