1 PARENT INFORMATION SHEET ACADEMIC REQUIREMENTS It is expected that all of our student athletes will maintain a satisfactory academic performance. Punctual attendance of all classes is expected and required. Unexcused absences or discipline issues in classes may result in suspension from the team. JUNIOR AGE The student-athlete must have been born in 2002 or 2003 and must be in Grade 9 or 10. EXPERIENCE No previous football experience is necessary. In fact, the majority of players have no previous experience before joining the program. A desire to work hard and improve is all that is required. COST The total cost to play is $ This provides the player with: a game jersey, practice jersey, compression shirt, t-shirt, shorts, game socks, 5 pad girdle, set of knee pads, a protective mouth guard, a player football banquet ticket and exhibition transportation fees. Payment is to be cash or cheque payable to Lorne Park Secondary School. PRACTICE SCHEDULE Pre-season Camp will run 9:00 11:00am, Tuesday, August 29 to Thursday, August 31. Formal Tryouts will run from Tuesday, September 5 th to Friday, September 8 th. Team practice will be every weekday after school from 3:00pm to 5:00pm. Attendance at practice is mandatory. However we don t want to deny a player due to outside school commitments, so If an accommodation is needed, please contact me. GAMES A printed schedule will be provided once it is finalized by ROPSSAA. It will also be posted on our website: FOOTBALL BANQUET Our annual banquet will be held on Thursday, December 7 at Lorne Park at 6:00pm. Players tickets are part of the playing fees. Friends and family may purchase tickets for $25.00 and are definitely encouraged to attend. You may pay for additional tickets with the player fee payment or at equipment collection in November. LORNE PARK FRIENDS OF FOOTBALL This is our program s Booster Club. If you are interested in helping out this coming year, please contact Christina Birch at christinabirch@me.com. The support offered by the Booster Club is vital for the success of the program so we would be grateful for any assistance you can provide. 3 LORNE PARK FOOTBALL PARTICIPATION FORM CODE OF BEHAVIOUR FOR STUDENT-ATHLETES AT LORNE PARK SECONDARY SCHOOL: 1) Uphold the highest ideals of sportsmanship and fair-play towards teammates, opponents, coaches, game officials, and spectators 2) Fulfill all academic responsibilities and maintain attendance, punctuality and appropriate behaviour 3) Dress according to standards established by coaches on game day 4) Travel via school transportation unless otherwise specified by the coaching staff 5) Accept responsibility for all equipment issued to them BEING A TEAM MEMBER IS A PRIVILEGE AND SUSPENSION/REMOVAL WILL RESULT FROM: use of illegal drugs or alcohol any incidents of vandalism, theft or behaviour that reflects badly on the program or Lorne Park SS receiving a suspension or in-school discipline due to breaking school rules gestures or language received as bullying or disrespectful by teammates, opponents or coaches failure to respect and keep our locker room and phys-ed hall clean after practice and games unauthorized absences from practice failure to abide by team rules set out by the coaches MEMBERS OF THE FOOTBALL TEAM ARE NOT ALLOWED TO PARTICIPATE ON OTHER INTERSCHOOL SPORT TEAMS DURING THE FOOTBALL SEASON Athlete Signature Parent / Guardian Signature
4 Lorne Park Secondary School is dedicated to fostering academic and personal excellence, pride and spirit in a dynamic and supportive environment. LORNE PARK SECONDARY SCHOOL Dear Parent/Guardian: As an extension of the extra-curricular program, your son/daughter is trying out for Lorne Park s Junior Boys Football Team. This activity is under the supervision of Mr. Moynihan, Mr. English and Mr. VanAlstine. This is a contact sport. Your son/daughter is invited to participate in voluntary pre-season camp, August 29th to August 31th and are required to attend the try-outs, September 5th to 8th. If he/she makes the team, mandatory practice will run Monday to Friday, each week of the season. Early equipment sign-out for incoming Grade 9 s and new Grade 10 players is on Monday, August 28 from 9:00am 11:00am in the Football Equipment Room at the southwest corner of the school. Students unable to make the early session may sign equipment out starting on Tuesday, September 5 at 11:00am. The team will leave school property during the season for exhibition games and regular season games in Mississauga, Brampton and surrounding regions. Transportation during the season will be by bus or taxi. The 2017 schedule will be provided when it is finalized by ROPSSAA. The cost for the football team is $ The cheque is to be made out to Lorne Park Secondary School, dated for September 5, Players will receive: a game jersey, practice jersey, team t-shirt & shorts, long sleeve compression shirt, game socks, belt, 5 pad girdle, knee pads and a mouthguard. The cheque also covers the player s football banquet ticket and program equipment expenses. There is also a $ equipment deposit cheque required. This cheque is to be post-dated for November 1, It WILL NOT be cashed unless borrowed equipment is lost or damaged. Both cheques will be returned if the student does not make the 55 player roster. The excursions are an extension of the regular school program. Accordingly, expectations regarding student behaviour are the same as those for the regular school day. While we do not anticipate any problems, any serious breach of expected conduct on the part of a student may result in that student being sent home at the expense of the parent and further disciplinary action may be imposed. Student information (parent contact numbers, medical information, emergency contact numbers etc.) contained on your child s school records will be taken along on the excursion and used only in the event of an emergency. Please ensure that all student information regarding address, phone numbers and medical or health concerns is upto-date. We are looking forward to an exciting season. I/We understand and acknowledge that the Peel District School Board will not be held responsible for any liability or medical expenses, due to an injury sustained by the student during this field trip/excursion. I/We grant permission to obtain medical treatment in the event of a medical emergency where attempts to make contact using information provided are not successful. I/We understand that the Peel District School Board's Code of Conduct as well as the Lorne Park s Code of Conduct, are in effect and apply to my/our son/daughter at all times during this excursion. I/We understand that my/our son/daughter is allowed to carry a cell phone for use in an emergency. Otherwise, the cell phone may not be used for any communication or for taking any photographs unless permission is granted by a staff supervisor. Posting photographs or images on the Internet (including social network sites) without adult permission violates the school's code of conduct. I/We understand and acknowledge that neither the Peel District School Board nor the School will accept responsibility for any money not refunded by the excursion service provider should I/we subsequently decide not to permit my/our child to travel on the excursion. Please note that your son/daughter, if 12 years old or older, must also sign this form to acknowledge their acceptance of the above.
