Top Dog Sponsor - $5,000 Limited to one sponsor, this exclusive option gets your company name on everything related to this golf tournament the slideshow, every document/e-mail (expect photos, scorecard and cart signs) and special signage. Dinner for Golfers $2,000 Limited to two sponsors, this premier option includes dinner and networking for two representatives Special Access to Leaders of Your Choice $1,500 Name your ideal networking opportunity! Sponsor provides three names of members you would like to golf with and we will work to get you one of these people. Nobody is off the table you name the person, we ll deliver! AWARD SPONSOR $1,500 Limited to one sponsor, support covers prizes for skill game winners and first, second, tenth and last place teams. Drive Beverage Cart $1,000+ Limited to three sponsors, the sponsor also pays the course for what is consumed. Sponsor is acknowledged and can drive one of three beverage carts on the course. Golf Hole Sponsor $1,000 Limited to 18 sponsors, this popular option includes lunch, dinner and networking for two representatives from your company. A table and chair/s will be provided at tee box. We will provide shuttle service to/from hole or your own cart. Food and beverage distributed on the hole must be purchased through the course. To avoid duplication of activities at the holes, the Foundation requests you share what you are planning to provide. Keep in mind there will be a trophy for the sponsor with the best spirit! Add a golf foursome for $750. Electronic Scorecard/Cart Sign Sponsor $1,000 Limited to one sponsor, your company logo will appear on scorecards and cart signs. Includes lunch or dinner and networking for two representatives from your company. Add a golf foursome for $750. Golf Photos $1,000 Limited to one sponsor, enjoy a professional photographer arranged by the Foundation who will take action shots of golfers. Photos will be shown during dinner with your logo and posted on social media sites following the event. This popular option also includes lunch and dinner. Reception/Dinner Beverage Ticket $500+ Limited to two sponsors, your company logo will be printed on the tickets. Putting Green Sponsor - $500 Limited to one sponsor, this option includes the opportunity to meet and greet golfers, and logoed signage at the putting green. Includes lunch and networking for two representatives Golfer Registration Giveaways $250 Sponsor provides 150 items approved by the Aging Services of Minnesota Foundation. Items are included in the registration materials for golfers.

Something for Every Business Partner You Set the Value Prize Donation/s Sponsor provides item/s of their choosing to be given away during the dinner presentation. All sponsors will be recognized during the reception/dinner portion of the event. Get creative ~ a trophy will be awarded to the most enthusiastic and spirited sponsor. Aging Services of Minnesota Foundation 2014 Golf Fundraiser Sponsorship Application and Agreement INSTRUCTIONS: Please read the sponsorship agreement and terms. Attach payment to the completed application. Payment is due in full. Checks should be made payable to Aging Services of Minnesota Foundation. Sponsorships may be charged to MasterCard, Visa or American Express. Please return payment with application to: Aging Services of Minnesota Foundation, Attention: Bob Dahl, 2550 University Avenue West, Suite 350 South, St. Paul MN 55114-1900. SPONSORSHIP AGREEMENT: Company must be a Business Partner or member of Aging Services of Minnesota to sponsor this event. Aging Services of Minnesota Foundation reserves the right to determine whether a company meets the criteria for sponsorship determined by its Board of Directors. If they do not, application to sponsor will be denied and any prepaid fees will be returned in full. Sponsorships cannot be processed without the required remittance and must be received no later than Monday, July 7, 2014, in order to guarantee all benefits outlined. No refunds once application is accepted. All recognition of sponsorship is solely the responsibility of Aging Services of Minnesota Foundation and its planning committee. No additional signage or display by the sponsoring company is permitted unless approved by Aging Services of Minnesota Foundation in advance.
