EXHIBT A - BID FORM SUNSET VALLEY GOLF COURSE I. CONTRACTOR MOBILIZATION Golf Course Master Plan Implementation Date: December 1, 2016 Includes the contractor mobilizing on-site with equipment, materials and personnel required for construction of the project. (GHA) Plans and Specifications. NOTE: Bidders shall use the GHA materials form to fill in final quantities and unit prices and then transfer the total cost to this line item. E & S CONTROL TOTAL....$ III. LAYOUT AND STAKING A. Layout and Staking - Includes the layout and staking of all proposed golf course features and all grade elevations required for the construction of the golf course features. The Owner will provide control points (elevation) within the golf course boundary. The Owner will stake utilities, wetland delineations, easements, property lines, gas lines and right-of-ways. Existing Turf Removal (roto-till) - Includes the removal of existing turf in all areas within the limits of disturbance in proposed grading locations and all fairways. 1

The turf will be roto-tilled with a rotadairon into the topsoil after an herbicide application by the Owner in the limits of disturbance areas delineated by the Contractor. SF Turf @ $. /SF..$ B. Existing Turf Removal (sod strip/bury) - Includes the stripping of sod on all existing tees that are to have surfaces re-leveled and/or expanded and re-seeded. Sod shall be buried in non-play areas and covered with a minimum of 24 of clean fill and 4 of topsoil. SF Turf @ $. /SF..$ C. Existing Bunker Sand Removal (by Owner) - Includes the removal of bunker sand by Owner (Note: assume an existing depth of 4.0 ), transporting, topdressing rough areas and/or burying in non-play fill areas on-site as designated by the Owner. Bury areas shall be within 2,500 LF of the bunker and the sand shall be capped with a minimum of 24 of clean fill material and 4 of topsoil. 0 SF Bunkers @ $0/SF. $0 Note: Tree clearing shall be done by Owner. Contractor shall supply a unit price for additional selective tree removal in the Bid Alternates. TOTAL SITE PREPARATION $ V. TOPSOIL MANAGEMENT To include the stripping, stockpiling and re-spreading of on-site topsoil to a minimum compacted depth of 4" in all areas to be graded with the exception of tee and green surfaces, sand bunker sub-bases and cart path sub-bases. Topsoil quantity shall also include topsoiling locations where existing cart path is being removed as delineated on the plans. Note: assume a 6 depth of topsoil to be placed in locations of existing cart path removal. CY On-Site Topsoil @$ /CY $ TOTAL TOPSOIL MANAGEMENT.$ VI. EARTHWORK To include the excavation, loading, hauling, placement and compaction of material as shown on the Plans and Requirements set forth in the Specifications. The quantity should NOT include the cut-to-fill material that remains within the same disturbed area. This cut-to-fill material is included in Rough Shaping. Contractor is responsible for all dewatering of the lakes required for the excavation of lake expansion areas. Note: As a 2

ROUGH SHAPING Rough Shaping includes the grading of the cut/fill material placed during the earthwork process as well as the cut-to-fills that are within the designated areas of disturbance for bunker, tee, green and detention area construction. All carry off lines from golf course features shall be solid pipe and connect with the storm drainage system (designed by GHA) or exit into the existing river or existing on-site lakes. Note: 4 perforated pipe quantity includes all green and bunker subsurface drainage. The Owner shall remove existing irrigation heads that are to be preserved and then reinstalled by the Contractor per Irrigation Designer s Specifications. NOTE: Contractors shall use the Irrigation Designer s materials forms to fill in final quantities and unit prices and then transfer the total cost to this Line Item. GOLF COURSE IRRIGATION TOTAL...$ X. FEATURE CONSTRUCTION A. Greens Construction - Includes the construction of eighteen (18) new greens and new putting green per the USGA recommendations for a method of putting green construction. The proposed green sizes include the green collar. SF greens @ $ /SF $ B. New Tee / Existing Tee Expansion Construction - To include the subgrade finish preparation and the installation of USGA approved 7:2:1 blend to a minimum compacted depth of 4" per the Plans and Specifications. C. Existing Tee Surface Leveling - To include the leveling of existing tee surfaces with existing tee mix and alignment of the tee surface per grading plans. SF tees @ $ SF $ D. Bunker Development - Includes the construction of new and renovation of existing fairway and greenside bunkers per Plans and Specifications. The bunkers shall be cored and compacted in preparation for the installation of 4" perforated drainage pipe and the installation of a minimum of 4" of compacted Antioch Root Zone bunker sand. NOTE: Drainage for all new bunker development is included in Section VIII - Golf Course Feature Drainage. SF Bunkers @ $ / SF.$ E. Bunker Liner Installation Includes the installation of Sandtrapper II Erosion Control Blanket on the faces (e.g. slopes 5:1 or greater) of all new bunkers. The bunker liner shall be installed per Manufacturer s recommendations. Note: For bidding purposes assume that bunker liner will be installed in 50% of the bunker sub base. SF Bunkers @ $ / SF.$ F. Lake Bank Erosion Control Includes the material, equipment and labor required to install Pec-Mat Erosion Control per Manufacturer s recommendations. Pec-Mat shall be installed on all lake banks that currently have rip-rap erosion control. The cost shall include removal of existing rip rap and burying in non-play areas or depositing in the bottom of lake expansion areas per specifications. Note: A Bid Alternate deduct is included in the Bid Alternates Form for Pec-Mat installation only on lake banks of new lake expansion areas. SF Pec-Mat @ $ / SF.$ FEATURE CONSTRUCTION TOTAL.$ XI. FINE SHAPING / SEEDBED PREPARATION Includes the blending of all slopes, fine shaping and in general, puts the golf course holes to a finished grade in preparation for seeding. Fine shaping includes all disking, box blading, raking, scalping existing turf, tilling, verticutting, core aerification etc. required for seedbed preparation per Specifications. Note: As a requirement for permit compliance, the Contractor must follow the process for grading and earthwork approval prior to seeding as outlined in the specifications. Includes the material, equipment and labor required for the application of all seed, sod and erosion control. Note: Pre-plant fertilization by Owner. A. Greens - Includes the seeding of all greens with Pure Distinction bentgrass seed planted at a rate of 1.5 pounds per 1,000 square feet. SF Greens @ $ /SF..$ B. Tees - Includes the seeding of all teeing surfaces with Crystal Blue Links bentgrass seed planted at a rate of 1.5 pounds per 1,000 square feet. SF Tees @ $ / SF $ C. Roughs - To include the seeding of all rough areas, with the exception of greens, tees, bunkers, turf nursery, wet meadow, wetland areas and cart paths with a rough seed blend of 95% RTF fescue / 5% HGT bluegrass planted at a rate of 240 pounds per acre. Bluegrass Sod - To include the sodding of areas designated on-site by Owner or Designer that immediately surround the proposed greens, tees and bunkers per plans and specifications. Fescue Sod (Bunkers) - To include the sodding of a 36 width around the perimeter of all bunkers per plans and specifications. The Tall Fescue shall be certified RTF sod (Payne Sod Farm) and approved by the Owner. 30,000 SF Sod Allowance @ $ / SF.$ G. Wet Meadow Area Seeding includes seeding rates and seed blend per the Technical Specifications. SF Wet Meadow @$ /SF $ H. Wetland Plug Planting The work consists of furnishing all materials, labor and equipment required for the planting of the wetland areas per specifications and as 6

delineated on the grassing plan. Contractor is responsible for all existing lake dewatering for the planting of plugs on the wetland shelves. Fescue Seeding Includes the seeding of graded areas with a Jacklin Seed fescue blend of 1/3 Ecostar / 1/3 Marco Polo / 1/3 Rescue 911 at a rate of 3 pounds per 1,000 square feet within the limits of disturbance per the grassing plan. Note: The Owner shall seed all fescue conversion areas in undisturbed rough locations. SF Seed @ $ /SF..$ J. Hydro Mulching All rough areas that are to be seeded, with the exception of greens, tees, turf nursery, wet meadow, wetland areas and cart paths shall receive Profile Hydraulic Mulch (HM) Wood with Tack at a rate of 2,500 lbs per acre. Existing Cart Path Removal - Includes the materials, equipment and labor to remove existing asphalt cart paths per plans and specifications. Existing asphalt shall be disposed of in accordance with local ordinances. SF of Existing Cart Path to be removed @ $ / SF $ B. New Path Installation - Includes the materials, equipment and labor to install the asphalt cart paths per GHA approved Construction Details and Specifications. Cart path installation shall include coring, sub-base compaction, 4 compacted aggregate 7

base (12 wider than cart path width dimension) and 2 asphalt surface. Note: All cart paths shall be 8 wide unless otherwise noted on the plans. SF New Cart Path @ $ /SF...$ C. New Path Installation Over Existing Gravel Path - Includes the materials, equipment and labor to install the asphalt cart paths per GHA approved Construction Details and Specifications. Cart path installation over the existing gravel path (#10 tee to #17 tee) shall include leveling and topdressing aggregate base as needed to produce a 4 compacted aggregate base (12 wider than cart path width dimension) and 2 asphalt surface. SUNSET VALLEY GOLF COURSE BID ALTERNATES Date; December 1, 2016 The following Bid Alternate items are required to be completed for the Bid to be considered by the Owner. The Owner reserves the right to reject any Bid if the Bid Alternates are not deemed to be complete. The Contractor must complete any Owner selected Bid Alternates within the specified project schedule timeframe. A. Bentonite Lake Sealant In lieu of on-site clay lining, provide a bid alternate for sealing the lake expansion areas with Bentonite incorporated into the soil at a rate of 2 pounds per square foot and compacted per manufacturer s recommendations.
