3 INJURY OR ILLNESS ACTION TO BE FOLLOWED 1 Move a patient only if the person s life is in danger. 2 CALL LEAVE THE LINE OPEN Inform operator 1. Nature of the situation i.e. Violence, Weapon etc. 2. Give location/address of facility Facility Address: Nearest Cross Street: 3. Identify the specific area in the facility. 4. Contact person and location where to meet police. 3 If trained in First Aid, check Airway, Breathing and Circulation (ABC s). If untrained Seek assistance. 4 Liaison with EMS personnel 5 Contact Supervisor as soon as possible 6 Direct media inquiries to your supervisor or supervisor on call.

4 SEVERE WEATHER (TORNADOS, SEVERE STORMS) ACTION TO BE FOLLOWED 1 Notify staff / patrons of the weather threat The announcement should be competed in a calm fashion as not to scare or frighten any of the patrons. Announcement: MAY I HAVE YOUR ATTENTION PLEASE! We are expecting a major storm in the local area. For reasons of safety we ask that all patrons and staff remain in the building and move away from all windows until further notice. Please follow all instructions and directions give by the staff. Thank you Repeat announcement Ensure all out door patrons/staff have been brought into the facility. Move patrons and staff to the facility inner room(s) or inner hallways AWAY from all windows under heavy furniture if possible Evacuate all indoor patrons/staff from Gymnasiums, large open areas, ice surfaces to the facility s inner room(s) or inner hallways AWAY from all windows. Listen to weather on portable radio (AM 680) (battery operated) or contact the weather telephone for Hamilton area Toronto area Evacuate - If determined and as advised by Police, Fire Department or Parks and Recreation Department Should facility suffer damage and or personal injuries occurs CALL Nature of the situation i.e damage or injuries due to serve weather 2. Give location/address of facility Facility Address:

6 MISSING PERSON ACTION TO BE FOLLOWED 1 2 Staff will search the area/building for the missing person. Continue the search for a maximum 15 minutes. If person not located within the maximum 15 minutes; CALL LEAVE THE LINE OPEN Inform operator 1. Nature of the situation i.e. Violence, weapon etc. 2. Give location/address of facility Facility Address: Nearest Cross Street: 3. Identify the specific area in the facility Contact person and location where to meet police. Notify parents/guardians. Liaison with Emergency Personnel. Contact Supervisor as soon as possible Direct media inquiries to your supervisor or supervisor on call.

7 SUSPICIOUS PERSON ACTION TO BE FOLLOWED Assess the situation. If you are in doubt follow the outlined procedure below. 1 Suspicious activity includes; intensely watching young children, lingers for an inappropriate time in the change room, acts in an exhibitionist fashion, asks inappropriate personal questions etc 2 Call non- emergency POLICE at Give specific information to the Police. 3 Liaison with Police upon arrival. If applicable, ask witnesses to give their statements to Police. 4 Fill out the Suspect Identification Sheet and incident report form. 5 Contact Supervisor as soon as possible
