HHMS Gym Please check the HHMS website for more tryout information and required paperwork prior to the tryout date. The attached forms are the ONLY acceptable forms. No student will be allowed to tryout for any team without submitting the proper paperwork. All forms must be filled out completely and are due on or before the tryout date.

2 Hammond High Magnet School Lady Tornado Volleyball 2018 Season March 14, 2018 Dear Student and Parent/Guardian, The 2018 Lady Tornado Volleyball season is quickly approaching. Tryouts for the 2018 team will be held Wednesday, May 9, 2018, at the HHMS gym from 3:45 P.M. - 5:45 P.M. Participants are required to wear proper athletic clothing and shoes. Please make arrangements for pick-up at 5:45 P.M. sharp. In order to tryout for volleyball, a copy of your current physical must be submitted on or before the day of tryouts. If for any reason this tryout cannot be attended, please let me know prior to the tryout date. Enclosed in this packet are the following items: tryout application, paperwork checklist, LHSAA medical history form, participation and permission form, substance abuse contract/consent form, drug screening disclosure form, concussion statement, and itemized budget form. All the items listed on the paperwork checklist must be completed fully and submitted to Coach Babin on or before the day of tryouts. For all girls who make the team, there will be a MANDATORY parent meeting Monday, May 14, 2018, at 5:45 p.m. in the HHMS cafeteria. The first payment of $ will be due at this meeting. 4 Lady Tornado Volleyball Itemized Budget Agreement Payment/Dates: Checks or money orders are only made payable to HHMS Volleyball. Some of the fundraisers will go towards your daughter s payments, and some will go towards the team. First payment of $ must be paid by Monday, May 14, 2018 Agreement to Pay for the Itemized Budget: I agree that the final cost of the budget will be determined at a later date for all new players and old players for the 2018 Lady Tornado Volleyball season. The practice gear set includes a practice jersey, practice tights, knee pads, team t-shirt, socks, polo shirt, shoes, and warm-up jacket. All of the clothing is for my daughter to keep. The estimated budget is usually around $ (This includes all gear, tournament fees, transportation, insurance, meals, etc). Players will have opportunities to raise money to pay their fees. Even if my daughter quits the team or is asked to leave the team for any reason, I will be held responsible for the remaining amount owed.
