It is recommended to stay in the same level for fall/spring to get the most out of the program year olds are the exception if the child has aged up. Discounts and Payment Info If you are a full time Racquet Club Member, you may deduct $10 from your fee total. If you have siblings in the program, you may deduct $10 from your fee total. If you are taking 3 or more classes a week, you have option to pay ½ tuition now and ½ by 2 months into the semester. For the half payment option, info@balletchildren.com for a form. Important Dates May 5: City wide auditions for ages 12 - up, as well as students moving to upper Level 3: Auditions for TBT Junior Company. May 14-19: Parent observation in the classroom to see teachers instruct students, for all levels. May 26: Children s Theater Festival at Rhodes featuring TBT Junior Co June-August: SUMMER CLASSES and CAMPS ages years and adults! June 4-8 and June 25-29: CBT/TBT Ballet and Contemporary Intensive at RCLUB for Levels SK - 12th grade and adults. July 30-Aug 4: CBT/TBT Ballet and Contemporary Dance Camp at Hutchison for SK-12th grades and young adults. Note: It is required of Levels 2-6 who plan to be in the 2018 Nutcracker to attend at least one week of the intensive weeks or attend Hutchison camp so that we can prepare students for their auditions and choreography. - continued on next page -

2 CBT Spring Ballet Recital, The Gala Performance and More. April Spring Recital for Levels PK - Level 2 of Racquet Club, CMDS, SAA, Roulhacs and Second Baptist students will feature the Twelve Dancing Princesses and the fun and beautiful Les Fille Malle Gardee". These students will have one recital practice right before their show day. Your child must be in a PKJK class or older to participate (2.5-3 Saturday class at Racquet Club doesn t participate in recital). CBT Students and TBT Junior Company of Levels 3-6 (with a special guest appearance of Level 2 Tuesday contemporary class) will also perform a Gala Performance featuring Les Fille Malle Gardee coached by all faculty and Mary Van Dyke as well as original contemporary and modern dance from Jade Primicias, Alyssa Lucero, and Erin Walter. Following the Gala Performance there will be a formal reception. The $65 recital fee has already been added into spring tuition price. This covers stage rental/technical costs, recital practices, two parent tickets (given at theater the dress rehearsal or the show day), and your child s pull on tutu skirt which she will keep (older students use a CBT costumes that you return). All children should wear a class leotard, pink tights, and hair in bun, and ballet shoes for recital. Contemporary dancers may need black yoga pants. 2 of 8 SUMMER Save the date! Camps and classes for years and adults are great fun with Fairy Ballet Camps for the little ones, and for children and teens the CBT Dance Camp: ballet, jazz, modern, pointe, classical choreography and a stage performance. Memphis Youth Musical Theater presents Alice in Wonderland in a Week and Wizard of Oz in a Week performance camps. 8 8 of 8 WAIVER OF LIABILITY, INDEMNITY AGREEMENT, AND ASSUMPTION OF RISK In consideration of permission to use, today and on all future dates the property, facilities, and services of Children s Ballet Theater, Memphis Youth Musical Theater, Racquet Club, Christ Methodist School or Church, Roulhacs, St Agnes, or Second Baptist (CBT programs) I, on behalf of myself, my heirs, personal representatives, or assigns, do hereby release, waive, discharge, and covenant not to sue, CBT programs, its directors, officers, contract workers, volunteers, independent contractors, and agents from liability from any and all claims arising from the ordinary negligence of any agent of Children s Ballet Theater or any of the aforementioned parties. This agreement applies to 1) personal injury (including death) from accidents or illnesses arising from the participation in our Ballet School activities including, but not limited to, organized activities; classes; observation; performances at GPAC theater, Buckman Theater or any other theater; and individual use of facilities, premises, or equipment; and to 2) any and all claims resulting from the damage to, loss of, or theft of property. Indemnification and Hold Harmless: I agree to HOLD HARMLESS AND INDEMNIFY Children s Ballet Theater from all claims resulting from negligence and to reimburse them for any expenses in investigating and defending a claim or suit if my claim is withdrawn, or to the extent a court or arbitration determines that Children s Ballet Theater is not responsible for the injury or loss. Severability and Venue: The undersigned further expressly agrees that the foregoing waiver and assumption of risks agreement is intended to be as broad and inclusive as is permitted by the law of Tennessee and that if any portion thereof is held invalid, it is agreed that the balance shall, notwithstanding, continue in full legal force and effect. Likewise, I agree that if legal action is brought, it must be brought in Memphis, Shelby County, Tennessee. Acknowledgement of Understanding: I have read this waiver of liability and indemnification agreement and fully understand its terms. I understand that I am giving up substantial rights, including my right to sue. I acknowledge that I am signing the agreement freely and voluntarily, and intend my signature to be a complete and unconditional release of all liability to the greatest extent allowed by the State of Tennessee. The undersigned parent and natural guardian does hereby represent that he/she is, in fact, acting in such capacity, has consented to his/her child or ward s participation in the activity or event, and has agreed individually and on behalf of the child or ward, to the terms of the accident waiver and release of liability set forth in Section 1. The undersigned parent or guardian further agrees to save and hold harmless and indemnify each and all of the parties referred to in Section 1 from all liability, loss, cost, claim, or damage whatsoever which may be imposed upon said parties because of any defect in or lack of such capacity to so act and release said parties on behalf of the minor and the parents or legal guardian. I hereby waive any claim of liability on behalf of Children s Ballet Theater programs, for any efforts to obtain treatment from a doctor or hospital for my child in event that my representative or I cannot be reached. IMPORTANT - REFUND POLICY: 75% refund is given if a student withdraws within 24 hours prior to a second class. No refunds will be given after this time except in the case of a physician-verified illness. There is no pro-rating and no makeup classes. Tuition is not transferable to another session. Students cannot attend class until full payment is made. There is a $30 fee for cancelled checks. CBT reserves the right to cancel classes due to inclement weather conditions or teacher illness. In the event of a weather condition, call the CBT office one hour prior to your class time at for a recorded message. There are no makeup classes for weather conditions.
