Copy of most recent Report Card 9. $25 Tryout Clinic Fee (Due April 6th) OMS CHEERLEADING TRYOUT PROCEDURE 1. To be eligible for tryouts you must: a. Attend interest meeting Tuesday, March 20 at 6:00 pm OMS Library b. Have a completed application packet turned in on time. (April 6th) c. Completed medical forms (Physical, concussion statement, cardiac arrest) d. $25 tryout clinic fee 3. Tryouts will be held in the OMS Gym April 9th-12th. Each candidate must wear tennis shoes, shorts, and a shirt, and have her hair secured and off the shoulders. Wear OMS colors if possible.. TRYOUT DATES and TIMES April :30-5:00 (Learn cheer, dance, jumps, and stunting) April 12 th 3:30-????? Try-out (potential cheerleaders will be dismissed at the conclusion of their tryout, therefore times will vary) 4. PARENTS OR FRIENDS WILL NOT BE ALLOWED AT THE PRACTICE OR TRYOUT SESSIONS. 5. Tryout results will be posted on the Oakland Middle School Cheerleader Webpage by Thursday night at 9:00 pm. 6. Tryout scores will consist of the following three parts: Tryout(60), Teacher Evaluation (30), Essay (10) Entrance (energy, spirit, enthusiasm, smile) Dance (knowledge, sharp, facials, confidence, coordination) Jumps (technique, toes pointed, height, arm placement Standing Tumbling (none-0, bhandspring- 2, tuck-4) 10pts 15pts 10pts 4pts

3 Running Tumbling 6pts (roundoff-2, roundoff bhandspring-4, roundoff series- 5, roundoff w/ tuck- 6) Cheer (loud, smile, sharp, knowledge, motions, Crowd appeal, enthusiasm) 15pts Total- 60 pts (will take average from 3 judges) Teacher Evaluation 30 pts (average from 3 teachers) Essay 10 pts

6 OMS CHEERLEADING CHEERLEADER AGREEMENT FORM I agree to be on time & ready to practice with a good attitude. I agree to represent my team and OMS in a positive manner- both in school & out of school. THIS INCLUDES SOCIAL MEDIA! I agree to respect my coaches, captains, & teammates with my words & my actions. I understand that if we compete, it is a possibility that I could be an alternate. If I am, I will still support & attend alongside of my team at all competition practices, events, & fundraisers. I understand that grades are important & if I do not maintain an average of 80 or above on a report card I will be removed from the team. I understand that all practices (both at Premier & OMS) are mandatory & I am expected to be there. If I miss a practice (unexcused), I will sit out at the next game. I understand that 3 unexcused absences may result in dismissal from the team for the school year. I understand that I am expected to condition at home, as set out by the attached schedule and that I will keep an accurate log with signatures. I understand that I am expected to encourage my teammates. It is my job to prevent drama, not create it. I understand OMS service hours are a required part of OMS athletics. I understand that all fundraising is mandatory for the season. Also, that all funds will go to the general team fund. I understand that I will participate in all spirit preparation days set forth by OMS including, but not limited to weekend or nightly meetings. I understand that it is mandatory that I attend all days of Premier Summer camp (May 29 and 30; June 11-13). I understand that my parent(s) and myself agree to assume the risk of physical strain and injury that are inherent to the sport. I understand that I will participate in all regular season and tournament games including, but not limited to out of district and region locations. I understand that there will be summer team building functions that I must attend. I understand that I must support the decisions set forth by the coaching staff, Athletic Director, and OMS Admin on all matters concerning the wellbeing of the team. I agree to return all OMS uniforms and equipment at the conclusion of the season (dry cleaned) I understand that if I do not follow the commandments of Cheerleading (as mentioned above), consequences will follow. 7 OAKLAND MIDDLE SCHOOL CHEERLEADING PARENT AGREEMENT FORM I understand & agree to pay the financial obligations that this team requires, on time. I understand that it is my responsibility to make sure my daughter/son has a ride to/from practice, games, fundraisers, & competitions. I understand that it is important that she arrives/is picked up promptly. If she is picked up more than 10 minutes late, she could be dismissed from the team (after the 3 rd time). I agree to do my best to maintain a level of respect between my daughter, her teammates, the coaches, & fellow parents. This includes refraining from gossip, badmouthing, & an overall bad attitude. I understand that all practices (both at OMS & Premier,) games, service hours & fundraising events are mandatory & my daughter/son is expected to be there. I understand that should we decide to compete, it is a possibility that my daughter/son may be an alternate. This only means she/he will not be on the mat at competitions. She/he will still be expected to support, attend & fundraise with the rest of the team. I understand that more than one F on a report card will result in dismissal. A D on the report card with result in sitting out until the grade has been raised. I understand that any disrespect, unnecessary drama, OSS, suggestive photos/language on social media, or any other action that brings discredit to this team will result in dismissal. 13 OAKLAND MIDDLE SCHOOL CHEERLEADING FINANCIAL OBLIGATIONS - Premier Stunt Camp Fee $75 (due April 17th). Make checks out to Premier Athletics. Turn in to Coach Crutcher. - Premier Practice Fee $115 due May 2 nd at first Premier Practice ($30 registration fee (required by ALL cheerleaders even if you are already enrolled at Premier), $85 monthly fee). This will need to be handled through Premier Athletics. If you need help please call Premier and speak with Lisa. - Premier Routine Camp Fee $165 (due May 14). Make checks out to Premier Athletics. Turn in to Coach Crutcher. - Commitment Fee $350 (due by June 6 practice) (This covers camp clothes- 3tops, 3 sports bras, 3shorts, 1 brief / bows/uniform rental) -Clothing/Equipment bill (due July 31) (Each cheerleader will receive an individualized Varsity invoice- this invoice will be personalized to what your cheerleader needs- i.e. warm up, bloomers, socks, competition bow, shoes, rain jacket, backpack etc.) -Cheer $40 (due September 1- make check out to OHS Cheer) -Premier Athletics $85 Monthly (mandatory May- Feb (except during the off period)) - Nationals bill (due December 1) (Each cheerleader will receive an individualized Nationals statement- this invoice will be personalized and have specific fundraiser money discounted off the total costs.) * Fundraising: Cookie Dough- Funds raised by each cheerleader will go towards their individual Nationals statement. Car Washes- Funds raised from ticket sales will go towards their individual Nationals statement. Funds raised while at the event will be spilt between the team to go towards Nationals. HomeComing Dance- Funds raised will be allocated to general funds. Give-Back Nights- Funds raised will be allocated to general funds. Pancake Breakfast- TBD Grub Hubs- TBD Sponsorships- Funds raised will be split between the team to go towards Nationals. **Other fundraisers are a possibility***
