TURN-AROUND JUMPERS: RELEASE, WAIVER AND ASSUMPTION OF RISK This form must be signed by all players and returned with registration forms. Instructions: Please have each team member read this page and sign the line on the application form stating they have read, understand, and agree to abide by the terms in this agreement. If a team member is under the age of 18, each of his/her parent(s) or guardian(s)should also sign. I desire to participate in the Turn-Around Jumpers 3v3 Basketball Tournament in Syracuse, NY on June 8-10, 2012 (the Tournament ). I affirm that I am voluntarily participating in the Tournament, am fully capable of participating in the Tournament, am in adequate physical condition to participate in the Tournament, and have appropriate medical insurance in the event that medical attention is needed. I understand that physical activity and participation in basketball and other Tournament activities, by their nature, carry with them certain risks that cannot be eliminated regardless of the care taken to avoid injuries. These risks can come from causes which are many and varied, may not even be presently foreseeable, and may include negligent acts or omissions of others. The specific risks may vary from minor injuries such as bruises and sprains, to major injuries such as joint or back injuries, concussions and heart attacks, to catastrophic injuries such as paralysis and death. In consideration of the permission granted to me to participate in the Tournament, I acknowledge, accept, and assume all risks of such participation, whether or not presently foreseeable and whether or not caused by the negligent acts or omissions of the Released Parties (as defined below) or others. I irrevocably and unconditionally release and forever discharge The Salvation Army (including all of its affiliated entities), Le Moyne College, The Hayner Hoyt Corporation, any other sponsors of the Tournament, and the respective officers, directors, trustees, agents, employees, representatives, and successors of any of the foregoing (collectively, the Released Parties ), from any and all claims I may have in the future, waive all such claims, and agree not to sue any or all of the Released Parties for any such claims, arising out of such my participation in the Tournament, including but not limited to claims arising out of the negligent acts or omissions of any or all of the Released Parties or others. I agree to indemnify and hold harmless the Released Parties from and against any loss, damage or expense sustained or incurred by any or all of them arising from any such claims, causes of action or liability, whether asserted by or on behalf of me, or by anyone else because of conduct attributed to me. I have read, understand, and agree to abide by the terms of this Agreement. I understand and agree that this Agreement is to be as broad and inclusive as is permitted by the laws of the State of New York, and that if any portion of this Agreement is held invalid, the remaining terms shall continue in full force and effect. This Agreement shall be binding upon me, as well as my successors, executors, personal representatives, heirs and assigns. If the participant is under the age of 18, each of his or her parents or guardians must also sign the registration form. The signature of a parent or guardian shall constitute the parent or guardian s agreement to the terms of this Agreement on behalf of the participant, as well as his or her agreement to the waiver, release of liability and assumption of risk provisions set forth above with respect to any rights he or she may have or subsequently acquire as a result of the participant s participation. By his or her signature, each parent or guardian also agrees to indemnify and hold harmless the Released Parties from and against any loss, damage or expense sustained or incurred by them arising from any claim, cause of action or liability which may be asserted by or on behalf of the participant, or by anyone else because of conduct attributed to the participant.

Official Rules General 1. Each team will have 4 players with the registration information provided to The Salvation Army by the entry deadline, May 25, 2012. No substitutions will be made for players after game play begins. 2. Players will not be allowed to participate on more than one team. Any player registered on two different teams will have to pick one team to play on before the tournament begins, otherwise that player will be disqualified from the tournament all together. 3. Teams are placed within divisions based on the experience of the players on the team. All brackets will be based on the age and gender of the teams. There are no co-ed brackets. 4. Wrist bands will be issued to every player; if the wrist band is removed the player can no longer participate. 5. No refunds will be given at any time. Scoring, Live/Dead Ball and Possession 1. 1 point will be awarded for each basket made. 1 point will be awarded for each free throw. Two points will be awarded for each basket made behind the 3-point line. Both feet must be behind the line for the full 2 points to be awarded. 2. A coin toss will determine the initial possession. Whoever wins the coin toss gets the ball first and the alternating possession process begins. If the game goes in to overtime, there will be another coin toss with the winner getting first possession and the alternating possession process begins again. 3. The Ball will be taken behind the 3-point line on all changes of possession. Failure to take it back will result in loss of possession and any points resulting for that possession. 4. The defensive player or game referee must check the ball before it is put into play. This will be decided at the beginning of the game. For play to begin, the offense team passing the ball in must be behind the 3-point line and must pass the ball in to a teammate. 5. The first team to score 15 points wins the game. To be declared the winner before the time limit is reached, the winning team must have a two point lead. If game reaches the 15 minute mark before a winner is declared, then the game goes into sudden death overtime - next team to score wins. Time 1. All games will have a 15-minute time limit. There will be a continuously running clock. 2. If a team does not have at least 3 players ready to play within 5 minutes after their official start time they will forfeit the game. 3. Both teams will have 3 minutes prior to the beginning of the game to warm up and to have at least 3 players on the court ready to play. If, when the game begins, there are not 3 eligible players on the court, the team will forfeit. 4. Player substitution is permitted during any dead ball situation. The substitute player is required to check in at the scorer s table before entering the game. Court The top, sides and bottom of the backboard are inbounds. The metal support pieces and base unit of the backboard are out of bounds. Stepping on the baseline, sidelines and mid-court line is out of bounds. Referees 1. Treat referees with respect. Being less than cordial to the referees, scorekeepers, opponents and/or tournament officials will result in being suspended from the game or the tournament. Non-playing coaches/parents are not recognized as part of the team and will be asked to leave the event site if they become rude or unruly. 2. Team captains can ask the referee/division head to explain any rule. If there is a dispute between teams and referees, the division head will make the final, irrevocable ruling. Once play resumes after a disagreement, the problem is considered resolved. Fouls 1. After each non-shooting foul, the team awarded the ball will take possession of the ball behind the 3-point line. 2. Non-Shooting Fouls 1 6: No free throws will be awarded but ball must be taken out to mid-court. Offense retains possession on defensive fouls. Defense gains possession on offensive fouls. 7+: Will be treated as shooting fouls. See rule # 3 below. 3. For every shooting foul, the shooter will have the choice to shoot a regular free throw or a 2 point shot from behind the 3 point line. If the offensive team misses the regular free throw, the offensive team would retain the ball. On a make, the defense gets possession. If the offensive team chooses the 2 point shot, possession is forfeited on a make or miss and the defense gets the ball. 4. Shooting Fouls with Basket Made (1 or 2 point shots) Basket will count and one free throw will be awarded to the fouled player. On a make or a miss, change possession. 5. Flagrant fouls will result in 2 free throws and possession of the ball. Only normal free throws will be awarded. 6. After a technical foul, 2 free throws and resumption of play at the point of interruption. If a player receives 2 technical fouls, they are suspended from the present game and the next game as well. If a player receives 3 technical fouls during the tournament, that player will be ejected from the tournament.
