2 ORDERING SUPPLIES Initial supplies will be sent to each track when a signed sanction is received. Initial supplies will include: Competitor License forms, Official s license forms, receipts, supply order forms, insurance posters, remittance sheets, pre-paid UPS labels, postage paid envelopes, Addendum A forms, and Accident report forms. Wristbands will be sent under separate cover. When ordering additional supplies please fill out a supply order form or log onto nascarmembers.com to complete the form, keep the back copy (goldenrod) or a copy if faxed, for your files and forward the original copies to the attention of Cece Owens. This includes only those officials at your track that make official calls and race decisions.
