COUGAR STUDENT ATHLETIC CODE OF CONDUCT Student Athletic Code Review Dear Parent/Guardian(s): Please review the Rochester Community Schools Student Athlete Code of Conduct and then sign the form below. The Student Athlete Code of Conduct is in your student s daily planner and on the RCS website at www.rochester.k12.mi.us. Your student s coach has read and discussed this code with all the members of the team. PHYSICAL EXAMS THIS SUMMER 2017-2018 PHYSICALS Middle and High School Students Interested in Athletics, Get Your Physical Here! Athletic physicals for the 2017-18 school year are once again being offered in June. All athletes must have a physical exam dated after April 15th, 2017 before they can tryout or practice for a sport next year. Exam dates are provided at Stoney Creek and Adams High Schools: Tuesday, June 20 @ Stoney Creek (5:00 8:30 pm) Wednesday, June 21 @ Adams (5:00 8:30 pm) Please call 248-726-3138 to schedule an appointment at either of the High Schools.
