Date 01 MAY 2018 To undertake a variety of mixed light weight scuba works within the harbour and various marinas on small leisure crafts. The work ranges from predominately hull cleaning to prop clearance / prop entanglement to changing anodes. 3. Task Summary: 1. Once team arrive onsite at the job/vessel, supervisor to check details of job requirements on the online system. 2. Supervisor to register in to the marina office and fill out relevant PTW while dive team set up equipment. 3. Site specific dynamic risk assessment to be completed on the online system, once deemed safe to dive by the supervisor, the surface swimmers to enter the water to assess the condition of the antifoul ensuring that the job it is possible and that the antifoul isn t pluming. 4. Surface swimmers clean the waterline and long reach, when finished the diver and standby diver get dressed in, pre dive checks are carried out and diver to enter the water and complete the clean. 5. While cleaning the boat without using breathing apparatus it is vital that the team work together using the buddy system procedure in RA11, no crossing the keel. If any damage is noticed then photos must be taken and ed to the client asap if possible these should be sent from site and notes should be made on the online system under the boat / job name. Once the vessel has been cleaned or the job is complete, an MMC tag is left on board where it can be easily spotted i.e.: wheel/tiller/ grinder etc. Special Considerations Hull cleaning can be rigorous and requires a high level of manoeuvrability. Where possible it is important to take the work load off the diver, and reduce manual handling in line with HSE guidelines at work INDG Information Supplied by Client: 5. Site Specific Hazards Identified By Client: The information supplied by the client should be or via text / , so that a paper trail between MMC and the client can be found. The vessel details are added to the works order then uploaded to the online system, along with a description of the works intended including the boat name, size, color, location, berth number and date job is requested. The client is never involved with the DPP, but a copy is always available to read on site in the sea file and /or on the online system. MMC must obtain a permit to work through the local harbour authority this form identifies any large vessel movements or works being carried out within the harbour limits. Each marina will also have a signing in and out process or dive permit that must be signed prior to any works being conducted. This document is used to sign in and sign out and gives the divers permission to enter the water. Any hazards or boat movements on the day are identified in the permit to work, and a wide berth/speed reduction is requested. Yes No Issued by 9 Any additional important information provided by the client about the task or dive site: ACOP 74: The diving contractor must specify the size of the dive team based on the details of the diving project and the risk assessment. There must be a sufficient number of competent and, where appropriate, qualified personnel to operate all the diving plant and to provide support functions to the dive team (Marina staff to act as the compliance persons in an emergency). ACOP 77: The required size of the dive team will depend on the risk assessment which should take into account the number of hours to be worked each day, the type of diving, plant and the techniques to be used, any decompression requirements and the appropriate number required for safety. ACOP 78: The minimum team size normally required to conduct a surface-supply dive safely within the scope of this Code is five- a supervisor, a working diver, a standby diver and a tender for the working diver and a tender for the standby diver (see paragraphs 81 and 82). Additional personnel may be required to operate or maintain specialized plant, for example winches, and to assist in an emergency. ACOP 79: The use of SCUBA may be acceptable for simple tasks such as visual inspection in clear water where there is no risk of entrapment and the management of an emergency has been considered (see paragraph 42). The minimum team size normally required to conduct a SCUBA dive safely within this scope of this Code is four a supervisor, a working diver, a standby diver and a tender for the working diver (see paragraphs 81 and 82). Additional people may be required to operate or maintain specialized plant, such as winches, and to assist in an emergency. ACOP 81: The diving contractor should be satisfied that the tender is competent. The tender should be familiar with the diving procedures to be used and the contingency and emergency plans for the project. 7 Part 3 Dive Plan: L104 2nd Edition ACoP Commercial Diving Projects Inland inshore - Diving at Work Regulations 1997 Task description: To undertake a variety of mixed light weight works within the confines of a harbour or marina, these works are carried out around small leisure crafts. The work ranges from: hull cleaning / prop clearance /entanglement / visual inspections and changing anodes, but predominantly hull cleaning. When cleaning yachts, surface swimmers are often sent in first to start the clean. Their job is to swim along the waterline and reach down approximately 1m (long reach), at this time the condition of the antifoul is evaluated and reported back to topside. Swimmers are briefed to use a buddy system so they are visible at all times and not to cross the keel. Once the surface swimmers have completed the initial assessment and clean, the diver enters the water to finish off. 1. After the boat is clean, any issues sub surface are reported to topside / where necessary photos are taken and ed to the client and all details are logged on the online system. If there are no issues subsurface then a note: no issues should be left on the vessels notes section on the system and the boat should be tagged with a black dive completed tag, this way the owner and crew know the divers have completed the works. Any concerns or unusual details noticed by the dive team must be photographed and discussed with the boat owner or skipper before the vessel leaves the dock. Special Considerations Where possible it is important to take the work load off the diver, reducing manual handling in line with HSE guidelines at work INDG143 Site Information: (ACoP Ref 30 c) (Site Specific) A RIB is an ideal vessel for this type of work due to the low soft sponsons, casualty landing net, and ability to work more comfortably between yachts reducing the risk of causing damage, it also enables easy access between pontoons. The RIB is visible to the public with safety signage and rigid A flag flying at all times, and someone on topside keeping a good watch for vessel movements, a horn available on site to warn other vessels. 2. Dive supervisors letter of appointment to supervise diving operations under the approved code of practice for Inshore Diving Operations. Following guidance obtained from the Health and Safety Executive (HSE) it is permissible for my supervisor as Diving Contractor, under the HSE and the Approved Code of Practice interpretation of Regulation 5 and 6, to instruct suitable employees to ensure the competence for each diving operation, prior to the diving operation commencing. Before a diving operation is being conducted, MMC s appointed supervisor has a duty as a named Diving Supervisor to:- 1. Be familiar with the Dive Project Plan under which the Diving Operation is to be undertaken. Review and amend the Dive Project Plan to suit the site specific RAMS or amendments to work (MMC08). 2. Has read, agreed with and have a copy of all the relevant risk assessments on site. 3. Have checked the competencies and qualifications of the diving team. 4. Ensure that good housekeeping is maintained at all times with equipment washed and maintained daily. 5. Be responsible for the dive procedure Operations MMC Q06 (Rev 6) are carried out and completed. The VHF channel is selected for the appropriate marina / harbour, a listening watch is maintained while all work is carried out and the authority updated on regular movements. A full site specific risk assessment will be conducted using the on line system and reviewed throughout the day on each vessel and on each change of location. 3. Where possible the divers work load is reduced by having a surface swimmer/snorkeler to start the work along the waterline and a long reach below waterline. 11 Part 4 Method Statement 1. Detailed Work Summary: This work summary is intended for light weight works within the marina, predominantly hull cleaning, with occasional prop entanglement, item recovery or anode replacement. The team use a 6.0m RIB spread where the diver can access the water easily between vessels. A permit to work is collected from the Harbour office, a list of the day s activities and the divers log can be found on the online system, this can be ed to the Harbour Authority on request. The supervisor must visit to the marina office on arrival at the dive site, on request a VHF call is made prior to divers entering the water and again once the diver exits the water. The dynamic risk assessment is undertaken on site prior to all operations, this is logged using the online system. Once the job is complete the supervisor must sign out by either visiting the marina office or using the VHF radio. An MMC dive completed tag is left onboard for the skipper / owner to see that the job has been done. 2. Access and Site Environment: Access is always by water in our Rib, VHF monitored and authorisation and permits approved prior to entering the water. In many/most of the marinas it is a requirement to sign in and out with the marina office either by VHF or in person. Lifting or associated operations: MMC have put together a method of helping to reduce the work load of the diver by introducing a buddy system See RA11 this enables the person cleaning the boat to do so without breathing apparatus. This reduces all lifting and manual handling. All manual handling in line with the HSE guidance at work INDG143. All work equipment will be certified in line with the requirements of The Lifting Operations and Lifting Equipment Regulations Site Controls / Responsibilities: Dive operations, sub surface works, any works that directly or indirectly may impact the diving operations are under the control of MMC Dive Supervisor. Only the Dive Supervisor can activate a diver, however any member of the dive or site team can call an all stop on operations. The exclusion zone above the working diver is the responsibility of the supervisor, designated diving zones away from overhead works & piling operations will be set out in the daily tool box talk.. 12 Part 5 MMC Gas Calculations and Endurance at Varying Depths MMC Gas Calculations and Endurance at Varying Depths Summary: The breathing rates used across the diving industry are within the limits expected for normal to light work load, they do not address the requirements for heavy workload. It is clear that working divers could only maintain a maximal breathing for a very short period before exhaustion would cause them to reduce the breathing rate or pass out. The amount of reserve gas available needs to allow the diver sufficient time to reach a place of safety. A risk based assessment of gas supply requirements has been conducted to identify the appropriate amount of gas required. A standardised design of supply coupling has been selected to allow provision of emergency gas to the diver. The breathing gas supplied is compressed air. Normal breathing rate for the diver at surface 40 litres a min but increases to 50 litres a min when in a panic, emergency situation or bailout. The stand by diver is calculated at 50 litres a min at all times. The calculations shown below shows only stored volumes without the use of the compressor. Diver and Stand-by Diver: Primary/ main air (40 l/min on the surface) for the diver is supplied from a LP compressor with one storage receiver. The delivery rate is 15 CFM and operating pressure is 11 bar and receiver is a 1 x 150 Litre capacity. 150 Litre x 11 bar = 1,650 Litres of air.
