2 2 Application as FIFA Preferred Producer for the FIFA Quality Programme for Football Turf In order to become a Preferred Producer for the FIFA Quality Programme for Football Turf, a set of requirements must be fulfilled in order for the company to be in line with the quality aspirations of the FIFA Quality Programme. For your application, you must fill in the form on the next pages and submit it signed with all required documents to FIFA. All documents must be submitted in electronic form. 3 Application as FIFA Preferred Producer for the FIFA Quality Programme for Football Turf 3 Eligibility criteria: 1. Applicant must have been a licensee of the FIFA Quality Programme for Football Turf for a minimum of two years. 2. Applicant must have a minimum of ten FIFA-certified fields (dates on certificates are decisive) at the moment of application. 2. Details of any distributor/sales, partner/affiliate, company/affiliate, individual or anyone legally representing the applicant. Full details must include name, contact details, country, confederation and the exact relationship between applicant and affiliate. A producer must acknowledge that it is responsible for all actions of their related third parties in connection with the producer or a field connected to the producer. 3. Applicant must confirm that the company has not had a legal dispute or commenced legal proceedings in the past two years with FIFA or a member association of FIFA. 4. Applicant must provide a valid pledge of the WFSGI (World Federation of Sporting Goods Industry) for all company owned manufacturing plants to proof compliance with the Code of Conduct. 3. List and details of all production facilities. 4. Full details of the civil engineer employed by the company including contract, CV and related qualifications (certificates). Experience must be documented within the curriculum vitae, indicating which football/sports fields have been installed by the engineer. Applicant must confirm these criteria in a written statement signed by two directors of the company. Application criteria: 1. List of all full-size artificial turf fields (according to the Laws of the Game) installed in the past 12 months (attached). Fields installed with products that have successfully been tested by a FIFA-accredited test institute should be listed with the date of installation, location, country, product installed as well as contact details of the site and the person responsible for the field. Fields with products that have not been lab-tested (other quality) should be listed with installation date, location, country and products installed. 5. Full details including curriculum vitae and contract of the person(s) employed to oversee maintenance and technical after-care of all installations.
