1 Hello Turkey Foot Parents and Students, I am sure that the school year will be full of surprises and exciting moments throughout the school year. For the past eight years I have been a part of some great moments here at TFMS. Our athletic programs have excelled in several sports including cheerleading, basketball and this year lacrosse. We have had football teams advance to their conference championship games and archers compete in the state tournament as well. We have enjoyed fabulous runners and girls who leave it all on the court during volleyball season. All of our successes have only been possible with a supportive administration team, incredible athletes, extraordinary support from our parents and dedicated coaches who give up a bunch of quality time planning practices and watching films to prepare for games. There is a middle school physical form from the KHSAA and any middle school athlete MUST have their physical on this middle school form. If your child will be completing in high school sports, the athlete must have a high school physical complete. NOTE: The middle school can t accept the high school form and the high school can t accept the middle school form. The entire packet can be found on our webpage with the physical attached for the middle school. Remember that 6 th grade students are prohibited from participating in high school sports per the KHSAA. Please be aware that all paperwork MUST be submitted before the student athletes can tryout or participate in any sport. This includes a student who is playing at the high school level. If all paperwork is not received before the first meeting date then the student athlete MUST sit out until all paperwork is received. Please make plans to get your physicals before July 15 th if possible. Physicals are good one year from the date on the physical the physician signs. Every year there is a problem with parents/guardians not turning paperwork on time. There are many reasons for that but I can t put your child in jeopardy. Remember if ALL paperwork is not received then the student athlete will need to sit out until it is received.

2 Also a copy of an insurance card is also needed prior to tryouts/participation. If you do not have insurance currently, there is information in the packet on how to obtain insurance through K&K Insurance Company although you can purchase insurance through any carrier you choose. If you have any questions regarding the insurance in the packet, please see me before purchasing the insurance. Athletics are challenging and rewarding for the student athlete. Sports teach children how to work with one another and build commitment and friendships along the way. I encourage each of you to support your child and the team in their efforts on an off the court or field. Whenever they participate, I hope that we can be a community of adults that can support our young athletes as they do their best to represent their team and school. We are proud of each and every one of our student athletes and I hope the seasons brings many successes. 3 Turkey Foot Middle School 2016 Summer Athletic Information Turkey Foot Middle would like to pass information along to you regarding athletic programs that will begin over the summer of The following contains information regarding football, cheerleading, cross country and volleyball. For all sports, all athletes will need to have an athletic packet completed and turned in prior to joining a team or even tryout for a team. The athletic packet, which includes important information that parents and athletes need to be familiar with, contains a liability waiver, an emergency medication form, and a KHSAA physical that we MUST have before participation. We will also need a copy of an insurance card as well. THIS IS IMPORTANT INFORMATION REGARDING THE PHYSICALS. All sixth graders need a physical once they enter middle school. The nurse will accept a physical on the KHSAA form that is needed for athletes as well. However, please note that the only physical form we can accept is the official KHSAA form. Athletic packets will be available on a table inside the front door of the school during the summer or on the website at There will be paperwork turn in dates here at Turkey Foot on July 12 th and 14 th from 5:00 to 7:30. You are encouraged to drop off your athletic packets at that time to make sure the entire packet is completed prior to the beginning of the sports seasons. Football- The first football practice will be July 25 th from 5:00-7:00 on the football field on campus. It is highly recommended that athletes arrive prior to that. Players must be ready and on the field at 5:00. Athletes can pick up a helmet and meet the coaches on the paperwork turn in night on July 12 th or 14 th. If you have your own helmet, please make sure that you have certification that it has been reconditioned prior to the first practice. 4 Cheerleading- There will be a meeting regarding cheerleading on Thursday July 14 th at 6:30 in the cafeteria at Turkey Foot. Tryout clinics will begin on July 18 th and 19 th from 9am to noon each day. Tryouts will be held on July 20 th starting at 10 am. Tryouts and the clinics will be held at Premier Athletics in Park Hills. Team selection will be announced on Friday July 22 nd on the school website. Cross Country- The first cross country practice be on Monday July 18 th on the track at Dixie Heights High School at 6pm. Most of the cross country practices will take place in the morning. Information can also be found at the school website page Volleyball- Volleyball tryouts will be held on July 21, 22, and 23 rd from 6-8pm at Turkey Foot. Students trying out should have knee pads. Unfortunately this is a sport where some girls will not make the team. The league is a competitive league and we normally keep about twelve girls on the team. 5 More INFO from KHSAA Beginning in , the following limits have been approved through the regulatory process and will be in effect: A student that turns Fifteen (15) years of age prior to August 1 of the current school year shall not be eligible for interscholastic athletics in Kentucky in competition against students exclusively enrolled in grades eight (8) and below; A student that turns Fourteen (14) years of age prior to August 1 of the current school year shall not be eligible for interscholastic athletics in Kentucky in competition against students exclusively enrolled in grades seven (7) and below; and A student that turns Thirteen (13) years of age prior to August 1 of the current school year shall not be eligible for interscholastic athletics in Kentucky in competition against students exclusively enrolled in grades six (6) and below; Is there a specific middle school physical form? Yes. KHSAA Form MS01 is posted on the KHSAA website. Local schools/districts/associations/groups of schools may supplement that form with additional material but must use the MS01 for the physical examination. If a student plays middle school and high school athletics, are both middle school (Form MS01) physical form and the high school (Form GE04) required? Yes. Both forms are needed since wording on the permission form is necessarily different. The information on the physical part from doctor is the same, but the parent permission/information forms are different and likely always will be for legal reasons. This part would have to be filled out separately for a student who played both middle and high school athletics. You could attach one to the other (if middle school form is completed, one could attach the doctor s part of the middle school form to the high school form) as a grandfathered student, 7th or 8th grader, who plays up to the high school level. Will the two forms eventually be combined for students that play both levels? No, not planned at this time. Information cannot be combined for regulatory and legal reasons. Parent permission information is completely different and entry portion (years in school, etc.) are not valid at middle school level. Does this now require all students to get new physicals on the new form or when the physicals expire? Beginning the first day of the school year, no other form is valid. Beginning in , physicals must be completed on Form MS01. Does the 6th grade physical that is required suffice for the middle school athletics physical? No. The 6th grade physical that is required in no way covers the same elements as an athletic physical. An entirely new physical form (MS01) would be required to be completed. It would be up to the approved individual that performed the physical if the requirements in MS01 required a new visit from the patient, or could be completed from existing patient records.
