ISD 15 has asked NeoPath to evaluate different options for how we can improve clinic accessibility, enhance the patient experience, boost utilization of the ISD 15 clinic and maintain this highly valuable service to the community. This document highlights a growing trend in Primary Care, a membership based service option with detail of the services provided, member experience and associated membership fees: Independent School District 15 is nestled in the woods and fields of northern Anoka County and southern Isanti County. This 165 square mile oasis of fresh air, clean water and livable space focuses on a quality of life that the residents feel is second to none, yet is a convenient drive from downtown Minneapolis or St. Paul. Membership Fees Single Member: $59 per month Family Membership: $99 per month Working in concert with your employer, Independent School District 15, the associated fees will be payroll deducted to enhance member experience. The school district annually educates more than 5,000 students and employs more than 800 staff members.

Lisa Ocker is a Family Nurse Practitioner, board-certified by the American Academy Nurse Credentialing Center, and licensed by the state of Minnesota to provide family-centered healthcare to patients of all ages. Her scope of practice encompasses health promotion, disease prevention, diagnosis and management of common and complex health care beginning in childhood and continuing throughout the aging process. Lisa began her career in health care as a Registered Nurse working in home care and hospice, school nursing and for county correctional facilities. Her broad range of nursing experience has prepared her to care for patients across the age spectrum. Lisa received her Master of Science in Nursing from the College of St. Scholastica, Duluth in 2011 and became a certified Family Nurse Practitioner. Since that time, Lisa has worked with patients in both convenience care and long term care facilities. Lisa has a unique combination of strong nursing skills and strives to provide the best health care possible. She believes in building a trusting relationship with patients. We empower individuals by placing them at the center of a healthcare network and giving them the tools they need to make healthier decisions.
