1 RESEARCH PROJECT RISK ASSESSMENT FORM TO BE FILLED IN BY THE RESEARCHER FOR ALL RESEARCH PROJECTS. THIS FORM NEEDS TO BE SIGNED OFF BY THE PI/SUPERVISOR AND THE HEAD OF DEPARTMENT Geography, School of Natural Sciences

2 RISK ASSESSMENT Important: The researcher, in consultation with the supervisor, must complete this form before any project begins. All designated parties must read and sign the form. Designated parties include; student, supervisor and/or technical officer. The form must also be updated before any major change in either personnel or procedures takes place. A copy of the form must be given to the Discipline Safety Officer. You may submit your form to geography.safety@tcd.ie. Separate assessments are also required for lone working and for certain hazardous activities. Note also that all laboratory work must be completed during normal business hours (defined as Monday to Friday). Where a laboratory manager defines business hours as less extensive than those above, those designated by the manager will apply. Work outside normal business hours is subject to additional risk assessment. 3 SECTION C: HAZARD AND RISK IDENTIFICATION DEFINITIONS: A Hazard is something that has potential to cause injury or damage A Risk is the likelihood of someone being injured by a hazard and the severity of the injury. When identifying and evaluating risks and appropriate control measures you should base your assessment on what a well-informed person would consider reasonable. CONTROL MEASURES: Control measures are actions that are taken to prevent or reduce the risk associated with a particular hazard. They can include hazard elimination and substitution, equipment provision, training, safety documentation, communication systems and protective clothing. MATRIX The following matrix is used to assess risk. Likelihood High Medium Low M H H* L M H L L M Low Medium High Likelihood to be graded as: Low: Rare may only occur in exceptional circumstances Medium: Possible might possible occur at some time High probable expected to occur in most circumstances Severity/Consequences Severity/Consequences to be graded as: Low: No injury or minor injury requiring first aid/doctors visit and small potential cost to employer Medium: Reversible or severe injury requiring hospitalisation and potential cost to the employer High: irreversible or serious injury requiring long term hospitalisation and substantial potential cost to the employer Yielding the Risk to be graded as: L - LOW M - MEDIUM H - HIGH (STOP) H* -VERY HIGH (STOP)

7 REPORTING SYSTEMS FOR FIELDWORK Each field worker should have a named Contact person. The contact person must be informed, before each day s field work of: The field worker s itinerary for the day The planned time of return at the end of the day The field worker should phone or text (or radio) the contact person at or before the return time, either: To confirm safe return To inform of any delay If there is a delay, the field worker must continue to keep the contact person informed until safe return can be reported. PROCEDURE TO FOLLOW SHOULD REPORTING SYSTEM FAIL If the field worker does not make contact within an agreed period after the planned time of return, the contact person must attempt to contact the field worker. If this fails: The Contact person is to contact the Emergency services (dial 999 or 112, or the number of the nearest Garda/Police station). The Contact Person is to contact the Supervisor. If the supervisor is unavailable the contact person is the safety officer for Geography or Geology.
