1 Dancer Information and Requirements 2018 Mini-THON Dancer must be stay on their feet for the duration of the event on Friday, February 23, (Doors open at 12 PM/Event will begin at 1 PM and last until 9 PM) Each Dancer must raise a minimum of $100 for the Four Diamonds Fund to participate in Mini-Thon. Checks made payable to Four Diamonds Fund Please put Wood and student s last name in the MEMO of the check. All pledge money and checks are due to your Mini-THON committee member on February 23, This is the money you raised for the Four Diamonds Fund. Dancer must hand in their Commitment Form, Permission Slip with Shirt Size and $10.00 registration fee in a labeled envelope to your Mini-THON committee member by January 25th. ( must be a cash payment) Dancer can bring in pledge money as often as needed. Every time a dancer brings in their money, they should bring a copy of their pledge card as well. Money can be handed into Ms. Murphy in room 236 All dancers may sign up for, and participate in any number of games/activities throughout the 8 hours of Mini-THON. Some competitions will necessitate sign-ups several weeks in advance. The Dance competition, Basketball and Lip Sync Battle etc. Please dress appropriately for Mini-THON. Shirts provided must be worn along with active attire, socks, and sneakers. Freshmen wear yellow, sophomores wear Green, Juniors wear Pink and Seniors wear blue. Do not cut your shirt, this is not appropriate, this includes but not limited to sleeves and midriffs. Be creative with your attire, this year s theme is Disney. 2 2018 Mini-THON Dancer Commitment/Permission Form Dancer Name(s) Extra lines for siblings Shirt Size Cash or Check Homeroom Number/ Year Dancer Agreement: I will be on my feet for the 8 hours of Archbishop Wood s Mini-THON on Friday, February 23, 2018 from 3pm to 10pm with the exception of a medical or family emergency. I will participate in all games and activities in good sportsmanship and spirit. I know that I am representing myself, my class, and my school, and therefore I will act with dignity and respect in all aspects of the event. Dancer Signature: Dancer Signature: Choices for shirt sizes: Small, Medium, Large, XLarge and XXL*, XXXL* *XXL and XXXL is $2.00 extra, total registration fee is $12.00 Registration fee: $10.00 (cash only) Parent/Guardian Permission Form: I hereby grant my child permission to take part in Archbishop Wood s Annual Mini-Thon event. I have made Ms. Murphy aware of any medical issues that may complicate this activity and if so give permission for Ms. Murphy to seek medical treatment necessary including but not limited to First Aid or call Emergency Medical Services. I do not hold Archbishop Wood and the Four Diamonds Fund accountable for any accidents, injuries that could be sustained before, during or after the event. I cannot seek accountability for injuries from reality or property affiliated with the Mini-Thon event not limited to injuries sustained in the blow up obstacle course, basketball, or glow sticks. Dear Parent/Guardian: Photos, videotape footage and personal interviews with Archbishop Wood students who are involved in various school related activities are often used as part of the district s community relations efforts (e.g. district publications, local television programs, newspapers, and on the school district s website). By signing below you are granting permission to Archbishop Wood to photograph or videotape the person(s) named above. The photographs would be used in school related publications, such as the district calendar, newsletter, district website, yearbook and local media. Archbishop Wood High School is sincere in its commitment to keep the public informed of the programs, activities, and events that are taken place in our schools. Without your permission, your child cannot be part of this endeavor. However, in situations in which large groups are participating, it is impossible not to photograph certain students.
