Max: 16 teams. Put together your 4person team. We ll help to find you a player or two if you need. Players must each be rated under 1200 USATT rating. If you re not sure of your rating, we will help.

2 NOTE: Tournament director has the right to estimate nonrated players and to disqualify any player appears to be above USATT 1200 level. Teams will be grouped to play roundrobin and two winning teams per group will advance into single elimination playoff. Each teamtoteam match will be in a sequence of 2 singles matches, 1 doubles match, 2 singles matches, 1 doubles match, and 1 singles if needed. The first team in the teamtoteam competition to win 4 matches will be the winner of the teammatch. For instance: Team1 (A, B, C, D) vs Team 2 (V, X, Y, Z) has the sequence: A vs V, B vs X, CD vs YZ, C vs Y, D vs Z (if needed), AB vs VX (if needed), (any A/B/C/D) vs (any V/X/Y/Z) if needed. All matches are best of five 11point games Each match win is 1 point. Each match loss is 1 point. Each match default is 2 points to the defaulting team. Winning team still gets 1 point. Two teams with the most points in the round robin group advance to playoff. First prize: $1000 per team. Second prize: $500 per team. Under6200 4person team: The total USATT ratings of all 4 players must be 6200 or under. NOTE: All players in this event must already have a USATT rating. Max: 16 teams. All teams will be grouped to play roundrobin and two winning teams of the group will advance into single elimination playoff. This highlevel team event requires strategic placement of your players on the roster with the objective of reaching 3 match wins first. Team captains must submit your team roster to the tournament director before their team match. Teamtoteam match will be as the following scenario: Team 1 (players A1, B1, C1, D1) vs Team 2 (players A2, B2, C2, D2). Playing sequence: 2 singles: A1 vs A2 and B1 vs B2 1 doubles: C1+D1 vs C2+D2 singles: C1 vs C2 (if necessary) singles: D1 vs D2 (if necessary) All matches are best of five 11point games Each match win is 1 point. If you don t have a team, sign up online at cosmostt.com by the deadline and we will find you a team. You ll get full refund if we can t place you. This is a USATT 2star sanctioned tournament. All players must be a current member, or join the membership of, the USATT or affiliated ITTF organization. USATT membership costs $49/year for adults and $25/year for juniors. All teams ratings will be as of Feb 19, 2015 on USATT website (1 st day of the Year of the Sheep). Players can only participate in one event (either U1200 or U6200). IN CONSIDERATION of being permitted to participate in any way in USA Table Tennis Sanctioned events, I and/or my minor child, our personal representatives, assigns, heirs, and next of kin: 2. ACKNOWLEDGE, agree, and represent that I and/or my minor child understand the nature of Table Tennis Activities and that I and/or my minor child are qualified, in good health, and in proper physical condition to participate in such Activity. I further agree that if at any time I believe conditions or equipment to be unsafe, I and/or my minor child will immediately discontinue further participation in the Activity. 3. FULLY UNDERSTAND that (a) TABLE TENNIS ACTIVITIES INVOLVE RISKS AND DANGERS OF SERIOUS BODILY INJURY, INCLUDING PERMANENT DISABILITY, PARALYSIS AND DEATH ("RISKS"); (b) these Risks and dangers may be caused by me and/or my child's own actions, or inaction, or the actions or inaction of others participating in the Activity, the condition in which the Activity takes place, or THE NEGLIGENCE OF THE "RELEASEES" NAMED BELOW; (c) there may be OTHER RISKS AND SOCIAL ECONOMIC LOSSES either not known to me or not readily foreseeable at this time; and I FULLY ACCEPT AND ASSUME ALL SUCH RISKS AND ALL RESPONSIBILITY FOR LOSSES, COSTS, AND DAMAGES I and/or my minor child incur as a result of my participation in the Activity. 4. HEREBY RELEASE, DISCHARGE, AND COVENANT NOT TO SUE USA Table Tennis, their respective administrators, directors, agents, officers, officials, volunteers, and employees, other participants, any sponsors, advertisers, and if applicable, owners and lessors of premises on which the Activity takes place, (each considered one of the "RELEASEES" herein) FROM ALL LIABILITY, CLAIMS, DEMANDS, LOSSES, OR DAMAGES ON MY ACCOUNT CAUSED OR ALLEGED TO BE CAUSED IN WHOLE OR IN PART BY THE NEGLIGENCE OF THE "RELEASEES" OR OTHERWISE, INCLUDING NEGLIGENT RESCUE OPERATIONS; AND I FURTHER AGREE that if, despite this RELEASE AND WAIVER OF LIABILITY, ASSUMPTION OF RISK, AND INDEMNITY AGREEMENT I and/or my minor child, or anyone on my and/or my minor child's behalf, makes a claim against any of the Releases, I WILL INDEMNIFY, SAVE, AND HOLD HARMLESS EACH OF THE RELEASEES from any litigation expenses, attorney fees, loss, liability, damage, or cost which may incur as the result of such claim. 6.
