1 TERMINATORS SELECT BASKETBALL PARENT AND PLAYER CONTRACT Terminators Select Basketball is a sports coaching and conditioning program located in Fort Worth, Texas. We are very proud to be select basketball coaches and members. Our training program is aimed at improving young athletes abilities and physical conditioning. Our GOAL and objective is to prepare each athlete for the next level of playing and to attain both their individual and team goals, thus making them more successful and confident on and off the court. The following is to be considered a CONTRACT and further explains our program. Please return each page, signed to Coach Mitchell before your first practice. Also, please save a copy of this contract for your records. WHAT TERMINATORS SELECT OFFERS: o It is our personal goal to make your child the best player they can be. o Club teams will have two intense and skilled practices per week, lasting for approximately 1.5 hours. The coach may add or subtract from the number of practices each month. o We will travel to 1-3 tournaments per month (weekends) as available, located mostly in the DFW area. o Your child will be exposed to advanced basketball plays, skills, player development, techniques, and higher levels of competition in an environment that provides optimal learning while offering an enjoyable, satisfying experience wherein your child has genuine fun throughout the process. o Select basketball offers players the opportunity to remain a team for many years throughout numerous age group increments. You have the option to stay on the same team all the way through high school, as our select program offers tournaments from eighth grade to high school. o Individual skills training is available upon request. o Physical conditioning, lacking in most junior sports programs, is an integral part of the Terminators Select Basketball program, including and based on strength, agility, and speed training. 2 COACHING STAFF: The Terminators Select Basketball organization is made up of an experienced director/head coach, and two organization assistants/managers overseeing all practices and tournaments. Individual teams will have coaches other than those listed below. Chris Mitchell (Terminators Select Basketball Director/Head Coach) Ataya and Tiarra Mitchell (Managers) SEASON DURATION: o Upon registration, players will engage in conditioning camp until the beginning of the regular season. o The official season begins when the roster reaches a minimum of seven players. At this time, regular weekly gym time will be scheduled, uniforms will be ordered, and the first tournament will be booked. o Tournaments will never conflict with school sports as they are on scheduled weekends with prior notice. o Tournaments outside of the DFW area will cost additional fees which should be covered through team fundraising. o Each season will last approximately 4-6 months. The number of games and tournaments may vary depending on the players on the team roster. o Players that register during a season that is already in progress may participate in that season. This is solely the coaches decision. TERMINATORS SELECT PROGRAMS: o Travel Club Team- This is for players wishing to play and participate in competitive select basketball tournaments. Teams will be arranged based on age and grade level. This is on a tryout basis. Players who do not make the team format will be offered the opportunity to participate in skills clinics to improve their playing ability to make the Club Team. o Lady Terminators Select (girls 12U-17U) o Terminators Select (boys 12U-17U) o 1-on-1 skills training/coaching Page 2 of 6
3 EXPECTATIONS: This is elite level basketball for players whose focus is on being competitive, elite players. The following are expectations and requirements for parents and players: Practices: o Practices are mandatory. o Players should be dressed out and equipped (practice jersey, shoes, playbooks, equipment, etc.) before the official practice start time. o Players should arrive to practice at least fifteen minutes before start time. o There can be no adults or children on the gym floor during practice unless they have been asked to join by a coach. Competitions and Tournaments: o Competitions and tournaments are mandatory. o Players are to wear the shooting shirt selected for that day s competition during the hour before the start of the first game. o Players should arrive to the competition location at least one hour before the first game. o Players are to be dressed in full uniform attire, including shooting shirts, one hour before games. Players will sit together during this hour and in between games. o There will be no parents or children on the bench during games. o Players will stretch as a group twenty minutes before game time. Tardiness and Absences: o Any notices of absences or tardiness to practices, games, and team events given outside of thirty minutes prior to start time will be unexcused. o Players that are late will be penalized individually and, may incur a team penalty. o Routine absences and late arrivals will be handled immediately. These will result in a loss of playing time. Other: o Parents must be fully committed to the program. o Players will complete at least four hours of team community service per month. o There will be grade checks every three weeks. o All fundraiser amounts are to be turned in by the last practice of the month. o Any medical wear (ankle braces, knee braces, etc.) must be on during practices and games. o Players must be respectful of teammates, opposing players, coaches, parents, and officials. 4 PLAYING TIME: o Terminators Select Basketball encourages attendance, participation, positive attitude, and effort. These will factor into playing time. However, performance will also be a determining factor in playing time. Players whose skills have advanced and superseded those of others will outweigh others regarding playing time. Club basketball is a competition and players will compete against one another for playing time. o Coaches reserve the right to decide who can participate in tournaments, practices, and all Terminators Select Basketball activities. INJURIES: o Terminators Select Basketball is not liable for any injuries sustained during or as a result of participation in club (the Terminators Select Basketball program) activities. SPONSORSHIPS: o Parents are expected to sponsor their child and assist in finding team sponsors to support the Terminators Select Basketball program and its players. PRICE: o Each player will be allowed a one-day tryout session. The cost of the tryout is $20 to cover the gym rental fee. This fee is due before players attend tryouts and is deducted from the registration fee total if the player decides to join a team. After that day, registration and the initial payment will be due to Coach Mitchell at an arranged meeting or at the next practice. o Each player is required to be a registered AAU member. There is an annual cost of $16. Membership allows athletes to participate in AAU sports events and includes insurance coverage for participation in AAU licensed events. Proof of the application is due to Coach Mitchell before the player s participation in their first competition. Parents can apply for their player s membership at o The price of the Terminators Select Basketball program (winter/spring season) is $600. Payments can be made in one full payment, two payments of $300, or three payments of $200. The first payment should be made at registration, and each of the following payments should be made on the 1st-3rd of the next consecutive Page 4 of 6

This package includes registration, membership, a team shooting shirt, a practice jersey, scrimmages, and local tournaments. o Effective February 1 st, 2018, there is a one-time $100 uniform fee. We will provide personalized home and away uniforms. Uniforms will be ordered upon registration and players will receive them within two to three weeks after. o There is a $50 court fee that is required and due by the 1 st of each month. This is to cover extra practices throughout the season and allows us to keep package prices low. This fee is expected to be paid even if a player is absent during that month. o Effective February 1 st, 2018, there is a $50 coaching/tournament fee that is required and due by the 1st of each month. This will cover the cost of the more expensive NCAA certified tournaments that we will attend. It will also cover coaches time, gas, and equipment. PAYMENTS: o Any payment submitted to the Terminators Select Basketball program upon registration or thereafter is considered an acceptance and understanding of this contract and the organization details. o Payments can be made through the Terminators Select Basketball online store, by cash, or check. All online payments will incur a 2.9% processing and service fee. o Payments are considered late if they are not received by the 3rd day of the month. Late charges will be added in as $20 each day until the payment is received in full. If the full payment is not received by the 5 th late day (7 th day of the month), the player may be excluded from activities and the Coach may choose to terminate the contract. o Please speak with Coach Mitchell if any arrangements are needed. o Players may not participate in tournaments if payments are not current. o Invoices will be sent out on the first of each month and will include all costs including monthly fees. o Any extra additions, apparel, equipment, and travel tournaments will be paid by the parents or through team fundraising. 6 RETURNED CHECK POLICY: o Any returned check will receive a $50 fee. CANCELLATION POLICY: o No refunds will be given. But, in the event of dissatisfaction, though not required, coaches do ask that parents explain the reason for their decision to cancel. o Terminators Select Basketball and its Director, Coach Chris Mitchell, reserve the right to terminate the membership of any player and parent as a result of nonadherence to the rules and expectations outlined in this contract, payment issues, or for any other reason that the organization deems in violation of its contract and policies I understand and have read and agree with all of the expectations, rules and requirements needed to participate in TERMINATORS SELECT BASKETBALL programs. 7 Medical Release/Liability Waiver: EMERGENCY MEDICAL AUTHORIZATION AND LIABILITY RELEASE RELATING TO EMERGENCY MEDICAL TREATMENT. The participant represents that a physician has found him/her physically capable of participating in youth basketball. I and my parent/guardian give my consent to have an athletic trainer, coach and/or doctor of medicine or dentistry or associated personnel to provide me with emergency medical assistance and/or treatment and agree to be financially responsible for the cost of such assistance and/or treatment. I and my parent/guardian also agree to release, hold harmless, and indemnify Terminators Select Basketball and its coaches, managers, officers, directors, and agents from all liability, loss, cost, claim or damage whatsoever, including death or damage to property related to or caused by the provision of emergency medical treatment, including necessary transport to a medical facility. GENERAL LIABILITY RELEASE. I and my parent/guardian fully understand that I will be engaging in activities that involve risk of serious injury, including permanent disability or death which might result from my conduct, the conduct of others, the rules of play, and the condition of the premises and of any equipment used, and from other unknown risks not reasonably foreseeable at this time. With a full understanding of the discharge, promise to indemnify and not to sue Terminators Select Basketball or the AAU, its affiliated organization and its coaches, managers, officers, directors, and agents, including the owners of the premises used to conduct all basketball activities from all liability to me, my parents/guardians, my next of kin and my heirs. 10 Terminators Select Basketball Photo Release Form I hereby grant permission to Terminators Select Basketball to use any photographs taken by other team parents, or a member of the Terminators Select Basketball organization. I allow them to post them on the Terminators Select Basketball website, as well as the Terminators Select Basketball Team Snap website, Facebook, Twitter, and any other flyers or materials for promotional purposes. These photographs will be representing and promoting Terminators Select Basketball during practices, games, tournaments, and team events. I acknowledge that Terminators Select Basketball has the right to crop or treat the photograph at its discretion for formatting purposes. I also acknowledge that Terminators Select Basketball may choose not to use any photograph, but may do so, at its own discretion. I also understand that once a photograph is posted on the Terminators Select website, the photograph can be downloaded by any computer user. Terminators Select Basketball may use these photos and/pr videos through these mediums from the time they are taken until they wish to remove them, regardless of parent and player membership status. Therefore, I agree to indemnify and hold Terminators Select harmless from any claims. Yes.
