Dear Runner, By using this form, you will be able to apply only for the Kms -Half Marathon (timed race for men & women), 10Kms Run (Timed race for Men & Women) and 4Kms Rotary Get Fit Run (Non-Timed Race for Men & Women). Please go through all the details mentioned in this Application form prior to submitting your application. Important Information for Kms -Half Marathon /10Kms Run/4K Rotary Get Fit Run (Non-Timed) applicants Your entry for the Half Marathon/10K Run/4Kms Rotary Get Fit Run (Non-Timed) will be confirmed based on the following Verification of documents, where applicable, submitted by you. Availability of running spaces under each category Entry Rules and Guidelines: A duly filled application form needs to be submitted with a copy of Govt. issued photo ID proof i.e. either Passport, Driving Licence, Election Card, Aadhaar Card or PAN Card. Running Number BIB Collection Centre: On 23 rd & 24 th February, 2018 (between 11 am & 6 pm). Venue for Bib Collection will be intimated later. APPLICATION MODES Online through the event website Forms can also be downloaded for offline applications from the event website In Juhu, forms will be available at the event office at Vijay Mansion, NS Road No 10., Opp. Kotak Mahindra Bank, JVPD Scheme, Mumbai between 10.00a.m. To 06.00p.m from Monday to Saturday. Duly filled in offline forms along withparticipation feecan be sent at the above address during the timing mentioned or can also be couriered or posted at the said address. ENTRY RULES 1) Age Criteria: Half Marathon applicant needs to be of 18 years or more on the date of the race, i.e. born on or before25 th February, 2000, 10K Run applicant needs to be of 15 years or more on the date of the race, i.e. born on or before 25 th February, 2003, 4K Run applicant needs to be of 12 years or more on the date of the race, i.e. born on or before25 th February, ) Half Marathon & 10K Run are compulsorily timed categories. 3) You may use a photocopy of the physical or downloaded form from the event website to send in your application. 4) Only one applicant can apply for the Event using a single form. Also, do not send in more than one entry per person; multiple applications will entail all applications being rejected.

2 5) Applicants must submit their entries only via the Application Modes given in this form. No individual/group has been authorized to collect entries on behalf of the Event. 6) GROUP ENTRIES will be accepted for the race categories.half marathon/10k Run/4K Run. 7) Before sending in your application, please check the following a. Filled in details against all mandatory fields prefixed with * in the application form; b. Attached a copy of your latest colour passport sized photograph on the form; c. Attached a copy of your Govt. issued Photo ID proof, i.e. either Passport, Driving License, Election Card, Aadhaar Card or PAN Card. d. Enclosed the requisite application fees (refer to Application Fees chart) e. Signed the waiver in the application form. f. Applicant meets the minimum age eligibility criteria for the chosen race category. 8) Where any discrepancy is found in any of the above mentioned mandatories, your application will not be accepted. 9) On entry confirmation HalfMarathon/10K Run participants will be further classified into age categories for prize money. For more information log on to the event website 10) Note: Foreigners residing in India who wish to participate and be eligible for the prize money have to produce valid proof of residence. The prize money will be given in Indian Rupees only. 11) Valid id is mandatory for all timed race participants. 12) confirmations will be sent to all the participants who have given their valid id in the application form. 13) Requests for cancellation and thereby, refund will not be entertained. 14) Request for change in race category will not be entertained. Be careful while selecting your race category at the time of registration. 15) Confirmed participants will not be given their running bibs on the race day. 16) It is mandatory for race participants to collect their bib and timing chip personally.in case of unavoidable circumstances, confirmed participant can send his/her authorized representative to collect the same. 17) Authorised representative must carry with him/her a. A printed copy of the confirmation (if using a smartphone, a copy of the confirmation forwarded by the participant). b. Photocopy of your (the participant) photographic proof of identity, i.e.: your Passport, Driving License, Election Card, Aadhaar Card or PAN Card. c. Photographic proof of identity of your representative visiting the registration counters on your behalf, i.e: his/her Passport, Driving License, Election Card, Aadhaar Card or PAN Card. 18) Confirmed participant s identity and proof of residence may be verified by a verification agency hired for validating the details provided by you in the form. 19) Kms-Half Marathon/10Kms Run/4Kms Rotary Get Fit Run participant s race day images will be screened visually after the race and if any of the participant is found not wearing the running number bib allotted to him/her, the participant registered for that BIB will be disqualified from the race. 20) This application form and the right to participate in the event and the rights and benefits available to the applicant under this entry form is at the sole discretion of the Event Organiser, Juhu Half Marathon Organizing Committee and cannot be transferred to any other person under any circumstances. The applicant alone shall be entitled to the rights and benefits arising out of such confirmation of participation.
