Automotive Safety Checklist Facility: Address: Date: Initials: General Is your Accident Prevention Program implemented and personalized to your facility? Is the Accident Prevention Program accessible to all employees? Can they locate it? Has the Safety Orientation been documented and kept on file for all employees? Are Safety Committee/Meetings being done quarterly/monthly? Is there a designated safety bulletin board? (Exclusively if you have 8 or more employees.) Is there a First Aid card holder on site from opening of business to closing of business? Is a portable first aid kit and infection control kit in place? Do employees have access to it? Are fire extinguishers inspected monthly (gauge, seal, hose) by GM or safety committee? (Initial and date back card.) Is there a bio-hazardous waste container on site? Are all MSDS (Material Safety Data Sheets) on site for all hazardous chemicals? Is an evacuation map posted on the safety bulletin board? (Identify all fire extinguishers, first aid kits, exits, and assembly areas.) Hazardous Communications Are all containers properly labeled? (It must be legible with product name.) Are hazardous chemicals not being stored in drinkable containers? Are all MSDS sheets accessible and can the employees locate and read them? If an eye wash station is required is it accessible? (Within 50 feet or 10 seconds reach and not behind a door that can be locked. Eye wash station must have 15 minutes of running water. Review the MSDS of the chemical in question.) Have the employees gone through training on Hazardous Communications? (Documented) Exterior Are there fire extinguishers outside? (if so are they getting monthly inspection?) Do employees find needles or drug paraphernalia in the parking area? (If so is proper training and PPE (Personal Protective Equipment) worn?) Is the parking lot, side walks clear and in good condition? Is the entrance, exit walkways, steps and ramps clean and marked? Do employee areas have proper lighting? (Parking lot, garages, entrances, etc ) Is the dumpster area properly maintained?

In the dumpster area is there clear access to areas and clean ground? Do all exterior equipment and stairways have protective rails and proper illumination? If there is inclement weather (snow/ice), is there rock salt or procedures in place to deal with slip & fall hazards? Show Room/Administration Is the customer entrance flooring area clean of potential hazards? (Ex: Wet floor signs placed when needed.) Are there extension cords that go from one area to another through a whole in the wall, door way or through a window? Are "Wet Floor" cones clean and in use if needed? Is there a natural transition from carpet to tile? (No potential trip and fall hazards.) Are "Exit" doors blocked? (Must be clear at all times.) Service Bay Personal Protective Equipment (PPE) Have the employees been trained on PPE and have them accessible? (Safety Glasses, Hearing Protection, etc ) When the employees are operating below vehicles are they wearing their safety glasses? (Prescription eye wear does not count if they do not have side shields or if they are not defined as Safety Glasses.) Are all employees wearing safety shoes? (if not, who?) Tools Is the right tool being used for the right job? Are there any hammers that have cracked handles or mushroomed heads? Is the hydraulic press and drill press stationary? (Earthquake Proof) On all the vehicle jacks (i.e. spin jacks, lift jacks, vehicle lifts, etc ) post a maximum weight capacity?

Do the brake lathes have their chip guards in place? Does the bench grinder have all its guards in place? (Tongue must be 1/4" or less to the stone, Tool rest must be 1/8" or less to the stone.) Is equipment disconnected from power source when being stored or cleaned? Are all vehicle lifts being maintained for maintenance? (Documentation is on file to owner s manual specifications.) Are air hoses and extension cords lying in the aisle way unattended? Is the ladder properly stored? (Secured to the wall or lay on its side out of the aisle way.) Is all compressed gas cylinders secured? Are oxygen and acetylene being stored together? (If not in use, this is not permitted. They must be separated by a minimum of 25 feet or more.) Is there a fire extinguisher on the oxygen and acetylene tank cart? On the solvent tank is the melting fuse link on the back of the panel in place? Exposed bulbs below 8 feet guarded? (Drop lights and fluorescent lights?) Do air blowers have a relief valve if ever dead ended against the skin? (If not psi cannot exceed past 30.) Are respirators being stored in an air tight container when not in use? Service Bay Are exhaust hoses being used when vehicles are running? Have the employees been trained on the hazards of Carbon Monoxide? Are the exhaust hose ports closed when not in use or when left unattended? Aisle ways clear at all times? (No curled air hoses, cords, or spills?) Have the employees been trained on proper lifting procedures? Are there extension cords that go from one area to another through a whole in the wall, door way or through a window? Are "Exit" doors blocked? (Must be clear at all times.)

Parts Department Are exposed bulbs below 8 feet guarded? Is there a maximum weight capacity posted on all industrial shelves? If there is an overhead storage area is the maximum weight capacity per square foot posted? Have the employees been trained on proper lifting procedures? Is the stair way/stair well free of debris? Are "Exit" doors blocked? (Exits must be clear at all times.) Are there extension cords that go from one area to another through a whole in the wall, door way or through a window? Paint Department Are MSDS sheets available for all hazardous chemicals? Have the employees gone through the Respirator Program? Are you documenting filter exchange for the paint booths and all respirators? Are all containers properly labeled? Are all flammable containers grounded? Are there extension cords that go from one area to another through a whole in the wall, door way or through a window? Are "Exit" doors blocked? (Exits must be clear at all times.)

Industry Specific Safety Requirements (If applicable) Forklift Has your fork lift program been implemented? (Employee trained and documentation kept on file.) If propane, is a fire extinguisher installed on the forklift if it goes outside? Is the forklift left running unattended? (Even the forks must be flat on the ground.) Is the operator running the forklift at a safe speed? Is communication in place? (Honking the horn, back up signals, etc ) Asbestos Program Has your asbestos training been implemented? Are you performing annual training for your techs on asbestos? (Is it documented?) Respirator Program Have your employees who wear respirators performed their medical examination? (Is it documented?) Has a fit test been performed with the respirator they are going to use? Are they documenting the times the respirator is in use? When the respirator is not worn, are they storing the respirator in an air tight container? Are your employee s clean shaven? (Facial hair is not to impede with the seal of the respirator.) Is the correct respirator being used for the job? Is eye protection being worn when spraying? (I.e. chemical goggles) Lock Out/Tag Out Has your Lock Out/Tag Out been implemented? Has the employee been trained on Lock Out/Tag Out? Does the employee trained on Lock Out/Tag Out have their own lock and key along with their own identification tag?
