PURPOSE The BASF Corporate procedure for Confined Space Entry, BC Section 4.1, General Requirements, states that each facility shall evaluate the workplace to identify all confined spaces as defined in the procedure. Specific guidance is given in the corporate procedure to accomplish this directive. It is then the purpose of this procedure to establish work practices for safe entry and work within a confined space after the space has been evaluated and deemed necessary to require an entry permit. 2. DEFINITIONS 2.1. Attendant An individual stationed outside one or more Confined Spaces who monitors the Authorized Entrant(s) and who performs all Attendants duties required by the site Confined Space Entry program Authorized Entrant An individual who is authorized, by the site s Confined Space Entry program, to enter into a Confined Space Bodily Enter For the purpose of Confined Space evaluation, means that a potential Authorized Entrant's entire body (including head, torso, and all extremities) can pass through an opening and fit within the space in question and perform the assigned work Contractor/Agency A company which will be either placing or bringing Contractor(s), Temporary Employee(s) or Contractor Employee(s) onto a BC site Contracted Employee A Contracted Employee is an individual or group of individuals for whom BC provides direct supervision in their day-to-day activities and direction as to how an activity is to be performed.
2 Procedure No.: MC Page: 2 of Contractor Employee An individual engaged by or through a third party to perform services principally for the third party and for whom BC does not provide direct supervision in their day-to-day activities and direction as to how an activity is to be performed Confined Space A space, (such as tanks, vessels, silos, storage bins, hoppers, vaults, pits, ditches and sewers) that: is large enough and so configured that a person can bodily enter and perform assigned work; and has limited or restricted means of entry or exit; and is not designed for continuous employee occupancy Limited or Restricted Means of Entry or Exit 2.9. Entry The inability to walk upright through a space entrance and or has significant obstructions within the space that would impede an entrant s ability to exit the space. The action by which a person passes through an opening into a Confined Space. Entry includes ensuing work activities in that space and is considered to have occurred as soon as any part of the entrant s body breaks the plane of an opening into the space Entry Supervisor The person (such as supervisor, resource personnel, operations owner or crew leader) responsible for determining if acceptable Entry conditions are present at a Confined Space, authorizing Entry, and overseeing Entry operations. NOTE: An Entry Supervisor also may serve as an Attendant or as an Authorized Entrant, as long as that person is trained and equipped as required for each role he or she fills. Also, the duties of Entry Supervisor may be passed from one individual to another during the course of an entry operation Explosive Atmosphere A range of concentrations of gases, dusts, or fibers with air that can support ignition. The range of concentrations is material-specific. The range for a material can vary based on pressure, temperature, and oxygen content.
3 Procedure No.: MC Page: 3 of Exposure Limit Value as published in ACGIH TLVs used as the primary guide in determining entry condition unless the OSHA Permissible Exposure Limit (PEL) or BASF internal exposure limit is more stringent. If no exposure limit has been established, MSDS, published information and process knowledge can provide guidance in determining acceptable atmospheric conditions Hazardous Atmosphere An atmosphere that may expose personnel to the risk of death, incapacitation, impairment of the ability to self-rescue, injury, or acute illness from one or more of the following causes: flammable gas, vapor or mist in any detectable concentration using a calibrated combustible gas indicator operating in the percent range of sensitivity. airborne combustible dust at a concentration that meets or exceeds its lower flammable limit. This concentration may be approximated as a condition in which the dust obscures vision at a distance of 5 feet (1.52 m) or less. atmospheric oxygen concentration below 19.5 percent or above 21.0 percent. atmospheric concentration of any substance that could result in employee exposure in excess of its occupational exposure limit. any other atmospheric condition that is immediately dangerous to life or health. Refer to Attachment 1 - BASF Exposure Assessment Occupational Exposure Limit Reference Value Determination in establishing acceptable occupational exposure limits in Section 6 Related Documents Immediately Dangerous to Life or Health ( IDLH ) Any condition that poses an immediate or delayed threat to life or that would cause irreversible adverse health effects or that would interfere with an individual s ability to escape unaided from a permit required space. An example is an oxygen deficient atmosphere containing less than 19.5 percent oxygen by volume Monitoring The process by which the hazards, that may confront entrants in the space, are identified and evaluated Operating/Owing Department The BASF unit or department that operates or is responsible for the daily operation of the process, area, or facility where work is being conducted.
4 Procedure No.: MC Page: 4 of Permit Revalidation The process of reviewing the work conditions specified on an existing permit(s), with a Permit Acceptor or a member of the work crew, to ensure the scope, conditions and safety precautions listed on the existing permit have not changed Permit System The site's written procedure for preparing and issuing permits for Entry and for returning the permit space to service following termination of Entry Rescue Service Personnel designated to rescue persons from Confined Spaces Retrieval System 3. SCOPE Equipment including (retrieval line, chest or full-body harness, wristlets if appropriate and a lifting device or anchor) used for non-entry rescue of persons from permit spaces. This procedure applies to all employees, contractors and visitors of the McIntosh site whose work involves entering a confined space. 4. PROCEDURE 4.1. General Authorized Entrants shall be made aware that they may witness the initial and follow-up testing of the confined space in which they are to enter In addition to a Confined Space permit, a Safe Work Permit is also required prior to entering any confined space. Both permits are to be filled in completely. The entry crew copy of the Safe Work Permit and Confined Space Permit shall be posted near the entry point of the confined space. Refer to MC (Safe Work Permits) for specific requirements related to completion of Safe Work Permits. Use of Safe Work permits/loto sheets for Confined Space Entries will be as follows: A Safe Work permit and LOTO sheet will be completed for placement of blinds during isolation of the Confined Space.
5 Procedure No.: MC Page: 5 of 19 Once blinds are in place, another Safe Work permit and LOTO sheet will be completed for the Confined Space Entry itself, siting the blinds as the isolation points. The bottom of the tags from the blinds should be placed in the lockbox. o The maintenance crew member(s) who installs the blinds is required to place a tag on each blind. Anyone going in the vessel is also required to place a tag on each blind. (It is not a requirement for the bottom portion of these tags to be placed in the lockbox. Only the bottom portion of the operators tag is required in the lockbox.) o As a part of the LOTO sheet, the valve used to supply air for the air horn will be tagged OPEN. The tag will specify that the air is to be used exclusively for the air horn Confined spaces must be identified in the field by appropriate danger or warning signs Openings of a confined space must be protected, demarcated, or barricaded to prevent persons from falling or stepping into the opening, as well as to prevent unauthorized access when the entrance is not attended by the hole watch or before permit has been issued) Authorized Entrants must be connected to a retrieval system. This includes wearing a full body harness attached to a retrieval line in good condition. Wristlets may be used if the use of the full body harness is infeasible or creates a greater hazard. The retrieval line shall be firmly attached outside the confined space to either a fixed point or mechanical device to facilitate retrieval without restrictions. For a vertical entry of more than 5 feet, a mechanical retrieval device shall be used. NOTE: A retrieval system is not required if it increases the overall risk to Authorized Entrants or does not contribute to the rescue. The entry supervisor must assess each confined space entry to determine the potential hazards of retrieval systems to Authorized Entrants before allowing entry without a retrieval system Electrical equipment, other than lighting, may be 120 volt if protected by ground fault circuit interrupter and meets area electrical classification Adequate illumination shall be provided, using vapor proof flashlights or approved low voltage illumination units. The units shall be 12 volt maximum, including associated wiring, inside the confined space When a portable ladder is required to enter a confined space, the ladder shall be readily available for exit to anyone in the confined space.
6 Procedure No.: MC Page: 6 of If a confined space must be entered to test the atmosphere, then air supplied respiratory protection is required while testing the atmosphere of the confined space. A provision must be made for emergency exit, such as escape pack, in the event the breathing air supply becomes unusable. EXCEPTION: Respiratory protection is not required for initial entry into open-air confined spaces with no potential for oxygen deficiency, flammable or toxic vapors Depending on activities, such as grinding, that will take place inside the CSE, issuance of the permit should include a discussion of the specific respiratory protection needed Preparation Isolation A. A confined space shall be isolated from ALL adjoining piping, including relief headers, drains and overflow lines, in order to insure that liquid, vapors or gases will not enter the confined space. Every effort should be made to isolate the confined space as close to the vessel as possible. A line shall be deemed isolated if one or more of the following steps are taken: A blank or stopper plate has been inserted in the line(s) and Owner Tags are affixed to the blind. The construction and thickness of the blank or stopper plate must sustain the pressure for which the line is rated. The Owner Tag numbers shall be listed on the corresponding LOTO sheet. A disconnect created by removing a spool piece or mis-aligning the process or utility piping so as to prevent liquids or vapors from entering the space. If the confined space contains an internal power driven mechanism, such as agitators, all fuses or safety jacks must first be pulled and all switchgear locked out; other power sources must be locked/tagged out in conformance with the existing LOTO Procedure. The key for the lock on the switchgear shall be kept by an Authorized Entrant for the confined space. For situations in which Lockout/Tagout devices must be temporarily removed from the Lockout Device and the machine or equipment energized in order to test or position the machine,
7 Procedure No.: MC Page: 7 of 19 equipment or component, the following sequence of actions will be followed: o Approval must be obtained from the Operating/Owning Department and authorized employees. o Machines or equipment must be cleared of non-essential tools and materials, and assurance made that components are operationally intact. Personnel must be cleared from the machine or equipment to ensure that all are safely positioned. o Testing shall be performed in the following sequence: The work crew lock/tags are removed first. Operating owning department locks/tags are then removed. The operating/owning department will then restore all valves, switches, etc. to their normal position and perform the test. Equipment/machines must be de-energized and energy control measures previously in place must be restored before servicing activities may resume. Utility lines to jackets, coils, and reboilers must be isolated. One exception to isolation of the utility lines is when cooling water is used on external coils / jackets during extreme hot weather. However, it must be proven that the jacket / coil will not leak into the confined space. For vessel jacket service only, the line can be isolated by a double block and bleed, where two block valves are closed and the bleed is opened between the two. Equipment Owner locks/tags must be affixed to the block and bleed valves. A true double block and bleed must be used, where the three valves are located in the close proximity to one another and are designed to be used as a double block and bleed. B. Isolation practices must be consistent with those described by MC (LOTO and Line Breaking) Cleaning Before entry, any confined space and inter-connecting equipment not isolated shall be cleaned of prior contents by washing, neutralizing or purging to eliminate flammable, toxic, corrosive or noxious materials, gases or vapors Atmospheric Testing
8 Procedure No.: MC Page: 8 of Order of Atmospheric Testing Atmospheric testing must be conducted in the following order: Oxygen Flammable/Combustibles Toxic Substances/Particulates All initial monitoring for atmospheric conditions shall be conducted as close to entry time as possible and at no time shall it exceed two hours prior to entry. If the space is unoccupied due to work breaks or delays, monitoring shall be performed as close to entry time as possible and at no time shall it exceed two hours prior to entry Continuous monitoring is required for oxygen content and flammability, while personnel are working in a confined space. The entry attendant shall monitor the space using a combustion gas meter with extended sampling line placed in the work area Location of Testing Testing shall be conducted at various heights and lengths throughout the entire space. NOTE: Initial entry by operations personnel, in order to verify atmospheric conditions prior to performing work, is not required if representative sampling from outside the confined space can be accomplished Instrumentation Maintenance and Calibration Monitoring instruments must be maintained properly and, at a minimum, functional tested or bump tested daily or before each use. Calibration must be performed at least monthly. Documentation must be maintained to demonstrate testing and calibration of the monitoring instruments. The last four digits of the serial number will be used to identify a gas meter and number will be recorded on the Confined Space Permit Acceptable Entry Conditions Oxygen A. When the level of oxygen is between 19.5% and 21.0% and the integrity of the atmosphere cannot be guaranteed, air supplied respiratory protection shall be used.

When the level of oxygen is between 19.5% and 21.0% and the integrity of the atmosphere can be guaranteed, entry may be made without respiratory protection provided the criteria for flammables/combustibles and toxins are met. C. Entry is prohibited if the oxygen and flammable concentration limits cannot be met. Exemptions can only be granted by the EHS Department after a thorough hazard review has been conducted. A Management of Change will be required for any exemptions Toxic gas/vapor A. <50% of the exposure limit - no respiratory protection required B. > 50% of the exposure limit - respiratory protection is required Entry is prohibited if toxic gas/vapor concentration exceeds published IDLH value for the chemical. Exemptions can only be granted by the EHS Department after a hazard review has been conducted. A Management of Change will be required for any exemptions. C. Appropriate skin protection may be required to prevent exposure, especially to chemicals which are skin absorbed or corrosive. D. If there is no published exposure limit and/or if there is no monitoring method available, alternate means must be used to determine level of personal protective equipment needed, e.g. ph, ph of wash water, wipe test, etc. Consult EHS group for assistance Flammable Gases/Vapors Confined Spaces containing detectable flammable gases/vapors using a combustible gas indicator operating in the percent range of sensitivity shall not be entered Combustible Dust A. Confined spaces containing airborne combustible dust at a concentration that meets or exceeds the lower flammability limit (LFL) or obscures vision at a distance of five feet or less shall not be entered. B. If work activities performed in a Confined Space generate combustible dust at a concentration that obscures vision at a distance of five feet or less, work activities must cease and immediate corrective action
10 Procedure No.: MC Page: 10 of Nuclear Devices must be taken to reduce their concentration (such as increase ventilation and / or reduce rate of dust generation Nuclear devices shall be rendered inoperative and properly shielded by a Radiation Safety Officer or an authorized instrument technician under the direct supervision of a Radiation Safety Officer A radiation survey must be completed before the confined space is entered. Refer to the site Radiation Policy for specific requirements applicable to nuclear devices Hazardous Energy Sources Introduced into Confined Spaces Consideration must be given to potential hazardous energy sources that are introduced into confined spaces as part of the work activities being performed. Such energy sources include, but are not limited to, the following: Hydro-blasting equipment Abrasive blasting equipment Welding, burning, and torch-cutting equipment (refer to 4.9) Hoses (air, water, vacuum, etc.) Portable power tools (e.g. Electrical or pneumatic tools, spark testers, electrical non-destructive test equipment) When such equipment is not in use and is unattended by the using work crew, the following practices shall be used to prevent inadvertent operation when other personnel (i.e., another work crew) enter the space: A. Remove the equipment from the confined space; disconnect the equipment (e.g. Portable power tools, utility hoses, etc.) from the energy source; and/or shut off the power source. B. Have each of the other work crew entrants lock/ tag (Maintenance Tag or Operations Tag) the power source or the disconnected hose(s) to indicate that the source cannot be re-energized. This specifically applies to the following equipment: Hydroblasting equipment Authorized Entrants shall hang a tag on Hydroblasting crews lock box. Abrasive blasting equipment Authorized Entrants shall hang a tag on the compressor controls or the disconnected air hose. Heaters (e.g. for curing of liners) Authorized Entrants shall hang a tag on the heater controls.
11 Procedure No.: MC Page: 11 of 19 Metalizers Authorized Entrants shall hang a tag on the control source. Vacuum units Authorized Entrants shall hang a tag on the vacuum source or the disconnected hose Ventilation C. After exiting from the Confined Space each entrant must remove their locks/tag(s), which were placed on any contractor equipment per #2 above Adequate ventilation (natural or mechanical) is required to ensure safe air supply to persons working in the confined space Where natural or mechanical ventilation cannot provide adequate oxygen supply ( %), approved air supplied respiratory protection is required. (See 4.12 Retention of Permits as Employee Exposure Records) Air movers used as ventilation devices must be capable of moving sufficient air exchange to workers location in the confined space(s) and: A. Must be properly grounded and secured to the confined space opening; B. Must not use utility air backed up by nitrogen; C. And must be tagged sufficiently to prevent removal or shutdown Any oil lubricated air compressor used for ventilation must meet carbon monoxide (CO) requirements. OSHA states that oil lubricated compressors must have either a high temperature or a carbon monoxide alarm or both. If only a high temperature alarm is used, carbon monoxide must be monitored at sufficient intervals to assure the 10 ppm limit is not exceeded Tank/Truck Car Confined Space Entry In addition to all other requirements: The tank/truck car will be prepared for opening and entering depending upon the specific contents and type of tank/truck car. Tank/truck car will be cleaned by washing, neutralizing and purging to eliminate all noxious, toxic, corrosive and flammable materials. Additional unit specific preparations may be required. Tank/truck car containing flammable vapors shall not be entered Prior to entry, air purge must be established.
12 Procedure No.: MC Page: 12 of Ladders provided for entry and egress must remain in place at all times personnel are inside Welding in a Confined Space Gas cylinders and welding machines must be located outside the confined space When welding inside a confined space is suspended for any substantial period (30 minutes or more), all welding whips shall be disconnected from the leads and the welding machine must be disconnected from the power source, if electrical, or engine shut-off if motorized Gas welding/cutting equipment, when not in actual use, shall be shut off and removed from the confined space in which it is being performed Mechanical ventilation is required for welding in a confined space Approved supplied air respiratory protection is required by all entrants in confined spaces where welding is taking place, and must continue to be used after welding is complete unless the O2 and LEL levels are retested A Management of Change will be required for exceptions to the above conditions Rescue Plan The McIntosh site Emergency Response Team (ERT) will develop rescue plans for each confined space entry. For planned confined space entries, the Incident Commander should be given 24 hours notice so that a rescue plan can be developed. However, in no case should the entry activities commence without the rescue plan in place. Then, at the time of the entry, the Incident Commander shall be notified by Operations/Owning department of the planned confined space entry, prior to commencing entry activities In the event of a rescue, the operating unit will: A. Immediately recheck area when rescue is requested to determine that all hazardous energy sources remain isolated (locks, tags, blinds, etc.) and retest confined space atmosphere with backup test equipment, if available, and communicate findings to rescue team. B. Provide additional assistance to rescue team as requested Notification to the site and rescue team shall be either:
13 Procedure No.: MC Page: 13 of 19 A. Notification by radio to Incident Commander on E-Net channel by Operations/Unit/work crew. B. Dial 2222 on any BASF telephone Request for the rescue team will result in the rescue team being dispatched In the event that the Rescue team is called to perform a rescue, all ongoing confined space entries on site will be suspended until the rescue has ended Confined Space Permit Revalidation If a change in Entry supervision occurs, the on-coming Entry Supervisor must revalidate or reissue the Confined Space Entry permit and inform representative personnel involved in the Entry of the change in supervision Permit Revalidation includes the review, agreement of permit conditions and signatures of the permit acceptor and Entry Supervisor Permit Revalidation is not allowed under the following situations: There has been an isolation modification Changes in work scope A new hazardous energy source is introduced Change in company/vendor performing work Work has been suspended for longer than 2 hours Confined Space Permit Closeout and Debrief Upon completion or discontinuation of the Entry, the Confined Space Entry permit shall be returned to an Entry Supervisor for debriefing and closeout Any hazards encountered during an Entry shall be noted on the Confined Space permit, so that appropriate revisions to the site program can be made as soon as feasible Authorized Entrants, Entry Supervisor, and Authorized Attendant shall be debriefed concerning the program and any hazards created or confronted during Entry at the end of any Entry operation Completed Confined Space Entry permits shall be retained for the period necessary to facilitate the program review stated in Section 4.15, but no

Permits do not have to be retained after the review period but documentation of the review results and corrective measures is required Retention of Data as Exposure Records For any Confined Space Entry in which testing indicates that personnel actually entered a contaminated atmosphere, the data must be retained for 30 years as a record of exposure. This includes initial entries made by the person(s) performing the atmospheric testing. This data must be retained even if PPE was worn to prevent actual exposure to the personnel. The data must be entered into the Corporate Industrial Hygiene Database. A copy of the Confined Space Permit must be forwarded to the site Industrial Hygienist, who will complete the data entry process. Examples of contaminated atmospheres include: Atmospheres containing less than 19.5% oxygen; Atmospheres with a flammable gas/vapor reading greater than zero (0); and Atmospheres with a toxic substance(s) reading which indicates that the level is greater than zero The Equipment Owner is responsible for reviewing the permits to determine if any data should be kept as exposure records, per the above criteria Training Training is required to ensure that persons involved with confined spaces understand their roles and responsibilities and that those persons can recognize the hazards associated with confined spaces This training shall be conducted, documented, and proficiency verified prior to assignment and at least every three years For rescue personnel, rescue training is required on an annual basis Program Review The effectiveness of Confined Space Entry practices shall be reviewed and documented at least annually to ensure that the Confined Space Entry management system is properly implemented and the elements adequately control Confined Space Entry activities. Documentation will be captured in the EHS Activities application within the Lotus Notes EHS Portal.
15 Procedure No.: MC Page: 15 of RESPONSIBILITIES 5.1. Equipment Owner The following minimum elements of the program shall be included in the review: A. Review the site procedure and permit to ensure all currently required elements are included. B. A review of all incidents and other nonconformance reports that involved Confined Space Entry activities and have occurred since the last Confined Space Entry management system annual review. The review should verify root causes have been properly identified and appropriate corrective actions have been implemented. C. A review of the completed permits to determine if the effectiveness, completeness, and accuracy of the permit system. D. Inspection of at least one Confined Space Entry job-in-progress to verify site practices comply with the site procedure. The inspection shall include verification of the training personnel involved in this jobin-progress have received. E. Interviews with a representative number of Permit Issuers, persons performing Confined Space Entry, and other affected employees, including Contractor and Contracted Employees where applicable, to verify they understand their role/responsibility. F. A review of applicable equipment and instrument maintenance and calibration records per the requirements of the standard. It is the responsibility of the equipment owner to: Evaluate and identify confined spaces in respective unit/area. Participate in the annual review of the confined space program. Ensure that completed Confined Space Permits are retained for one year. In the event of an incident involving a confined space entry, the associated Safe Work Permits and Lockout Tagout Sheets should be retained as part of the incident records, in addition to the Confined Space Permit Entry Supervisor It shall be the responsibility of the entry Operations Leader, or their designee, to:
16 Procedure No.: MC Page: 16 of 19 Prepare, authorize by signature, issue, and terminate confined space entry permits. Know the hazards that may be faced during entry including information of the route, signs and symptoms, as well as consequences of exposure. Notify the Incident Commander of planned entry so ERT can review confined space and prepare rescue plan. Please refer to section 4.10 for more details relating to preparation of the rescue plan. Personally verify, in the field, that all tests, procedures and equipment specified by the permit are in place before endorsing the permit and: Isolation procedure is complete Required personal protective equipment and rescue equipment is provided and in good condition Proper ladder is available if required Provide entry attendant and Authorized Entrant personnel for initial entry. Provide attendant with a two-way radio and or audible alarm device for communication. Ensure that approved illumination is provided. Verify that rescue services are available and can be summoned when needed. Verify that appropriate respiratory protection is available for emergency use. Assure that acceptable conditions are maintained throughout the entry. Assure that proper decontamination procedures and equipment are in place as the job dictates. Terminate the entry and cancel the permit as required. Notify the Incident Commander once entry activities are completed. Ensure training of attendants and Authorized Entrants Initial Entrant
17 Procedure No.: MC Page: 17 of 19 NOTE: Initial entry by operations personnel, in order to verify atmospheric conditions prior to performing work, is not required if representative sampling from outside the confined space can be accomplished. The sampling must meet requirements identified in It is the responsibility of the person entering the space for initial entry to: Know the hazards that may be faced during entry including information of the route, signs and symptoms, as well as consequences of exposure. Carefully inspect respiratory protection and all other personal protective equipment, harness and rescue lifeline. Verify that all required preparations/procedures are completed prior to initial entry. Test the atmosphere of the space in accordance with section 4.3 Use supplied air respiratory protection and other personal protective equipment as specified on the permit. Exit the space immediately if ordered, as conditions indicate, or if an evacuation alarm is activated. Properly decontaminate all personal protective equipment, as needed, when leaving the confined space. Initial permit where results of atmospheric monitoring are required Authorized Entrant It is the responsibility of the person entering the space to: Observe the initial and follow up testing of the confined space, if desired. Carefully inspect and use supplied air respiratory protection and other personal protective equipment as specified on the permit. Know the hazards that may be faced during entry including information of the route, signs and symptoms, as well as consequences of exposure. Exit the space immediately if ordered, as conditions indicate, or if an evacuation alarm is activated. Properly decontaminate all personal protective equipment as needed when leaving the confined space.
18 Procedure No.: MC Page: 18 of 19 Maintain communication (i.e., radio, voice, visual, etc.) with the attendant as specified by the Safe Work Permit Entry Attendant It is the responsibility of the attendant to: Be knowledgeable of all emergency procedures, equipment and alarms, rescue procedures and equipment, and hazards encountered in a confined space. Know the hazards that may be faced during entry including information of the route, signs and symptoms, as well as consequences of exposure.. Sign the permit accepting responsibility for the entry crew. Monitor Oxygen content and flammability. Check rescue equipment for proper operation prior to any entry. Maintain entry log and constant surveillance at man-way at all times while entry is in progress. Attendant may not perform other duties that might interfere with primary duty to monitor and protect entrants. Prevent unauthorized entry into the space. Remain outside the confined space during entry at all times until relieved by another authorized entry attendant. Order the entrants to evacuate if prohibited condition occurs. Maintain communication (i.e., radio, voice, visual, etc.) with the entrant(s) as specified by the Safe Work Permit. Immediately sound the alarm and summon help should an emergency occur. The attendant, MUST NOT, under any circumstance, attempt rescue without proper rescue training, supplied air and backup Site EHS Department Maintain, update, and interpret this procedure as needed Certify annually that this procedure is current and accurate.
