Please read this packet with your parents thoroughly before you make the decision to try out. If you have any questions or concerns, please contact the coaches.

Eagle s Landing High School - Cheerleading Tryouts 2015 ALL cheerleaders will be chosen through a try-out process. Previous cheerleading experience at ELHS or any other school or gym does not guarantee a place on the squad. The Football/Competition Squad may consist of rising 9 th, 10 th, 11 th, and 12 th graders. This is one squad that will cheer football games and compete as a competition squad as well. Varsity Football squad will consist of 11 th & 12 th graders from the competition squad; the JV Football squad will consist of 9 th & 10 th graders from the competition squad. JV squad members may be invited to cheer Varsity games based on academics, behavior and skill. All decisions are at the discretion of the coach. The Varsity & JV Basketball Squads may consist of rising 9 th, 10 th, 11 th, and 12 th graders. All candidates, regardless of past experience, will be required to try out on March 13 th, 2015. Candidates may cheer on more than one squad if they tryout for more than one squad and make them. The dates and times for the ELHS Cheer Clinic for all cheerleading candidates are as follows: Monday March 9 th 4:00-6:00PM Clinic Tuesday March 10 th 4:00-6:00PM Clinic Wednesday March 11 th 4:00-6:00PM Clinic Thursday March 12 th 4:00-6:00PM Clinic Competition Candidates only Friday March 13 th 4:00-Until Tryouts All paperwork must be submitted by Wednesday, March 4 th, 2015 by 3:30pm to Coach Brown in order to participate in tryouts. All candidates must have a ride home by 6:15PM everyday of tryout week this will be noted. ALL FORMS MUST BE COMPLETED & RECEIVED BEFORE STUDENTS ARE ALLOWED TO PARTICIPATE IN TRYOUTS! All forms are included in THIS PACKET: (1) Application for Cheerleading (2) Athlete Agreement (3) Progress Report Form (4) Tryout Guidelines * must be notarized. (5) Cost Agreement* must be notarized. (6) Henry Co. Consent Form (7) Copy of your insurance card (8) Heat Policy (9) Concussion Awareness Form **Notaries can be found at most Post Offices, UPS or FedEx stores as well as some public libraries. ELHS Bookkeeper & Public Notary, Laurie Gable will be available at the February 26 th Tryout Meeting to answer any additional questions and to notarize forms. PLEASE bring your ID with you if you would like to have her notarize your forms. (10) Henry Co. Physical Exam (on official Henry Co. form no other forms excepted.) (11) $150 deposit. The following conduct is strictly prohibited while attending all phases of tryouts and /or participating on an ELHS cheer squad 1) smoking, 2) use of alcohol or drugs, 3) use of profanity, 4) public display of affection 5) unsportsmanlike conduct, 6) undisciplined behavior and 7) fighting. Social media and technology is included in all situations listed above. This behavior warrants immediate dismissal.

Eagle s Landing High School - Cheerleading Tryouts 2015 ELIGIBILITY FOR TRY-OUTS Candidates trying out for cheerleading must meet the requirements below NO EXCEPTIONS. Must be in good academic standing (not failing more than 1 class). Progress Report form will need to be completed by all teachers and will need to be turned in with the cheer packet. All parts of the cheer packet must be turned in and properly notarized. Must not have any money owed from previous years of athletics at ELHS (or other schools), nor have any rented uniforms in your possession. TRY-OUT DAY Appropriate dress code must be followed (blue or black shorts, plain white t-shirt, socks, tennis shoes). No tank tops or spaghetti straps will be allowed. YOUR BRA SHOULD NOT BE VISIBLE. No clothing signifying any prior cheerleading experience will be allowed. No jewelry, nail polish or false nails. Hair should be in a secure ponytail with no strands or bangs in the face. Failure to follow dress code = inability to participate in tryouts & you will be asked to leave. Try-outs are closed. During try-outs, the try-out area will be restricted to candidates, judges, tabulators, and any other school personnel whom the coaches or administration feels are needed for tryouts to be properly conducted. Siblings/Cousins/Boyfriends/Aunts, etc. will not be allowed to stay. Video recording of the try-outs will not be allowed. Results will be announced on the school website within a week of the day of tryouts. All decisions made by the judges/coaches/athletic director/administration are final. Categories to be judged: Official Scoring rubric & score sheets can be seen on ELHS cheer website. o Cheer o Jumps o Dance o Stunting (Competition only) o Tumbling (Competition only) Cheerleading is a demanding sport. An individual must be prepared to devote time in the summer, after school, and on weekends for practices, fundraisers, games and competitions. SUMMER PRACTICE Football/Competition Cheerleaders will be required to attend choreography camp during the summer at ELHS from Monday, June 8 th to Friday, Junes 12 th, 2015. This is MANDATORY. Additionally, the summer practice schedule can also be found on the ELHS Cheer website. Please see the calendar before planning summer vacations. We (coaches) have done our best to create long weekends, and no practice weeks in order to provide vacation breaks. COMMITMENT Work and other school-related activities you decide to participate in after making the Squad should not interfere with practices or games. Eagle s Landing Cheerleading must take precedence over any extracurricular activities or work. It is your responsibility to dedicate the necessary time to the Squad. Failure to do so could result in your removal of position on the squad. Again, WORK is not an excuse. MANDATORY FITTING: March 26th, 2015 from 3:45-5:45PM @ ELHS for ALL cheerleaders. MANDATORY PARENT MEETING: March 26th, 2015 ALL cheerleaders and a parent/guardian will be required to attend a meeting at 6:30PM @ ELHS. The second payment must be made at this time. At the meeting, the cheerleading behavioral contract will be discussed. Parents and cheerleaders will be asked to sign a statement indicating that they have read and agree to abide by the rules. Information on the fundraisers for the new season will also be provided. Cheerleaders WILL NOT be allowed to participate until the contract has been signed by cheerleader AND a parent/guardian.

Eagle s Landing High School Cheerleading Tryouts 2015 Athlete Agreement I, (student), have read all of the information regarding ELHS cheerleading. I understand that being a part of a cheerleading squad is a commitment. If chosen for a squad I agree to: Be on time and attend all practices, all required athletic events, all fundraisers and all competitions. If there is any reason that I will be late or that I am unable to attend any cheerleading event then: I (or my parents) must contact my coach directly via phone/text/email in advance or I will be unable to participate in that event and/or future events. If there is a medical emergency, then: I (or my parents) will contact my coaches directly via phone. I will provide written notification for my absence and/or injury from a doctor. I will always be prepared for practices and athletic events: By removing all jewelry, pulling my hair completely back and out of my face, ensuring no nail polish or false nails, wearing appropriate clothing (including shoes and sports bras) and bringing any additional equipment needed (poms, etc.). As I am a representative of Eagle s Landing High School, I will: Always show good sportsmanship and respect towards my coach, my teammates, judges, coaches of other teams, members of opposing teams, and most importantly myself whether in person or electronically (Including social media not limited to Twitter, Instagram, Tumblr, Snapchat, etc.) Refrain from smoking, using alcohol or drugs, using profanity, public displays of affection, fighting, and any other actions that are deemed inappropriate by the coaches/faculty/administration. I understand that if I display any of the behaviors listed above then I will receive consequences stated in the 2015-2016 ELHS Cheerleading Contract and/or lose my place on the squad. I understand that my role on the squad is one of leadership, both on and off the mat/track/court, both in and out of the classroom, and both on and off social media. I acknowledge that even with the best observance of the rules, injuries are still a possibility. On rare occasions, these injuries can be so severe as to result in total disability, paralysis, or even death. I acknowledge that I have read and understand the warning presented herein. I agree not to hold the school or anyone acting in its behalf responsible for any injury occurring to my child in the proper course of such athletic activities. Eagle s Landing High School Cheerleading Tryouts 2015 ELHS Cheerleading Cost Agreement *This form must be notarized* The cost of cheerleading will be handled through a 7-step payment plan. If your child is selected to be a part of the 2015-2016 squads, you will be responsible for all payments being made in full and on time. The final payment will be determined from the balance owed based on the equipment ordered for your child. Your child will be given a monthly statement with payments made and monies owed. If payments are not made on time then a cheerleader s equipment will not be ordered thus preventing them from participating in practices and/or games/competitions until full payment is received and equipment is ordered.
