Bainbridge Island Lacrosse Puget Sound Youth League Summer 2013 --Grades 6/7/8 The Bainbridge Island PSYL Summer Program for one 6/7/8 team for 2013: Summer lacrosse is perfect for keeping your skills sharp or introducing you to the game before the regular season arrives in the spring. PSYL teams are run by top-notch coaches and practice on Mondays and Wednesdays so it does not interfere with other sports. Bainbridge Island is proud to be a participant in the Summer 6/7/8 2013 PYSL Lacrosse Program. BI PYSL will field a Middle School team (6 th / 7 th /8 th ) comprised of 20 players for the Summer Season (Jun-Aug). The team will participate in Jamborees against the other participating PYSL programs. Emphasis is on developing skills to prepare players for the coming Spring Season 2014. Practices and Games: Practices will be held twice weekly, Mondays and Wednesdays from 6/24/13 8/21/13 from 6:00 pm 8:00 pm. Players will be responsible for their own travel arrangements to and from practice and jamborees. Games will be finalized in the coming weeks. Cost: $250.00 Bainbridge Island PYSL (SUMMER Season) Fee Breakout: $250.00 Fee Registration, PSYL shorts, all local games and tournament fees, Coaches stipends, field time and referee costs. Does not include individual travel. Expectations of Player of Conduct The Expectations of Conduct shall serve as a guideline for how individuals are expected to behave when participating in BI PSYL Sports or activities. BI PSYL players should represent their club in a way that does not detract from the reputation of the PSYL organization they are representing, and to act morally and display sportsman-like behavior before, during, and after any competition. Guidelines of conduct include, but are not limited to the following: Participants Shall Not: - Use drugs, except for medical purposes, while traveling, competing, socializing and/or using facilities spaces. - Consume alcohol while traveling (from the time a team leaves to the time it returns), competing or spectating. - Strike, attempt to strike, or otherwise physically abuse an official, opposing player, spectator or coach. - Violate any host facility rules or policies. Violations of the Standards of Conduct may result in: - Forfeiture of all BI PSYL team travel rights for the Summer Season Team Responsibilities will include: - Educate club members of the Standards of Conduct and consequences of misconduct. - Ensure that all team members adhere to the Standards of Conduct. BAINBRIDGE ISLAND PSYL (Puget Sound Youth Lacrosse) RELEASE AND WAIVER OF LIABILITY, ASSUMPTION OF RISK AND INDEMNITY AGREEMENT (Parental Consent for Minor) I acknowledge that I have enrolled my child, in a lacrosse program promoted by PSYL and I agree to this release and waiver of liability in consideration of the opportunity for my child to participate in lacrosse. ACKNOWLEDGMENTS: My child and I understand that lacrosse: 1. Involves playing an aggressive contact sport with others. 2. The sport inherently involves a high degree of risk that could result in serious bodily injury or death to my child or to other participants. AGREEMENT: By enrolling my child in the lacrosse program, I certify that my child and I are cognizant of all of the inherent dangers of participating in lacrosse. I further certify that my child is familiar with the rules of the game and of the field and will to the best of their ability play under control and avoid injury to themselves and other persons playing the game. I understand that my child should not participate in this program if he/she is under the influence of drugs or alcohol or if there are any other physical conditions that may impair his/her ability to understand instructions or to participate without creating risk to him/herself or others. RELEASE AND INDEMNIFICATION: For and in consideration of the right for my child to participate in lacrosse, I hereby assume all risks associated with lacrosse, including the negligence of PSYL and will hold PSYL and its sponsors, subcontractors, directors, officers, employees, agents, and/or instructors harmless from any and all liability, actions, causes of action, debts, claims, demands of every kind and nature whatsoever which may arise out of or in connection with child s participation in any activities arranged for my child by PSYL. I accept full responsibility for the cost of treatment for any injury suffered by my child while taking part in the program. The terms hereof shall serve as a release and assumption of risk for my heirs, executor and administrators and for all members of my family including any minors accompanying me. I further state that I am of lawful age and legally competent to agree to this affirmation and release: that I understand the terms herein are contractual and not a mere recital. I have agreed to this document as my own free act and if I have any doubts concerning the contents of this release agreement, I will consult an attorney before agree to it. If I am caught using or under the influence of alcohol or narcotics other than those prescribed by a doctor, I may be sent home immediately from practice or a jamboree at my parent s expense and/or kicked out of the program. 2. I will always follow the basic rules of Fair Play. I will respect the rules of the game, my opponents and the officials and their decisions. Everyone on the bench will participate and I personally will maintain my self-control at all times. 3. On game day, all players shall arrive at least 45 minutes prior to game time for warm-ups. Players shall be fully dressed and ready to go at least 30 minutes prior to game time. This period will be necessary to discuss team strategies and game plans. If you require more time to dress, come earlier. 4. On practice day, players shall arrive at least 20 minutes prior to practice start time. 5. Players shall participate in every game and every practice except for illness, injury or school events. If a player must miss a game or practice, he/she must contact the coach as soon as possible. 6. School shall take precedence over Lacrosse. 7. Foul language towards anyone, be it teammates; coaches; officials; opponents or spectators, will not be tolerated. 8. Players shall not be critical of teammates and never discuss teammate s abilities except to encourage good team play. 9. Hot-dogging will not be tolerated. 10. Players are responsible for their own equipment and thus they must ensure that it is complete, safe, and in good condition at all times. 11. Lacrosse is a team sport. I am on a team and I will be a team player. 12. I will respect all my teammates, the officials and my competitors. 13. I will always listen to my coach s instructions. 14.
