1 Courses can be conducted in multi-languages One set of hardcopy course notes will be sent to client for printing and distribution to course participants. The courses will be held at the client s training room facilities equipped with computer, overhead projector, flips charts, and video players. 1. HAZOPS 2 days HAZOPS is a study team technique for reviewing the design and operation of process units onshore at refineries and chemical plants and offshore production facilities. It identifies potentially hazardous failures in a systematic, thorough and creative way using guidewords to determine what can go wrong outside the design envelope. HAZOPS can be used throughout the development of a project and is normally revalidated throughout the life of a process plant. 2. Safe Ship/Shore Operations - 3 days A Training Course for Maritime & Terminal Personnel to ensure safe operations at the ship/shore interface while handling crude oil and product carriers, chemical tankers and LPG vessels. The course is designed to equip responsible ship/shore personnel with the knowledge necessary to fulfill their joint responsibilities in meeting international standards and statutory requirements on ship-to-shore activities. Interactive discussions between ship and shore experts and videos make the subject interesting and enhance understanding. Provide assurance that personnel receive a level of training necessary to achieve competency in ship/shore interface activities. 3. Static Electricity 1 day This course introduces the two principal hazards of electrical discharge, frequently encountered in petrochemical plants and refineries. Sparks and arcs may ignite mixtures of air and flammable gases or vapors, resulting in explosions and fires. Electrical shock may cause fatalities or serious injuries. Upon completion of this training course the course participant will know: Inherent hazard of flammable liquids in generating static electricity Conductivity and insulation properties of flammable liquids Why it is important to understand static electricity What conditions promote the build-up of static electricity? How a number of actual explosions and fires were caused by static electricity Preventive measures in handling flammable liquids to prevent static discharge covering rail loading, road truck loading, drum filling, ship loading, and storage tank filling. Safe Preparation of Plant for Maintenance - 3 days A 3-day practical workshop to equip operations and maintenance staff with the necessary knowledge to shut down plant and prepare it for maintenance safely. The workshop contains numerous case studies such as Pasadena, Motiva, Piper Alpha, Avon Tosco refinery which were all associated with maintenance and failure of the work permit system. The following modules will be covered: Shut down of the plant Draining, venting and purging Isolation (process, hydraulic and electrical) Testing to include use of explosimeter, oxygen analyzer and toxic measurement tube Entry into confined spaces, ventilation and hot work High pressure water jet cleaning Work on flare systems Hot tapping Turnaround Pre-startup safety review In addition practical risk assessments (job safety analysis) will be demonstrated with exercises to ensure priority and safeguards are directed at the most high-risk tasks. 5. Safe Operation and Maintenance of Storage Tanks 3 days Raw materials and finished products are stored in large storage tanks in your facility. But understanding the complex issues and problems of these tanks and implementing good design, inspection, operational or environmental solutions to storage tanks are not at all easy. Without proper training and a sound knowledge of the most basic concepts about storage tanks, many individuals and companies have made major blunders/errors that result in high costs, shortened equipment life, ineffective inspection and maintenance programs, environmental damage, or accidents and injuries as well as the threats of more national legislation. This course covers the fundamental principles of atmospheric storage tanks that will make any tank facility safer and more reliable, and help any individual, company and industry using atmospheric storage tanks to improve their performance in the areas of safety, environmental responsible operations, and implementation of good practices. Risk Based Process Safety Management /HSE Management Systems 3 days Process Safety Management Systems address the management of process hazards in the design, construction, startup-up, operation, inspection, maintenance and modification of assets with the potential for a major catastrophic release of a toxic or flammable material. Process safety management systems have become an integral part of regulatory requirements in many countries including Europe through the Seveso Directive (COMAH) and the USA through OSHA 29CFR and the EPA Risk Management Program. 7. Job Hazard Analysis/Job Safety Analysis 1 day This course is designed for those who need to systematic identify potential hazards the workplace as a step to controlling the possible risks involved and to understand the requirements for safe systems of work. Much of the course time is allocated to group work on realistic examples drawn from a range of realistic scenarios. During the workshop participants will be trained to breakdown work activities into tasks and identify HSE hazards of each task and to determine if existing controls/safeguards/precautionary measures are sufficient, using a Risk Ranking Matrix. The workshop also includes how to determine if additional safeguards are required to reduce risk to an acceptable low level and how to monitor controls and safeguards. 8. Incident Investigations using various Loss Causation Models including Tripod Beta - 3 days The course demonstrates to the participants how to perform an in-depth and professional investigation following an incident. The participants will be trained to identify the real causes of the incident (root cause analysis), estimate the likelihood of recurrence and potential for major loss, determine any trends by considering similar incidents at other facilities, develop control measures to prevent recurrence or minimize the consequences, as well as to demonstrate corporate management's concerns and/or satisfy regulatory requirements. This course will equipped the participant the following skills and techniques: How to define near misses and how to institute a program to learn all you can from all incidents How to initiate investigation and gather evidence How to apply the Tripod Beta loss causation models for investigating process and non-process incidents How to avoid future incidents by developing appropriate recommendations to address root causes How to structure reports for optimum effectiveness Page 3 of 5

4 Realistic industry examples and workshops are used extensively throughout the course to illustrate key points and to allow participant to practice new skills. 9. Safe Handling of Liquefied Petroleum Gas (pressurized and refrigerated) 2 days This course discusses the Properties and behaviour of LPG, the principles for pressurised bulk storage and road/rail loading of Liquid Petroleum Gas. Incorrect handling of LPG has caused incidents such as Feyzin, Mexico City and the Spanish Campsite Disaster. Guidance notes provides detailed technical information on safety measures relating to the, refrigerated storage and ship/shore loading of LPG including: siting, layout, liquid rollover and bunding, ship arrival, cargo transfer, linked emergency shutdown systems, disconnection of loading arms and refrigerated storage, pressure relief systems, isolation of leaks, detection of leaks, instrumentation for level, temperature and pressure measurement, de-watering and sampling procedures, inspection, fire protection: water drench, fireproofing of structures, road and rail loading facilities and operations. Case studies include: Fire at an LPG recovery installation of a refinery Propane pump seal failure causes major fire Over pressurisation of refrigerated butane storage tank and propylene vapour release during ship/shore loading Overfilling of an LPG vessel Rail loading, a road tanker incident Valve and pump failures cause fire The Spanish campsite disaster and sabotage 10. Safe Operation of Furnaces 2 days Furnaces are deceptive in appearance. They look dangerous when they are in operation, and they appear to be harmless when not in operation. Actually, the furnace may be most hazardous when it is inoperative. This hazardous reveals itself when attempt are made to light the furnace. Most furnace explosions occur when burners are being lighted and result from failure to follow operating instructions carefully. This course will help operator to understand how furnace work and the associate hazards of firing and provide general rules for safe firing. 11. Total Asset Integrity Management 2 days Safe and continued operation of a process facility depends heavily upon the inspection and maintenance performed in the facility. Adopting a balance between under or over inspecting and maintaining process plant affects operational costs and hence profitability. In most facilities, a significant portion of total unit risk is concentrated in a relative small percentage of Page 4 of 5

5 the equipment items. These potential high-risk components may require a greater degree of attention that others, therefore it is important to know which areas to prioritize. This course provides an overview of risk-based approach to Total Asset Integrity Management (TAIM), introduction to the principles of Risk Base Inspection (RBI) and Reliability Centered Maintenance (RCM) and the software tools for: Managing the efforts of the inspection and maintenance program Prioritizing potential process equipment failures so that effective failure management strategies can be developed to control the risk of losses. Potential benefits from this approach are to increase operating times and maintain same level of risk. 12. Safe Storage Tank Design 3 days A comprehensive 3 days course providing vital technical information that you can immediately apply to design storage tanks safely. The course introduces various established standards that can be applied to design and construct new storage tanks, current regulatory requirements relating to storage design and recommended good practices for constructing of tanks to meet operating standards. Discuss design, capacity and cost factors to be considered in optimizing tank selection and other factors include corrosion prevention systems and environmental regulations. 13. Confined Space Entry and Hot Work 2 days The hazards associated with confined spaces can be particularly dangerous to the inexperienced person. Confined spaces have the potential to kill without warning and in the worst-case scenarios they can cause multiple fatalities. It is therefore imperative that anyone who is required to perform confined space entries in the workplace is trained to perform this function efficiently and effectively. This confined space training course initially covers the following general aspects of confined space safety; what is a confined space, how it is defined, why are confined spaces hazardous, what are the hazard, etc. The participants will be taught the ten steps process on how to perform a confined space entry. The roles and responsibilities of the Entrant and Attendant, emergency responses and rescue during confined space operations. 14. Integration of HAZOPs with LOPA 2 days 15. Process Safety in Project Management 1 day 16. Quantitative Risk Assessment 5 days 17. Fundamentals of Process Safety Management 2 to 4 days Page 5 of 5
