3 AUTHORIZATION By signing this agreement, I understand that passes at Centennial Golf Club are annual pass agreements running from opening day through closing day and that annual pass fees are non-refundable and nontransferable. This Agreement is entered into between Centennial Golf Club, and ("Purchaser"). Pass # The following policies shall govern all pass holders at Centennial Golf Club and are agreed to and accepted by the undersigned Pass holder. Centennial Golf Club reserves the right to amend the club policies from time to time and determine fees on an annual basis. Conduct that is likely to endanger the welfare, interest or harmony of the Club; willful or repeated infractions of club rules or of Centennial Golf Club policies (including failure to pay all dues, fees, or other indebtedness); and acts or conduct deemed disorderly, injurious or hostile to the interests or the objects or other pass holders of the Club; all shall be cause for suspension or revocation of pass holders, whether such acts, infractions or conduct are performed by the undersigned applicant, anyone included in the applicant's season pass, or any of the applicant's guests. Should any pass holder, guest of a pass holder, or person included in the pass holder's agreement so conduct him or herself so as to give rise to cause for suspension or revocation of the seasonal pass, written notice outlining the charges shall be sent to the pass holder. Pass holder shall then have ten (10) days in which to appear before the management to defend him or herself against such charges. After said hearing, or in the event the pass holder fails to appear, the management may, at its discretion: revoke or suspend privileges; drop the charges; require reparation be made by the pass holder; or take any other disciplinary action it deems necessary. Should the pass holder fail to make reparation or fail to perform whatever actions are required of him or her within the time period requested by the management, the pass holder's agreement may be immediately revoked or suspended upon the expiration of said time period, in which case the management will notify the pass holder of such revocation or suspension. Should revocation of the season pass by the management take place, the season pass fees will not be refunded. *NO CALL, NO SHOW* Annual Pass Holders are responsible for 100% of any tee times booked and not Cancelled prior to 24 hours in advance. Annual Pass holders who do not show up for their tee time or without all the players reserved for the tee time will be charged the full rate for each player not showing to the pass holder s credit card account number provided in this Annual Pass Agreement. Centennial Golf Club reserves the right to host Outside, or functions/outings during the week and weekends as it deems necessary in order to maintain business volumes. Centennial Golf Club has provided Purchaser with a copy of this 2017 Annual Season Pass Agreement dated as of September 24, Purchaser agrees to comply with the terms and conditions set forth within this Annual Pass Agreement and wish to purchase an Annual Pass for the 2017 season. This Agreement shall become effective only after Centennial Golf Cub has received full payment of Seasonal Pass and after this Agreement has been co-signed below by Centennial Golf Club. **All accounts are due and payable prior to the first round of the 2017 season. 5 SCHEDULE OF FEES AND CHARGES (Effective September 24, 2016) In accordance with the Centennial Golf Club Pass Holder Plan, the Club establishes the amount of fees and other charges. Pass Holder deposits, dues, fees and charges are subject to change from time to time. 8 Season Pass Tee Time Booking Policy In effort to maximize the availability of Tee Times at Centennial Golf Club for all pass holders and guests we have updated our booking policy. Please read thru the bullet points below to review and understand or policy Tee Time Booking Policy: Each pass holder is entitled to book tee times 14 days in advance Pass holders may book tee times for up to 8 players. Anything more than 8 players must be approved by the Head Professional or General Manager The person whom books the tee time will be held responsible for all players booked. Tee time cancellations must be made 24 hours prior to tee time Pass holders will be held responsible and charged the guest rate for tee times that are not canceled 24 hours in advance. All pass holders must check in at the golf shop prior to playing. We thank you in advance for the cooperation of this policy and look forward to a great 2017 season.
