2018 Special Olympics Ohio Annual Awards Banquet Nomination Form Ohio Special Olympics Ohio (SOOH) would like to recognize and show appreciation for the exceptional work performed by Athletes, Coaches, Businesses, Families, and Volunteers throughout the year. These individuals are dedicated to fulfilling the mission of Special Olympics and deserve to be awarded for their outstanding achievements. Any and all supporters of SOOH are encouraged to fill out the following nomination form to honor these individuals. Award winners will be selected by the Awards Committee which is composed of representatives from the state office, board members, and athletes. All award winners will be recognized during the SOOH Awards Banquet at the Fall Leadership Conference on October 26 th. Award Categories: Athlete of the Year: Presented to an athlete who demonstrates commitment, a positive attitude, and exceptional sportsmanship during his/her participation with Special Olympics Ohio. This athlete embodies the mission of Special Olympics and is a role model to his/her peers. Nominees must have participated in at least one competition during the year. Coach of the Year: Presented to a coach who displays a high degree of technical skill, personal commitment to the development of SOOH athletes, and passion for the organization. The Special Olympics coach must be up-to-date with all trainings and certifications and completed one season of coaching. Volunteer of the Year: Presented to a volunteer that provides the highest level of quality voluntary actions that contribute to providing athletes with an extraordinary experience during state or local level competitions. Members of local groups and Sport Directors may be nominated. Family of the Year: Presented to a family who shows continued support to Special Olympics Ohio and are actively involved with making lasting contributions that will benefit the lives of all athletes. Outstanding Support by a Business: Presented to a business whose commitment and support to the mission of Special Olympics Ohio has positively contributed to the success of the organization. Spirit of Special Olympic Award: Presented to any stakeholder of Special Olympics Ohio (athlete, coach, volunteer, family, business) who displays the true spirit of courage, joy, sharing, and friendship in all aspects of their lives.
