2 REGISTRATION PROCESS Please read before completing entry form: Review schedules for Athletic Events, Silver Arts Events and Special Events. If a schedule conflict occurs, the participant must decide which event to attend. Participants responsible for securing all partners for doubles/mixed doubles events at registration. Pre-registration required no registration accepted at event. Fees: Senior Games and Silver Arts Entry Fee: $15 due upon registration Awards Banquet: $10 per person - reservation deadline Friday, April 28 If required, event fees are due at registration. Golf: $50 (includes green fee, cart fee) Bowling: $6 per competition ($2.50 shoes paid at location) Entry Form: Complete all sections AND sign liability waiver. Participants must be a Haywood County resident for a minimum of 3 consecutive months per year. Event Dates: April 20 - May 11, Please wear your t-shirt while competing! Athletic Events: Competition is among those of the same sex and participants will compete against those in their age groups: 50-54, 55-59, 60-64, 65-69, 70-74, 75-79, 80-84, 85-89, and 90+. Age is determined as of 12/31/17. Art Events: Competition is among participants according to sub-category (painting, photography, poems, etc.). Awards: Medals will be presented to 1 st, 2 nd, and 3 rd place winners in each athletic & art event at the Closing Ceremony. Each athlete or artist will receive one medal for each place. Additional awards will be in recognition only. Cancellations: Games Officials reserve the right to cancel an event due to insufficient registration and/or combine age categories for competition purposes. Inclement Weather: Games Officials reserve the right to cancel or postpone events due to inclement weather or unusual and extenuating circumstances. There will be no refunds for a canceled or postponed event. State Final Qualifiers: Athletics 1 st, 2 nd or 3 rd place. Golfers must place 1 st, 2 nd or 3 rd AND meet a minimum performance score to qualify for State Finals. SilverArts Heritage/Visual 1 st, 2 nd, 3 rd place Literary 1 st, 2 nd place Qualifiers will receive entry form & other information at the Awards Ceremony. 5 LIABILITY WAIVER (Required and must be signed) In consideration of being allowed to participate in any way in the Haywood County Senior Games (athletics/fitness/ sports/arts) program & related events & activities, I, the undersigned agree that prior to participating, I will inspect the facilities and equipment to be used, and if I believe anything is unsafe, I will advise the coach, volunteer, or supervisor of such conditions and refuse to participate. I acknowledge and fully understand that I will be engaging in activities that involve risk of serious injury, including permanent disability and death, and severe social and economic losses which might result not only from my own actions, inactions, or negligence, but the actions, inactions, or negligence of others, the rules of play, or condition of the premises or of any equipment used. Further, I acknowledge that there may be other risks not known to us or not reasonably foreseeable at this time & I assume all the foregoing risks and accept personal responsibility for the damages following such injury, permanent disability, or death. I understand that to promote total health and physical fitness, events of longer duration and lower intensity are strongly recommended and that higher-intensity types of activities are offered for the conditioned, trained athlete. I release, waive, discharge, and covenant not to sue Haywood County on behalf of Haywood County Senior Games, its affiliated clubs, their respective administrators, directors, agents, coaches, volunteers, and other employees of the organization, other participants, sponsors, advertisers, and if applicable, owners and lessors of premises used to conduct the event, all of which are hereinafter referred to as releasee or released party, from any and all liability to each of the undersigned, my heirs and next of kin for any and all claims, demands, losses, or damages on account of injury, including death or damage to property, caused or alleged to be caused in whole or in part by the negligence of the releasee or released party or otherwise. I agree that all press coverage of the Senior Games may be used for promotion of the Haywood County Senior Games or for promotion of the North Carolina Senior Games. Anyone officially connected with Haywood County Senior Games or with radio, television, or the newspaper may interview or take my picture for official use only with Senior Games. Finally, I agree that it is my responsibility to check with the local Senior Games Coordinator immediately following my Local Senior Games to determine if I qualified for State Finals and to get a State Finals Entry Packet. The State Finals Entry Form must be received in the NCSG office in Raleigh by 5:00 p.m. on August 1st. I understand this deadline is STRICTLY enforced in fairness to all, and understand it is MY responsibility to mail my entry form in on time. Go 1.7 miles and park will be on the left before the asphalt plant. Countryside Bowling Lanes 347 Jackson Plaza, Sylva Travel South on US 23/74 on the Great Smoky Mountain Expressway. Take Exit 83 toward Sylva. Continue straight on Grindstaff Cove Road. At the next stop sign, turn left on Beaverdam Rd. and go under I-40 and Immediately take a right onto Silkwood Drive. The complex is on the left next to NEO Corporation. (right) Courts on right near tennis courts. 5K - turn left at Foundation for Evangelism on Weldon Way. Meet at the open-air gym. Maggie Valley Country Club 1819 Country Club Drive, Maggie Valley From US 19 traveling towards Maggie Valley take a right onto Moody Farm Road at the stop light, then take a right onto Country Club Drive. The club house will be on your right. Old Hazelwood Gym (HCS Conference Center) 71 Virginia Ave, Waynesville From Hazelwood business area, go through light (Brown Ave); turn right on Virginia Ave. Pass Folkmoot. Park along street & enter at Conference Center sign. More parking in rear gravel area off Brook St. - enter from back door of gym at end of gravel path. Pisgah High School Pisgah School Road, Canton From New Clyde Hwy (Hwy 19/23 which becomes Park St) in Canton, turn onto Reed St. (Hwy 215 South). Yield right onto Penland St.; pass Canton Middle School (on the right) and Canton Armory (on the left). Just past the Armory, turn left onto Pigeon St. & follow into school parking lot and park on the right. OR Turn right on Pisgah School Road, pass the tennis courts and turn left to park behind the track/field. River s Edge Park 734 Thickety Road, Clyde From Carolina Blvd. in Clyde, turn on Charles St. toward the Pigeon River. At the stop sign go straight (old school on left) and cross bridge. At stop sign turn right on Thickety Road & go 3/4 mile. The park will be on the right. Parking lot is at the far end of park.
