We understand that the BRJGT sponsors, volunteers, and participating facilities are not liable for accidents or injuries sustained while traveling to or from, playing or participating in the BRJGT events. We jointly and severally agree to fully indemnity and hold harmless BRJGT tournament officers, employees, volunteers and participating facilities from claims for injuries or damages sustained during or resulting from any BRJGT event by the above-named participant and from any and all expense including attorney s fees, related to any such claims. We hereby grant authority to BRJGT, in our absence, to authorize treatment by a qualified and licensed health care provider which in the opinion of the health care provider and any representative or BRJGT is necessary to prevent loss of life, disfigurement, physical impairment or undue discomfort, if delayed, and to administer first aid or medical treatment as required to the above named child. We agree to be financially responsible for any such treatment. 3 BLUE RIDGE JUNIOR GOLF TOUR, Inc Parent/Spectator Guidelines This form MUST be signed by both parents and player and returned with the tournament application entry forms, before tournament entries will be accepted. All tournament application forms will be returned if they are not completed, signed and received at the BRJGT address: Blue Ridge Junior Golf Tour, Inc. Parents and spectators are an essential part of making the BRJGT a great summer tour! We urge you to support the development of young people in golf by following these guidelines precisely: All juniors must carry their own golf clubs except for the developmental 9-hole age groups. (Pull carts only if permitted by golf course.) Keep a proper distance from the players (50 feet is recommended). Act as a forecaddy. (Stay far enough ahead of the players not to interfere with their play but to spot errant tee shots and help locate lost balls for all players in a group.) Spectators must stay on the cart path or in the rough with the exception of around the greens. (Spectators may walk near the greens to watch the players putt out.) DO NOT GIVE ADVICE TO ANY PLAYER! (Rule 8, USGA Rules of Golf). Advice is any counsel or suggestion, which could influence a player in determining his or her play, the choice of club, or method of making a stroke. Penalty in stroke play is 2 strokes! A conversation with a player may be construed, as giving advice-best policy is just not talk to the participants during a competitive round. Abusive language, swearing, and alcoholic beverages will not be permitted on the premise or golf course. Do not disrupt the pace or flow of play. Please pick up any trash you may see left by the juniors on the course. If you think you see a rules infraction, inform a rules official as soon as possible so the situation may be resolved prior to the player signing his/her scorecard. Do not suggest or give rulings on your own. If a player is in doubt about a rule, or situation, spectators may only remind them of the second ball rule. (Rule 3-3, USGA Rules of Golf; the player must then ask the rules official for a ruling as soon as possible. Spectators using golf carts will observe both the clubs rules as well as BRJGT rules in operating that vehicle. I hereby release the Blue Ridge Junior Golf Tour, Inc., and any golf course on which any BRJGT event is held, and their respective officers, directors, employees and volunteers from any and all claims for any injuries or damages arising out of, resulting or related to any BRJGT event. I understand and agree to support and abide by the BRJGT Code of Conduct and the Parent/Spectator Guidelines at all times. 4 TOURNAMENT REGULATIONS 2017 BLUE RIDGE JUNIOR GOLF TOUR CODE OF CONDUCT All tournament fields are based on course availability at each individual club. Three forms must be completed and returned to the BRJGT office before acceptance into any event. The three forms are: Medical Release Form Parent/Spectator Guidelines Code of Conduct All tournament entries will be returned if the above forms are not on file in the BRJGT office. No entries will be accepted at the tournament site or by telephone or fax, unless otherwise specified. No confirmations of accepted entries are mailed, however, tee times are available at least 5 days prior to the event by calling the host club or website: Players are required to walk and carry their own golf bags. (Pull carts may be used if the host club permits) No caddies or coaches are permitted. Players should arrive at the course one hour prior to their scheduled tee time and are required to register at the registration table in person at least 30 minutes prior to their scheduled tee time. Failure to register at the specified time may result in your place being filled by an alternate. DRESS CODE GUIDELINES These guidelines are in effect from the time you enter the host club facility until you leave that facility. All boys must wear golf shirts with collars and shirt tails tucked inside shorts or pants; T-shirts, tank tops, athletic jerseys, etc. are not acceptable Soft spikes are required at all golf courses. Girls may wear collared golf shirts with shirt tails tucked inside shorts or slacks Bermuda length shorts must be mid-thigh or longer. Unacceptable apparel includes; tennis shorts, gym shorts, short-shorts, or cut-off jeans. Caps and visors must be worn in the bill-forward position CONDUCT GUIDELINES Any conduct that is deemed detrimental to the Blue Ridge Junior Golf Tour will not be tolerated. Such conduct includes but is not limited to the following: Consumption of any alcoholic beverages Smoking and/or use of any tobacco products Drug use Gambling Abusive language or profanity Club throwing Damage or abuse of host facility property or golf course Any flagrant breach of the above guidelines will result in automatic disqualification from the event where the incident occurred. The degree of flagrancy is entirely at the discretion of the officials in charge of the competition. The player will also be disqualified from the next event entered without refund of entry fees. Two disqualifications will result in suspension from the Blue Ridge Junior Golf Tour for the remainder of the season.

5 OFFICIAL PROCEDURE FOR WITHDRAWAL A player must notify the Executive Director Dewayne Belcher by telephone at ( or or by at brjgt@yahoo.com) at least 48 hours in advance of the tournament from which they intend to withdraw. This is the only acceptable procedure for official withdrawal; Any other means including s, telephoning the host club golf shop, notes given to starter, or faxes are not acceptable. Failing to properly withdraw from a tournament and/or failing to show up for the competition will forfeit the entry fee and the player will be suspended from the next entered tournament. Credit will be issued only for proper withdrawals and refunds will be mailed at the end of the season. CARE FOR THE GOLF COURSE All players are expected to maintain as well as improve the conditions of the golf course by replacing divots, repairing ball marks, raking bunkers, picking up and properly disposing of trash or debris left on the course. Leave the golf course better than you found it! CHEATING Blue Ridge Junior Tour events subscribe to the highest standards of honesty and integrity. Cheating of any type will not be tolerated and will result in immediate disqualification and suspension from the remainder of the schedule of events. I have read and fully understand the information as set forth in the 2017 BRJGT Tournament Regulations and the player s Code of Conduct. Photo Release Opt-Out Form Due to the competitive nature of the BRJGT, photographs will occasionally be taken and posted to the BRJGT PhotoBucket which is located on the official website. The BRJGT realizes that there can be situations where parents/guardians do NOT wish to have a child s picture published. If this form is not returned to the BRJGT, we will proceed with the understanding that you are giving permission for your child to have photographs and/or videotape of your child used in local newspapers, or on the BRJGT website.
