2 Summer Season Program Brochure 2017 Welcome! This summer will be an exciting time for the Recreation Department. We are excited to be opening our new disc golf course which has been a ongoing year and a half long project. This course will be open to the entire community to enjoy. Ongoing improvements will continue to be made in the years to come. We hope that with the addition of this course we will be able to offer our community yet another way to stay active year round. 4 General Program Information 4 Easy Ways to Register! ON Line Registration Go to our website and click on Register NOW button to sign up via Blue Sombrero. In-Person- Registration forms and computer access are available in our office during our hours of operation. Cash, check, or credit card are accepted. Please bring exact change. 24 Hour Drop Box- We have a drop box conveniently located outside our office doors. Registrations forms can be securely placed in our drop box any time of day (or night). Mail in- You may have received our registration information via or through our website. You can mail in your completed registration form to: LMRD 120 E. When such errors occur, our staff will do everything possible to correct the situation promptly. We thank you for your patience and understanding when these situations arise. 5 General Policy Information Concussion information: In 2011 the State of Wisconsin passed Act 172. Which requires any child under 19, who participates in an organized athletic game or competition, to receive information about concussions and their symptoms. Annually parents and the athletes must return a signed copy of the Parent and Athlete Agreement prior to their participation in a rec sport. Weather Cancelations: In the case that school may be canceled due to bad weather we will be canceling all recreation department activities. Weekend cancelations will be done via , Facebook posts or contact from your child s coach. Residency: For program enrollment residents are those who reside within the Lake Mills Area School District boundaries, open enroll to LMASD, or work within the school district. Residents pay the fee directly before the R. The second fee listed is for non-residents NR. Non-Residents are those who do not reside within the Lake Mills Area School District or attend school at LMASD. Residents support the school district through tax-dollars, while non-residents pay to participant through higher registration fees. Non-Residents will be responsible for paying a minimum of an additional $10 per activity and participant they register. Insurance: The Lake Mills Area School District and Lake Mills Recreation Department do not provide medical or accident insurance for any program participants both residents or non- residents. Participants are strongly encouraged to obtain their own insurance coverage prior to participating in programs. Photo Policy: Participants or their parents (if participant is under 18) permit the taking of photos, audio and videotapes during Recreation Department activities for publication and use as the Recreation Department deems necessary. If you do not want a picture or video taken of yourself, please inform the director or instructor prior to the start of the program. 5

6 Fees/Late Registration: Fees are based off of the minimum amount of participants needed to successfully run a program we will do our very best to anticipate needs for now and the needs for the future of our programs so that we may set our prices accordingly. To ensure proper planning we may set deadlines for our programs. Those who register after a set deadline will have to pay an additional $10 late fee and may also run the risk of not being able to participate if the program is full. If you don t hear from us, that s good! After signing up for a program you may not receive a confirmation. Consider yourself registered for that program. Please be sure to mark your calendar with the dates for each program. If a program requires specific information we may send out an informational sheet via . You may also check your Blue Sombrero account for a list of all past registrations from your account. Notice of Non-Discrimination: No person shall, on the basis of sex, race, national origin, ancestry, creed, marital or parental status, age or disability, be excluded from participation in, be denied the benefits of, or be subjected to discrimination under any program or activity, including employment. Age/Grade Requirements: All youth program age restrictions will be based on the first day of the program unless otherwise specified. All grade restrictions will be based on the school year. Summer will officially mark the new school year. Age and grade requirements are set to benefit all children and make instruction easier for the program leader. Looking to the future! 7 Events Madison Mallards - Lake Mills Night at the Duck Pond Registration Deadline: July 28 - Online portal will close! Day/Date: Saturday, August 12 Time: Game Starts at 6:35 Location: Warner Park Duck Pond N. Sherman Ave, Madison Fee: Group promo Code: $10- includes ticket and hat lake mills hat $20- includes ticket and food before the game lake mills rec Register at: (enter Lake Mills as group password) Show your support for Lake Mills and the Mallards as they take on the Battle Creek Bombers! This year we will have 2 ticket options. A $20 all you can eat option 90 minutes at the ballpark before the game OR a ticket and hat option for $10. To purchase tickets go to enter the group portal password. All community members are invited to attend the game! Transportation Required. More information will be posted on our website. Coming Soon!!! LMRD One Day Events What are one day events? Our one day events will be a mixture of trips and themed events that will be taking place directly after Summer school for kids/families. Supervision will be provided by LMRD Staff. Lunch will not be provided but we will have time between the end of summer school and the start of the event. Parent pick up is required promptly at the end of the event. 7

8 Adult Athletics All participants must fill out a waiver prior to participation. Waivers will be available for print on line or hard copies are available at each open gym. These programs (Basketball & Frisbee) are free to all community members. Adult Basketball Open Gyms This is your opportunity to play some basketball, meet some new friends and get some exercise in! This program is open to all adults interested in playing pick-up basketball. As this is an ongoing program be sure to frequently check the facilities calendar for possible time/location changes or cancelations. 9 Adult Ultimate Frisbee - Outdoor Pick Up Games Day/Date: Thursdays, May 18 - Ongoing Time: 6:00-7:30 pm Location: LMHS Practice Fields (behind Track and Tennis Courts) Fee: FREE Have you heard of Ultimate Frisbee? Imagine combining the football field with goals at each end, the constant movement of soccer and a Frisbee! This is a non-contact sport that s easy to learn and fun for all ages. All games will be co-ed and teams will be divided evenly each week. SUMMER SPORTS CAMPS LMHS Youth Basketball Camp - Boys and Girls Registration Deadline: May 26 LMHS Boys Youth Basketball Camp Rising Grades Time 3-5 Grade Boys 1:00-2:30 pm 6-8 Grade Boys 2:30-4:00 pm Dates: June Location: LMES Gym Fee: $45 Camp Goals: It is our goal to maintain a high standard of teaching and player development. Our approach is to focus on important areas of player development while having fun. Shooting, ball handling, footwork, and decision making skills will be emphasized at all levels. Camp coaches are Steve Hicklin- Varsity Boys Basketball Coach, Dena Smith-Varsity Girls Basketball Coach, and members of Boys and Girls Basketball teams. Camp includes a take home item if registered by May 26. Walk-ins accepted on day of camp. 10

11 Co-ed Tennis Camp Registration Deadline: June 2 Rising Grades Time K-2 Grade 5:00-6:00 pm 3-5 Grade 4:00-5:00 pm Dates: June Location: Lake Mills High School Tennis Courts Fee: $25 Camp Goals: This camp incorporates exciting and challenging games and drills in an introduction to basic tennis skills for beginning players! Our focus is on having fun and developing the skills for a life-long sport. Each participant will receive a customized tennis ball canister! The camp will be led by Varsity Tennis Coach Cam Dary and Varsity High School tennis players. This camp is a fundraiser to raise money for the high school tennis program. Racquets recommended but not required. Walk-ins accepted. Co-ed Football Camp Registration Deadline: July 7 Rising Grades Time 3-5 Grade 1:00-2:30 pm 6-8 Grade 3:00-4:30 pm Dates: July 24 - July 26 Location: High School Practice Fields - E. Lake Park Place Fee: $30 Camp Goals: This non-contact camp is designed to introduce participants to the fundamental skills, basic strategies, and teamwork of football. On the last day players will compete in fun games including PPK and ultimate football games! Grades 6-8 are encouraged to attend, this is a great way to start warming up for the season. Please wear t-shirt, athletic shorts, and tennis shoes or non-metal spikes. This camp will be led by LMHS coaching staff and players! Bring along: water or sports for hydration and mosquito repellant. Camp includes t-shirt if registered by July 7, walk-ins accepted the day of camp with no shirt guaranteed. 11

12 Co-ed Soccer Camp Registration Deadline: July 14 Rising Grades: 5K-8 Dates: July 31 - August 4 Time: 8:30-10:30 am Location: Rotary Park Fee: $35 Camp Goals: This soccer camp will help polish your technique, improve your tactics, educate your feet and get a jump on your season. All players will receive a water bottle and Soccer bag! The camp will be led by the Varsity Boys and Girls Soccer Coaches and the High School Soccer players. This camp is a fundraiser to raise money for the high school soccer program. Camp includes Water Bottles, and snacks for the first 125 players, walk-ins accepted the day of camp. Campers may bring their own ball to camp or we will provide balls to be used during camp. Rec Basketball Camp Boys and Girls Registration Deadline: June 16 Rising Grades Day Time Dates: 4-8 Grade Boys Thursdays 2:00-3:00 pm 7/6-7/ Grade Girls Thursdays 3:00-4:00 pm 7/6-7/27 Location: Lake Mills Middle School Gym Fee: $25/R ; $35/NR LMRD summer basketball camp will focus on the fundamentals of basketball, while having fun and improving their skills. New drills and skills will be taught each week. If you are looking to improve your basketball skills then this is the camp for you! Minimum of 10 participants in each group. 12

13 Youth Sports IMPORTANT!!! All grade registrations are based off of the school year. Co-ed Volleyball Camp Registration Deadline: June 23 Rising Grades: 5-8 Grade Day/Dates: Fridays, June 30-July 28 Time: 1:30-2:30 pm Location: Lake Mills Middle School Gym Fee: $25/R ; $35/NR This camp is for players that want to continue working on volleyball skills during the summer. We will work on setting, passing, and attacking the ball. Campers will need a water bottle, knee pads and tennis shoes. Minimum of 10 participants. What s your passion? If you have a hobby or skill that you would like to share with others in the community, and get paid to teach! Then we have an opportunity for you! All new programs would be starting in the Fall! Currently, we are looking for Senior citizen activity instructors, PreK activity leaders, outdoor activity leaders, and more! Feel free to call us at for more information! 13

14 Youth Sports IMPORTANT!!! All grade registrations are based off of the school year. Pickup Basketball 7-8 th Graders Grades: 7-8 th Day/Date: Wednesdays, June 21 August 2 (No Class July 26) Time: 2:00 pm - 3:30 pm Location: LMES Gym Fee: FREE! Pickup basketball is for Middle School 7th and 8th grade boys and girls. Players will learn to pick fair teams each week and call their own fouls. Coaches may also work in some skill development from time to time. Players can stay the whole time or just drop in. We encourage fair play and accept all ability levels. This program will be supervised by several volunteer coaches. Sport Sampler Pre-School Registration Deadline: May 12 Rising Grade: 3-4 years old Day: Wednesdays Dates: June 14 - July 19 (No class 7/5) Time: 5:00-5:45 pm Location: Wallace Park Baseball Complex Fee: $20/R ; $30/NR This is a great way to introduce your children to a variety of sports. Each week we will cover skills that are used in sports. This program use a variety of sport and balance equipment in a fun and playful atmosphere! Adult involvement is required. Min 10/ Max 20 14

15 LMHS Disc Golf Disc Rentals Day Rental: $3 Weekend: $5 (Pick up Fri, Return Mon) Location: LMRD Office Looking for something fun to do over the summer, how about try our NEW disc golf course! Each rental will include 3 discs and a carrying bag. All rentals can should be picked up and dropped off at the LMRD Office. Rentals will need to be returned by 4:30pm at the latest. A $25 security deposit is required for all rentals. FREE RENTALS will be offered every Wednesday between May 31 and August 30 th. What is Blue Sombrero? In 2014 we launched our new registration website through Blue sombrero. This unique tool will allow participants to sign up anytime of the day (or night) for all of their favorite programs. Below are a few tips to help you get the most out of the system! Create one profile per family Be sure to keep your up to date Write down your user name and password Keep your receipts for your records If you cannot remember if you signed up for a program, first look up your orders in your account Register early Late registrations will be assessed a $10 late fee automatically OR programs may be closed! 15

16 NFL Flag K-1 Mini League Registration Deadline: July 21 Rising Grade: K-1 Grade Day/Dates: Thursdays, September 14 - October 19 Time: 5:00-6:00 pm Location: Faville Park Fee: $30/R ; $40/NR Is your youngster looking to get their game started off on the right step? NFL FLAG Mini Camp is a volunteer coached program that will teach the basics of flag football. All participants will receive a reversible NFL FLAG jersey along with their own set of flags to keep! Players will learn the basics of catching, throwing, hand offs, flag pulling, defensive play, and rules of the game. No additional practices will be scheduled. Late sigh ups will be charged an additional $10 plus additional shipping costs for jersey and equipment. NFL FLAG Football League Registration Deadline: July 21 Division/Grade AFC 2-3 Grade NFC 4-5 Grade Day Tuesdays Mondays Game Dates: September 11 - October 17 Game Time: 4:45 pm or 5:45 pm start times Rising Grade: 2-5 Grade Location: Faville Park Fee: $40/R ; $50/NR Can I hear you say Are you ready for some flag football! All participants will receive a reversible NFL FLAG jersey along with their own set of flags to keep. Game times will depend on number of teams, games may start as early as 4:45 pm. Coaches will pick their own practice time depending on their availability. Any registrations after the deadline will pay an additional $10 plus shipping costs for jersey and equipment. There will be a coaches meeting on Tuesday, July 25, at 5:00-6:00pm in the Middle School commons. 16

17 Tackle Football Grade Registration Deadline: May 26 Grade Fee: 6 Grade $90/R ; $100/NR 7 Grade $90/R ; $100/NR 8 Grade $90/R ; $100/NR Game Days: Saturday Mornings between 9:00am and 1:30pm Game Dates: August 26 - October 14 (no games 9/2) 1 st Practice: Monday, August 7. Weigh in: Wednesday, August 5:00 LMHS Picture Day: August 9 directly following weigh in Concussion Baseline Testing: Date and time TBD We will host a mandatory parent meeting/on Tuesday July 25 from 6:00-7:30 MS Cafe. Equipment Hand Out: August 2 nd. Any player who misses the meeting or those who need to exchange equipment will be fitted on Monday, Aug. 7 at 4:30 Café. We will be participating in the USA Football Heads Up program. All coaches will be Heads up Football certified. LMRD Tackle Football Program participates in the Jefferson County Youth Football League. All home games will be played on Saturday mornings at the Campus field. Each individual coaching staff will set a practice schedule which will be sent out to their team. Each coach will have team specific player/parent meeting to discuss their team specific goals for the season. All teams will practice at the High School Football Practice Field. *Note: Players must have a physical performed or an alternate year card filled out before participating, so schedule your appointment early! Participants must also take a baseline concussion test, dates are listed below. Volunteers needed! The Youth Tackle Football program relies on the support of our parent volunteers to provide a quality game day experience. Up to 8 volunteers are need to run just one game. Volunteer positions include: Chains, Water Person, Concession Stand, Broadcast Booth, please indicate on registration form. 17

18 Fall Rec Soccer 5K-5 Grade Registration Deadline: July 13 5K-5 grade Leagues: Grade/Division Description 5K-1 (U6) The Pee Wee league will play on a condensed field. They will field a 3v3 team with no goal keeper using a #3 soccer ball. This league is focused on fun and learning the game of soccer. 2-3 (U8) The Youth league will play on a condensed field. They will field a 4v4 team with no goal keeper. Scores will not be kept, this league is focused on fun and learning the game of soccer. 4-5 (U10) This is a more advanced level of play. They will field a 6v6 team with a goal keeper. Game Dates: Saturdays, September 9 - October 21 Game Times: 8:15 am - 1:00 pm Location: Rotary Park Fee: $25/R ; $35/NR LMRD recreational soccer program is for children in 5K-5 grade. Players will have fun while learning the fundamentals, rules and skills of the game. Practices will start 2-3 weeks before the season. Both games and practices will be held at Rotary Park. Participants will be contacted by their coach with league schedules & team practice information. Coaches wanted! Each soccer season we need around 34 coaches to run this program. Please consider coaching your child s team! Mandatory Coaches Meetings: LMES Commons on August 1. Coaches will pick up equipment, rosters and schedule practice time. 5K-8: 6:00-7:00 pm 18

19 4K Parent/Child Soccer Skills Registration Deadline: July 13 Grade: 4K Day: Saturday Dates: September 9 - October 7 Time: 8:15-9:00 am Location: Rotary Park Fee: $25/R ; $35/NR The 4K league will meet on Saturday mornings only (no week day practices), they will learn basic fundamentals in fun game situations. This program will be led by an instructor. Our lead instructor will guide the parents and participate through each drill. Each parent will learn the skills so that you can continue to teach your child after the program is over. Each player will receive a water bottle! (U12) Junior Soccer Registration Deadline: July 13 Grade: Grades Day: Saturdays Game Dates: September 9 - October 21 Game Times: 9:00 am - 12:00 pm Location: Wallace Park Fee: $25/R ; $35/NR The Middle School level will play a combination of 8V8 and 11V11 games. Teams will be set by the coaches week to week. Coaches will work together to plan mass practices during the week and coach games on the weekend. Participants will get the opportunity to play their last game on the LMHS Varsity Soccer Field! We may have the potential to join a the Jefferson County Middle School Rec Soccer League. More info will come out as this new league develops. 19

21 Lake Mills High School Volleyball Camp Presented by Midwest Volleyball Performance Dates: August 7-9 Time: 5:00 pm -7:00 pm Registration forms available at the LMRD office Checks payable to: MVP or Midwest Volleyball Performance Midwest Volleyball Performance Midwest Volleyball Performance is coming to Lake Mills to run the 2017 Team Camp! MVP is made up of a group of current and past professional players who are excited to pass along the volleyball knowledge and passion to the next generation of athletes. Check our website for more on our staff! Camp Overview At MVP we stress the all-around player as well as learning through live play. We will focus morning sessions around individual skill development and position specific training. Lunch breaks will include Chalk Talk sessions focusing on topics benefiting the next generation on and off the court. 21
