Grades A, B, C, D and E divisions are being offered. Grades may combine if entries/schedule warrant. We will be running two fields. Prizes will be awarded for 1 st 3 rd Place, Best Player, and Best Horse in each division, plus Outstanding Junior Player. Community grill provided for Friday night Bring Your Own Meat to Grill and dinner. Saturday night Corn Hole Competition cash prize. The Sugarloaf Canteen will be on site for breakfast and lunch on Saturday and Sunday! All Teams MUST bring an Umpire Horse! ELECTRIC PENNING ONLY AT FIELD. PENS MUST BE MUCKED OUT BEFORE DEPARTURE. Limited stabling available off-site, please contact the tournament director. Turn right onto MD-121. Turn right onto MD-117. Turn left onto Bucklodge Rd (low bridge). Destination will be on your left. From the South Take I-495 N to I-270 N. Take exit 6 for MD-28 W towards Poolesville. Turn right onto Bucklodge Rd. Destination will be on your right. 2 1. Entries: a. All horses must have current Coggins, in accordance with MD statutes. NO EXCEPTIONS. Coggins must be scanned and ed to Kathleen Balogh, If you cannot scan please contact Scott Rockafellow to find a solution. b. A complete entry includes completed entry form, payment, a current coggins and a signed release form. Please to Kathleen Balogh. If you do not submit a COMPLETE entry by August 20 th, Entries received after this date are subject to a $25 late fee and entries received after August 26 th, 2016 will not be accepted. c. The entry fee includes Saturday night dinner for the player only. Additional dinners are available for $20 per person and must be paid for in advance. d. APA players with current ratings should provide their ratings on the entry form. Teams will be placed in a division based on total team rating. If there are any questions about this, the tournament committee will be available for discussion on a case-by-case basis. Decisions of the tournament committee with respect to assigning teams to divisions are final. e. The tournament committee reserves the right to alter the tournament format, as it deems appropriate and all decisions of the tournament committee are final f. Teams will not be announced until the morning of August, 29 th. g. Tournament organizer reserves right to switch teams to appropriate division, based on cumulative ratings and to alter the tournament format to full teams, if circumstances warrant. 2. Rules: a. We WILL NOT tolerate underage drinking in accordance with Maryland laws and APA policies. b. APA approved helmets (per the APA rulebook) and appropriate footwear must be worn at all times when mounted. NO EXCEPTIONS. c. The tournament committee reserves the right to refuse any entry. d. You must be a member of the APA to play at the tournament, if you are not a current APA member you must pay the APA day membership fee of $20 to be allowed to compete. e. Each team must provide 2 volunteers for scoring, timing and goal-tending. f. Each team is responsible for providing the umpires with a suitable horse for their grade level. Teams are responsible for finding and asking their umpires if they require an umpire horse. 3. Housekeeping: a. Trash you will need to take your trash with you when pulling gout on Sunday afternoon. b. Do not block any road or entrance with personal vehicles or trailers. c. All pets must be on a leash or in a kennel at all times. d. Smoking under the tent is not permitted. e. Open fires are not permitted. f. The grounds will officially open at 2:00pm on Friday, September 2 nd. The grounds will close at 8:00pm on Sunday, July 7 th. If you plan to arrive early and/or depart late, please notify Scott Rockafellow in advance. g. Upon departure hot pen areas need to be cleaned and hay and manure needs to be removed. All manure and old hay may be placed in the tree line, but must not be put into the farmer s field or in the tree line cut-throughs. Please pick up manure from around your trailers as well. 4 Potomac Pony Club Field RELEASE, HOLD HARMLESS, and INDEMNIFICATION AGREEMENT I/we have inspected the facilities located on the property known as Potomac Pony Club Field ( PPC Field at Bucklodge Road, Boyds, MD 20841) and adjoining properties including the parking areas, dressage rings, polocrosse field, stadium jumping area, the crosscountry course ( adjoining properties ) and am/are thoroughly familiar with the conditions and hazards of all these facilities. Further, I/we fully understand that the riding and handling of horses is DANGEROUS, and I/we fully accept all the responsibilities and liabilities arising from placing, riding and handling horses on said facilities. I/we agree to hold harmless and exculpate the United States Pony Club, a corporation, the Sugarloaf Mountain Polocrosse Club and the Potomac Pony Clubs, their officers, members, members families, employees, heirs and assigns as well as the owners of the PPC Field and adjoining properties, their heirs and assigns against any claim or loss, injury, damage, or communicable disease caused to third parties or to the property of third parties by any horse I/we bring to PPC Field and adjoining properties. Knowing and understanding this, I still desire to participate and attend SMPC sponsored events. In consideration for my participation in and attendance to these events, I hereby, for myself, my heirs, executors and assigns, discharge and hold harmless; The Sugarloaf Mountain Polocrosse Club, its officers, directors, members and agents, and all other persons or organizations in any way connected with this event including, but not limited to properties, their owners and operators, their officers, directors, employees, agents, representatives, heirs, administrators and assigns, from any and all rights, claims or liability for damages from any and all claims of any kind of nature, that might arise out of participation in any activity in any way connected with this event or taking place upon the grounds. I further acknowledge that this release will extend to any accidents, damages, or claims arising out of my participation, caused by my own act or the acts of anyone or animal within my control. I hereby certify that I am covered by medical, health and accident insurance and/or I am responsible for any costs I may incur for my own medical injuries. By signing this release and waiver, I am assuming all risks of this activity in which I will be engaged and releasing all of parties indicated above of any and all liability for their negligence of any description whatsoever. It is my intent to assume all risks and to waive and give up my rights (for myself and my heirs) to sue. And I do so knowingly and voluntarily. PARENT OR GUARDIAN RELEASE AND WAIVER I am the parent/guardian of, a minor, and on the minor s behalf of all parents and guardians, I accept the release and waiver of liability above as an inducement for allowing my child to participate in polocrosse games, practices, clinics and other related activities. Signature: Date: CODE of CONDUCT As a member of the American Polocrosse Association (APA), and following the rules and regulations established by the APA, the Sugarloaf Mountain Polocrosse Club (SMPC) is proud of its reputation for good sportsmanship, horsemanship, teamwork and well-behaved members both on and off the polocrosse field. Participants are expected to conduct themselves at all times in a manner which is in keeping with representing the SMPC and APA, and will not bring discredit in any way upon either the SMPC or APA. The SMPC expects appropriate behavior from all members, parents and others participating in any SMPC sponsored activity. Persistent irresponsibility, unsportsmanlike conduct or disrespectful behavior will not be tolerated, and will be dealt with following the guidelines set forth by the SMPC by-laws and the APA. Inappropriate behavior includes, but is not limited to: Possession, use or distribution of any illegal drugs; Underage possession, use or distribution of alcohol; Sexual or excessive harassment; Failure to follow rules; Cheating, and; Abusing a horse. Participants will place horses welfare above all other considerations. Animal abuse as identified by a doctor of veterinary medicine will not be tolerated. Participants will respect the person and property of others, the rules established by SMPC, and will obey all local, state and federal laws. Destruction of property, theft, or violation of local state or federal laws will not be tolerated. ENFORCEMENT: The SMPC, acting in good faith, may take such action as is necessary to prevent the inappropriate behavior from harming people or animals, or disrupting the activity to the detriment of other participants including but not limited to suspending the member from play or participation in event-relate activities immediately. Knowing and understanding this, I still desire to participate in, and attend, the club practices and tournaments. In consideration for my participation in, and attendance to these events, I hereby, for myself, my heirs, executors and assigns; waive, release, discharge and hold harmless; the American Polocrosse Association and the APA club, it s officers, directors, members and agents, and all other persons or organizations in any way connected with this event including but not limited to the field, their owners and operators, their officers, directors, employees, agents, representatives, heirs, administrators and assigns from any and all rights, claims or liability for damages from any and all claims of any kind or nature, that might arise out of my participation in any way connected with this event, or taking place upon the grounds. I further acknowledge that this release will extend to any accidents, damages or claims arising out of my participation, caused by my own act or the act of anyone or any animal within my control. It is my intent to assume all risks and to waive and give up my rights (for myself and my heirs) to sue, and I do so knowingly and voluntarily. *** The APA has a Code of Conduct and has adopted a policy in accordance with the local State Laws of any APA sponsored event of zero tolerance for consumption of alcohol or controlled substances by minors. This policy includes all minors, APA members or spectators while under the influence of alcohol or controlled substances during any APA sponsored event. Any adult who is knowingly contributing to the violation of the policy by providing alcohol or controlled substances to minors will be subject to the same punishment set out below: 1. Any infraction of the policy shall be reported by two credible sources to the event s Tournament Committee with an appeal to the APA Grievance Panel. The Grievance Panel is appointed by the President as a standing committee. The Tournament Committee, if it finds the report reliable, will suspend the minor from play at that instant and then report the incident to the APA Grievance Panel in writing. 2. Upon receipt and verification by the APA Grievance Panel, the Panel shall issue a suspension of up to one year of the offending member s membership beginning from the date of the incident. Upon the second violation, the punishment will be suspension of a Junior s APA membership until age 21 and suspension of an adult s membership for life. 3. Appeal may be taken upon receipt in writing of suspension of membership. Any appeal must be made in writing to the APA Administrator within 30 days of notification by the Grievance Panel. The APA Board of Directors must act upon any appeal and notify the member of its action in writing within 30 days after receiving the written appeal. Suspension will remain in effect throughout the appeal process. As an APA member, I agree to abide by the By-Laws, the Code of Conduct and all Rules and Regulations of the American Polocrosse Association. I agree to comply with all State laws pertaining to the possession and distribution of alcohol to minors. Further, I acknowledge that it is my responsibility to comply with all APA rules, including those pertaining to equipment and helmets. The safety of the equipment and the animals that I use is my responsibility. 7 February 2013 AMERICAN POLOCROSSE ASSOCIATION CODE OF CONDUCT ALL APA RULES AND POLICIES SHALL BE IN EFFECT FROM 12:00 PM THE DAY BEFORE TOURNAMENT PLAY BEGINS UNTIL 12:00 PM THE DAY AFTER THE TOURNAMENT PLAY ENDS. Participants in events sanctioned by the American Polocrosse Association ( APA ) are exercising a privilege afforded them by tournament committees and the APA. In consideration of this privilege, participants agree to abide by this Code of Conduct. 1. GENERAL GUIDELINES: Participants will place horses welfare above all other considerations. Animal abuse as identified by a doctor of veterinary medicine will not be tolerated. Participants are expected to conduct themselves at all times in a manner which is in keeping with representing the APA, and will not bring discredit upon the APA. Persistent irresponsibility, unsportsmanlike conduct or disrespectful behavior will not be tolerated. Participants will respect the person and property of others, adhere to rules and guidelines of the APA rule book, the rules established by the Event Organizer, and obey all local, state and federal laws. Destruction of property, theft, or violation of local, state or federal laws will not be tolerated. 2. ENFORCEMENT: Tournament Committees, acting in good faith, may take such action as is necessary to prevent the inappropriate behavior from harming people or animals, or disrupting the activity to the detriment of other participants including, but not limited to, suspending the participant from play or participation in event-related activities immediately. The Tournament Committee is then responsible to: 1. Report the incident to the APA Grievance Panel in writing. 2. Upon receipt and verification by the APA Grievance Panel, the panel may decide no further action is necessary, may issue a one year suspension of the offending member s membership beginning from the date of the incident, suspend the membership for life, or any such degree of discipline in between, at its will. 3. Appeal may be taken upon receipt in writing of suspension of membership. Any appeal must be made in writing to the APA Administrator within 30 days of notification by the grievance panel. The APA Board of Directors must act upon any appeal and notify the member of its action in writing within 30 days after receiving the written appeal.
