2 UNDERSTANDING WORK AT HEIGHT WHY USE FALL PROTECTION? Falls from heights are the single biggest cause of death or serious injury in the workplace. For businesses whose staf and maintenance teams need to work at height quickly and efectively, fall protection is increasingly important. Health and Safety legislation and the controlling organisations are enforcing stricter rules and best practice for safe access and working at heights. The regulations noted here apply for New Zealand; similar guidelines are used globally. Please check with your country s Health & Safety Act for speciic details about your obligations when working at height. WHO IS RESPONSIBLE? To meet the requirements of the Health and Safety in Employment Act 1992 and Regulations 1995, Reform 2015, an employer shall take all practicable steps to ensure where any employee may fall more than three metres: Means are provided to prevent the employee from falling. Any means so provided are suitable for the purpose for which they are to be used. Where there is a possibility of serious harm from a fall of less than 3 metres, fall protection is still needed. Consideration should also be given to situations where a person may slide down an inclined surface before reaching a point at which the fall can occur LEGISLATION & COMPLIANCE The requirement for fall protection is deined by law, and to guidelines set out by safety standards authorities. The Ministry of Business, Innovation & Employment publishes the Best practice guidelines for working at height in New Zealand. This document provides practical guidance to employers, contractors, employees and all other engaged in work associated with working at height and how they can meet their obligations under the Health & Safety in Employment Act 1992 and its associated reforms and regulations. To meet the requirements of the Health and Safety in Employment Act 1992 and Regulations 1995; Principals, PCBU s and their agents such as architects and engineers have a responsibility to ensure that the project is designed to be erected, used and maintained without putting persons at risk of serious harm. Those who own, lease or use buildings or plant have a responsibility for the safety of those involved in its maintenance and repair. Areas that require regular service and maintenance should be provided with permanent safe access and work platforms. In less frequented areas, permanent anchorages for scafolding or fall arrest systems may be appropriate. Persons with control of places of work should provide training or induction procedures that will make outside contractors aware of the hazards in the area where they are to work. DUE DILIGENCE Is where a company and or individual has done everything within reason to reduce and or eliminate the hazard or risk associated with a job. It is not only about meeting legislation, companies and or individuals are discovering that they must go beyond the current legislated minimum to properly protect the worker and subsequently protect themselves. FALL PROTECTION STANDARDS AND BEST PRACTICE GUIDELINES These set the standard for the manufacture of equipment and application of safety systems. Fall protection systems should conform to the relevant Safety Standards. Scope, Purpose, Application, Exceptions 1.1 Scope - This standard establishes requirements for the performance, design, marking, qualiications, instructions, training, inspection, use, maintenance, and removal from service of connectors, full body harnesses, lanyards, energy absorbers, anchorage connectors, fall arresters, vertical lifelines, and self-retracting lanyards comprising personal fall arrest systems for users within the capacity range of 130 to 310 pounds. 1.2 Purpose & Application This standard addresses only personal fall arrest systems incorporating full body harnesses This standard addresses equipment for personal protection against falls from heights and applies to the manufacturers, distributors, purchasers, and users of such equipment Body belts, window cleaner belts, chest waist harnesses and chest harnesses when used for body support or work positioning are not addressed by this standard Before any equipment shall bear the mark Z359-1 all requirements of this standard must be met. 1.3 Exceptions The requirements of this standard do not address the Construction Industry (SIC Division C), Window Cleaning Belts, & Sports-related Activities. A10-14 ANSI Safety Requirements for Construction and Demolition Operations 1. Scope & Purpose 1.1 Scope - This standard establishes performance criteria for the manufacturer and use of body belts, harnesses, lanyards, lifelines, rope grabs, and shock absorbers for construction and demolition. It does not include linesman body belts and pole straps, window washers belts, or safety ladder belts. 1.2 Purpose - This standard details fall arrest/restraint systems, equipment and methods that shall be used when an employee is exposed to a fall hazard. 3

4 WORK AT HEIGHT TECHNIQUES THE HIERARCHY OF HAZARD CONTROLS Work at height is changing rapidly. Selecting the right method of fall protection is no longer a case of just choosing a fall arrest harness and hoping there will be an anchor point nearby. Now, the aim is always to minimise the risk of working at height and to minimise the consequences of any fall. A hazard assessment shall be carried out for all work at height. People working at height must now be trained and competent, with the order of controls as shown here; Can the hazard of working at height be eliminated? Where possible, if the job can be done eliminating the need for working at heights by working on the ground, this is the best option. However, elimination is not always possible. Can the hazard of working at height be isolated? Isolating is the next method of control of work at heights and includes using safe working platforms, guardrail systems, edge protection, scafolding, elevated work platforms, mobile scafolds and barriers to restrict access. In many cases PPE equipment will be required to erect the isolating components, or be worn in conjunction with isolating controls. Can the distance and impact of a fall be minimalised? If neither elimination nor isolation are practicable then steps should be taken to minimise the likelihood of any harm resulting from falling. This means considering the use of work positioning systems or travel restraint systems, fall arrest systems, safety harnesses, industrial rope access systems and soft landing systems. THE HIERARCHY OF FALL ARREST The hierarchy of fall arrest systems is a decision making tool which places the diferent methods of fall protection in order of preference according to the degree of risk involved, just as it is with the hierarchy of hazard control, Eliminate, Isolate and Minimise. Working methods that are classed as higher up the hierarchy are preferred to those lower down, e.g. preventing a fall (fall restraint) is preferable to allowing a fall to occur in the irst place (fall arrest) Fall arrest systems and their related safety harnesses, lines etc ( with the exception of fall restraint and restrained fall) are designed to catch and hold a person in the event of a fall. In every case, the particular hazards associated with each job and location needs to be assessed before selecting equipment for the least hazardous fall arrest protection. This hierarchy is explained here in order of safety preference; Fall Restraint (or Total Restraint System) Preventing the possibility of a fall by removing the risk is preferable when working at heights. Fall restraint systems such as travel restraint devices will actually prevent a worker from falling over the edge because they physically cannot reach the edge, allowing access to an unprotected edge while preventing the operator from falling. Restrained Fall Any fall where the person sufering the fall is under less than the full inluence of gravity due to the action of a restraint device such as a pole strap, inertia reel, or is sliding down a slope of greater than 15degrees. Limited Freefall Working with risk of falling up to a maximum distance of 600mm. The use of inertia reels or short lanyard length that reduces the total fall distance to less than 600mm. 1,8 m height of person +1 m safety factor Freefall (Fall Arrest) Any fall or part of a fall where the person sufering the fall is under the unrestrained inluence of gravity over any fall distance, either vertically or on a slope on which it is not possible to walk without the assistance of a handrail or restrained by a safety line. A freefall is a fall of between 600mm and maximum 2m. Work Positioning Enables a person to work supported in a harness under tension in a way that a fall is prevented. Generally the arrangement allows for the worker to maintain a stable position and to work hands free while completing the task. The harness arrangement should not allow a fall of more than 600mm, which is generally achieved using short lanyards or work positioning lanyards. It is advisable to include Fall Restraint or Fall Arrest systems as part of safe Work Positioning. Rescue 15 Limited Free Fall with 2m Lanyard It is essential during all work at height that a suitable rescue plan is in place to recover an incapacitated person in the event of a fall, and that they are rescued quickly. A rescue plan normally includes having a speciic rescue kit available at all times and ensuring all users are trained and competent in the use of this equipment. 4

5 FALL DISTANCE & FALL FACTOR When setting up fall arrest systems, fall factors and fall distance are critical factors to be considered. With the equipment set up to reduce the shock to the harness of less than 6kN a maximum fall factor of 1 is critical. The fall factor is the length of the fall, divided by the length of the lanyard. For best safety, always anchor above the shoulder (Fall Factor 0) whenever possible. Worst case scenario, free distance below working surface must be 6.2m (see drawing below) for fall factor of 2. A fall factor of 2 is very dangerous and not recommended but may be only option in some circumstances. 13 HEIGHT SAFETY EQUIPMENT CHECKLIST Personal Safety is Important! Working at heights is dangerous. Only trained & qualiied persons should use this equipment. For correct use of this equipment and fall protection safety, follow these essential safety guidelines; Manufacturer s Instructions; read and understand the Instruction Manual and Fitting Guide included with the equipment, before use. Training; ensure this Height Safety equipment is prescribed/itted/attached according to current AS/NZS and Department of Labour guidelines and that you are adequately trained in its use. Correct Selection; always be sure the equipment selected is correct for the purpose intended as advised and selected by qualiied & competent persons. Repairs & Modiications; do not add to, adapt or repair using components that are not recommended or approved by the manufacturer. Return equipment to manufacturer for servicing. Recertiication; all equipment must be recorded, inspected and tagged to current safety guidelines by a competent height safety specialist with the relevant qualiications. Pre-use Checks: always check equipment carefully before every use for any deterioration-do not use equipment if there is any deterioration, damage or doubts about its structural integrity. Lifespan; check the label and do not use equipment beyond the manufacturers speciied expiry date for safe working lifespan. Storage; protect your equipment from dust, dirt and liquids, store in a dry, cool area with good ventilation and away from direct sunlight. Dry any damp gear away from direct sunlight before storage. Impacted Equipment; immediately withdraw from use any equipment that has been impacted or been used to arrest a fall. Rescue Plan; always prepare a rescue plan for every worksite and work undertaken. For work at height, it is essential that in the event of an uncontrolled fall the helmet is retained on the head and can take multiple impacts. 136 ZERO MOBILE TANK EXTENSION PLATFORM RS-300 The Zero Mobile Tank Platform is designed to allow safe access for workers when loading hatches, servicing road tankers and trucks, storage tanks and many other jobs where safety access is required from on top.
