Atherton Dance Team Tryout Packet 2015-2016

Dear prospective Dance Team member, I look forward to seeing you at the tryout. The tryout date is Tuesday May 5th, and Thursday May 7 th 2015 from 4-6:30 pm for all students who want to tryout. If there is a conflict, please notify the coach immediately. Final team selections will be made after the tryouts have concluded. During the tryout you will be taught a dance that contains aspects of Jazz, Pom, and Hip Hop. Your ability to execute jumps, leaps, turns, motions, and display flexibility while having a positive attitude, work ethic, and willingness to work with others will also be evaluated during the tryout. Dance/athletic shoes and appropriate clothing must be worn. No jewelry of any kind should be worn and hair should be pulled back. You may start signing up and turning in the required documents at 3:30 pm on the 5th. We will meet in the small gym. The completed Atherton Sports Physical Packet can be turned in to the athletic director before May 5 th for approval. No one will be allowed to take part in the tryout without all the necessary forms completed and the insurance fee paid. Atherton High School Estimated Dance Team Costs Bloomers- $10-$11 Black Jazz Shoes- $40 Tan Jazz Shoes- $40 Body Tights- $23 Jazz pants- $29 White Tennis shoes- $30 Remaining insurance fee- $15 *The team may decide to purchase team t-shirts for special events which would be approximately $10-12 a piece. * The team will attend a studio technique class once a week, cost will be determined on the number of team members. *Fundraising activities are used to help with the cost of being on the dance team. More information will be given out after the team is selected.

Atherton Dance Team General Information and Expectations Please review the information below as it deals with general expectations that Dance Team members will follow if selected for the team. The Dance Team is scheduled to practice at least three days per week for two hours. Dates and times may change due to competitions and gym availability. Dancers are expected to attend all home varsity football and basketball games. Dates will be announced as schedules are released. Dancers are expected to attend all competitions that the team is involved in. Dates will be announced as competitions become available. Dancers will be expected to replace any lost or damaged uniforms/equipment that belongs to the team. Dancers will be responsible for purchasing personal items such as shoes, tights, bloomers, shorts, t-shirts, bows, etc (personal items they will keep). Dancers must meet all academic and conduct requirements for athletes. Dancers must provide their own transportation to practices and games unless otherwise instructed by the coach. Coaches are not responsible for transportation and parents should be on time to pick up dancers from practices and events. 2014-2015 Atherton Sports Physical Package The attached package must be completed prior to conditioning or tryouts. The package should be completed, notarized (Page 6) and returned to the Athletic Director in number order. You can keep pages 4, 11, and 12. All pages must be completed and signed. Partially completed packages will not be accepted. Contact Steve Shartzer, Athletic Director (502 485-8202), with questions. Below are some helpful tips for completing: Pages 1 & 2 Physical. Must be on KHSAA form. All sections must be completed. Please make sure the doctor signs, dates and indicates his recommendations on participation in athletics (bottom of Page 2). Physicals are good for 12 months. Page 3 Jefferson County Public Schools Addendum to KHSAA Physical Form. Top two sections must be completed and initialed by parent. Bottom section must be signed by parent, student, and physician. Page 4 Part IV-Consent Information to Participate, Acknowledgment of Risk, Acknowledgment of Eligibility Rules, Liability Waiver and Consent and Release. Please read.
